INTRODUCTION: Displaced fractures of the base of the proximal phalanx (PP) remain a challenging surgical problem. Regardless of the osteosynthesis material chosen (K-wires, screws, plates), when open reduction is required, the surgical approaches typically used are either dorsal or lateral. These approaches inevitably cause surgical trauma to the extensor apparatus which, via the intrinsic system, envelops the dorsal and lateral surfaces of PP at this level. To preserve the extensor apparatus, the authors propose a palmar approach combined with anterior plate fixation.

PURPOSE: Since 2006, we have been performing minimally invasive plate osteosynthesis with a palmar locking plate and without division of the pronator quadratus muscle for repairing distal radial fractures. A case is reported of a young girl who presented with macrodactyly of the right middle finger and tumour masses on the palmar side of both interphalangeal joints. The lesions were fibrocartilaginous and appeared to be hyperplastic palmar plates. The macrodactyly and the hyperplasia of the palmar plates were attributed to trauma.

This study compared functional and radiological outcomes after treatment of extension-type distal radius fractures with conventional titanium nonlocking T-plates or titanium 1.5-mm locking plates. A 63-year-old woman was unable to actively flex the interphalangeal joint of her right thumb after a hyperextension injury. Surgical exploration revealed an avulsion of the distal insertion of the palmar plate, allowing the bulk of the plate to block the joint during attempted active flexion. Resection of the palmar plate restored active flexion and caused no joint instability. We do not believe this lesion, which resulted in this disability, has been previously described.

Five different palmar fixation plate designs were compared in a distal radial osteotomy cadaver model with regard to their biomechanical properties. INTRODUCTION: Thumb metacarpophalangeal joint instability can have a considerable functional impairment. Acute injuries are usually overlooked in children due to the high pain threshold and the presence of ligamentous laxity. Chronicity of the injury results in failure of conservative treatment. Literature is poor in defining such injuries. The purpose of our study is to determine the clinical and radiological tools for the diagnosis of metacarpophalangeal joint instability and we propose a simple surgical technique for their management.
