[Operative treatment of paranasal sinus diseases in childhood].

An exact knowledge of the developing anatomy and physiology, and very conservative operative technique are the prerequisites for surgical therapy of the nose and the paranasal sinuses in children. These requirements are fulfilled by uncapping of the ethmoidal cells in chronic and especially acute ethmoid sinusitis. Intranasal antrostomy allows diagnosis to be made and limited local treatment to be given in acute and chronic maxillary sinusitis. More radical surgical procedures are not indicated in children, except in those cases, where severe orbital or endocranial complications have already d

Paranasal sinus disease in horses.

The paranasal sinus system of horses is complex, comprising six pairs of sinuses. Disease processes that can develop in the sinuses include ethmoid hematomas, cysts, neoplasia, and bacterial and fungal infections. Horses that develop paranasal sinus disease vary widely in age. A significant number of clinical conditions involving the paranasal sinuses require a combination of diagnostic tests to determine the specific pathologic process. Because of the anatomic location of the paranasal sinuses and associated chronic conditions that affect many patients, many disease processes involving the pa

Nasal nitric oxide concentration in paranasal sinus inflammatory diseases.

In normal upper airways, nitric oxide is generated by the paranasal sinus epithelium and then diffuses into the nasal cavities. This study examined whether or not nasal NO concentration is affected by paranasal sinus inflammatory diseases. The influence of obstruction (nasal polyposis) and/or inflammation (allergy or chronic sinusitis) of the paranasal sinuses on nasal NO concentration was evaluated in nasal allergic (n=7 patients) or nonallergic (n=20) polyposis, nonallergic chronic sinusitis (n=10) and Kartagener's syndrome (n=6) and compared with control subjects (n=42). A score of alterati

[State of the Art. Diagnostic imaging of paranasal sinus diseases].

The following article describes a diagnostic concept of paranasal sinus imaging, based on more than 10 years close collaboration of the authors. CT and MR are the primary imaging modalities for investigating paranasal sinus lesions. Conventional X-ray images add little information due to insufficient sensitivity. Angiography is restricted to those patients who require preoperative tumor embolization or emergency treatment of intractable epistaxis. After thorough medical treatment of inflammatory paranasal sinus disease, CT becomes the method of choice to clearly depict preoperative osseous ana

[Investigation of inflammatory paranasal sinus diseases with scintigraphy (author's transl)].

In the course of inflammatory diseases affecting the paranasal sinuses, a number of complications may occur. One of those is the involvement of neighbouring skeletal portions. By this study evidence is given, that there is a high rate of inflammation, spreading to adjoining bone areas. This applis also to early stages and first manifestations. Especially in cases of surgery, complete recovery is rare. The pattern of chronic recurrent osteomyelitis may develop.

Acidic glycosaminoglycans in the ground substance in paranasal sinus diseases.

Acidic glycosaminoglycans (AGAG) in the normal maxillary sinus mucosae were analysed by electrophoretic separation and densitometric quantitation, and then compared with those in chronic maxillary sinusitis, nasal polyps and maxillary sinus cancer. The total amount of AGAG in the edematous maxillary sinusitis and nasal polyps increased significantly. Both increases were almost twice as large as the normal content. When the changes of each AGAG in the total amount were compared with the normal content, chondroitin sulfate A and C showed the most conspicuous increase, chondroitin sulfate B was r

Treatment of chronic paranasal sinus pain with minimal sinus disease.

A common problem for otolaryngologists are patients who present with recurrent, persistent sinus pain that appears out of proportion to the findings on physical examination. Often these patients have a history of recurrent sinusitis that required antibiotics or surgical intervention. Many have had repeated surgical procedures because of this pain. Other common past medical histories may include allergic rhinitis, facial trauma, or dental disease. Patients who have experienced documented acute sinusitis in the past will often present de novo with similar symptoms, but lack any objective evidenc

[Holmium:YAG laser surgery. Possibilities for use in inflammatory paranasal sinus diseases].

A pulsed Holmium: YAG laser (lambda = 2120 nm) was used during functional endoscopic endonasal surgery in ten patients with recurrent chronic inflammation of the paranasal sinuses. The Holmium: YAG laser energy was coupled to a 600 microns core diameter nylon fiber. Power settings of 950 mJ per 2.5 microseconds pulse at 5 pulses per second were used. Tissue responses to treatment were examined by light microscopy. The photoablative mechanism of the laser-tissue interaction led to bone and soft tissue ablation with only a minor thermal component in the target zone. In comparison with other lase

[Ultrasonography in paranasal sinus disease (author's transl)].

Diagnostic A- and B-mode ultrasonography is used for detection of paranasal sinus disease. Various cases of maxillary, frontal and ethmoidal sinus pathology demonstrate the indication and limitations for this new method. Differential diagnostic of an opacified antrum is possible changing gain control and patients head position during examination.

Headaches due to Nasal and Paranasal Sinus Disease.

Headache and rhinosinusitis are 2 of the most common conditions seen in clinical practice. Consider sinusitis in those with new-onset headache, along with nasal congestion, maxillary tooth discomfort, anosmia, cough, or fever. Most chronic and recurring headaches, especially if migraine features are present, are not due to sinus disease, with the possible exception of rhinogenic headache due to nasal contact points. Nasal endoscopy and neuroimaging with computed tomography or MRI can confirm diagnosis and guide treatment with antibiotics, adjuvant therapies and surgery.

Headaches due to nasal and paranasal sinus disease.

Sinus infections are much less common today than they were in the preantibiotic era, but they still are overdiagnosed. Acute sinusitis, a relatively uncommon cause of headache, is the result of infection of one or more of the cranial sinuses. Acute sinusitis usually is characterized by purulent discharge in the nasal passages and a pain profile determined by the site of infection. Sinusitis is overdiagnosed as a cause of headache because of the belief that pain over the sinuses must be related to the sinuses. In fact, frontal head pain more often is caused by migraine and tension-type headache
