[Paraneoplastic cerebellar degeneration].

Paraneoplastic cerebellar degeneration (PCD) is a rare complication of systemic cancer. PCD may present as a "pure", severe pan-cerebellar syndrome of subacute progression or be only one clinical feature in the setting of extensive CNS disease. The most characteristic form of "pure" PCD is associated with the presence of an anti-Purkinje cell antibody (AB), called anti-Yo, in patients with breast or ovarian cancer. The primary tumor is very often unknown when the cerebellar signs occur, and extensive investigations, including laparotomy or prolonged follow-up may be required to demonstrate its

Treatment of paraneoplastic cerebellar degeneration.

Paraneoplastic cerebellar degeneration is an uncommon autoimmune disorder characterized clinically by progressive, ultimately incapacitating ataxia and pathologically by destruction of cerebellar Purkinje cells, with variable loss of other cell populations. The disorder is most commonly associated with gynecological and breast carcinomas, small cell carcinoma of the lung, and Hodgkin's disease and in most cases comes on prior to identification of the underlying neoplasm. The hallmark of paraneoplastic cerebellar degeneration is the presence of an immune response reactive with intracellular pro

[Paraneoplastic cerebellar degeneration with asymmetrical pan-cerebellar syndrome].

INTRODUCTION: Paraneoplasic cerebellar degeneration is seen clinically as a pancerebellar condition which is usually symmetrical. Different families of tumours are associated with this, particularly (in view of its frequency) oat cell pulmonary carcinoma, gynecological tumours and Hodgkin's lymphoma. CLINICAL CASE: Signs of cerebellar atrophy were seen on MR and cortical hypoperfusion was seen on Single Photon Emission Computerized Tomography (SPECT). We present the case of a 76 year old woman who presented with an asymmetrical pancerebellar disorder of gradual onset, with positive anti-self

Clinical insights into paraneoplastic cerebellar degeneration.

Neuroimaging is usually unremarkable in paraneoplastic cerebellar degeneration (PCD), at least in the early stages of the disease. A patient with proven PCD is reported in whom it could be shown that cerebellar atrophy evolved very rapidly and was present in early imaging studies. Even with the use of the whole spectrum of modern diagnostic tools, the underlying malignancy can be difficult to diagnose. In addition to mammography, MRI is recommended in these cases and repeat FDG-PET may be necessary.

Proteasome antibodies in paraneoplastic cerebellar degeneration.

Antibodies to proteasome have been detected in several autoimmune diseases, including multiple sclerosis. We have investigated the presence of such antibodies in patients with paraneoplastic neurological syndromes, by Western blotting and immunohistochemistry. Antibodies to 20S proteasome were detected in the majority of patients with paraneoplastic cerebellar degeneration (PCD), but in only one of nine sera from patients with paraneoplastic encephalomyelitis/sensory neuronopathy (PEM/SN), and were not found in cancer patients in general. The results suggest that the immune responses in PCD di

Breast cancer and paraneoplastic cerebellar degeneration.

Of the remote effects of cancer on the neurologic system, paraneoplastic cerebellar degeneration (PCD), characterized by global cerebellar dysfunction, is second only to paraneoplastic neuropathies in frequency. Recent evidence, including the finding of anti-Purkinje cell (now termed anti-Yo) antibodies directed against specific protein antigens shared by Purkinje's and tumor cells, supports an autoimmune etiology for this disorder. Increasingly, a link between breast cancer and PCD is being recognized. The authors report a case, as well as a comprehensive overview, of 62 women with breast can

[Paraneoplastic cerebellar degeneration in Hodgkin's disease].

HISTORY AND CLINICAL FINDINGS: A 30-year-old previously healthy man suddenly developed double vision, unsteady gait and some difficulty in speech articulation. Within 4 weeks he had become markedly ataxic, unable to walk, stand or sit down unaided. Neurological examination indicated a severe cerebellar syndrome. There were no other abnormal findings on physical examination. INVESTIGATIONS: There was no pleocytosis and no oligoclonal bands in cerebrospinal fluid (CSF). A test for anti-Purkinje cell antibodies was negative in both CSF and serum. Computed tomography and nuclear magnetic imaging

Paraneoplastic cerebellar degeneration associated with cerebellar hypermetabolism: Case report.

RATIONALE: Paraneoplastic cerebellar degeneration (PCD) is an immune-mediated neurological deficit affecting the cerebellum. Anti-Yo antibody positive PCD is a rare occurrence most likely associated with gynecologic or breast malignancies. The identification of the underlying tumor is a diagnostic challenge in many of these patients. PATIENT CONCERNS: We present a 68-year-old woman with acute symptoms of PCD as a first sign of underlying occult malignancy. Further investigation revealed a positive anti-Yo antibody. Although brain magnetic resonance imaging (MRI) was unremarkable, positron emi

Paraneoplastic cerebellar degeneration. III. Cerebellar degeneration, cancer, and the Lambert-Eaton myasthenic syndrome.

We studied nine patients with a subacute onset of a pancerebellar syndrome. Six had known cancer (three small-cell carcinoma of the lung [SCLC], one metastatic small-cell carcinoma, one small-cell carcinoma of the prostate, and one non-Hodgkin's lymphoma). Six of eight who had neurophysiologic testing, including the three patients without detectable cancer, had coexistent Lambert-Eaton myasthenic syndrome (LEMS). In two of the patients, LEMS was discovered only by neurophysiologic testing. We looked for anti-Purkinje cell autoantibodies in all patient's sera and in four patients' CSF. We also

Paraneoplastic cerebellar degeneration with fallopian tube adenocarcinoma.

BACKGROUND: Paraneoplastic cerebellar degeneration (PCD) is rarely caused by fallopian tube adenocarcinoma. CASE: We present a patient with PCD and fallopian tube cancer. Anti-Yo antibody, one of an anti-neuronal antibody, was positive in serum and cerebrospinal fluid. She was also positive for HLA A24, which is common in patients with PCD. Radical surgery did not significantly ameliorate her neurological impairment, although the dysarthria improved slightly. CONCLUSION: This case highlights the importance of detecting the underlying malignancy in patients with subacute neurological impairme

Paraneoplastic cerebellar degeneration heralding fallopian tube adenocarcinoma.

The objective of this paper is to describe an 81-year-old woman with subacute cerebellar degeneration due to fallopian tube adenocarcinoma. Serum anti-Yo antibodies were used to screen for pelvic malignancy. Their presence led to a meticulous search, which included bilateral salpingoophorectomy. Subsequently an occult fallopian tube adenocarcinoma was discovered. This case report highlights the diagnostic value of antineuroneal antibodies in females with subacute neurologic impairment in the form of paraneoplastic syndrome.

Paraneoplastic cerebellar degeneration associated with thymic germinoma.

Paraneoplastic cerebellar degeneration (PCD) is characterized by a subacute, severe pancerebellar syndrome, which is related to an underlying tumor. The presence of CSF or serum onconeural antibodies confirms the diagnosis and indicates the underlying tumor type. However, the association between PCD and extragonadal germ cell tumors with the absence of an onconeural antibody has rarely been described. We present a 55-year-old man who developed a pancerebellar syndrome, which made him unable to walk alone after 10months. Routine blood analysis, brain MRI and CSF examination were normal. Despite

Paraneoplastic cerebellar degeneration secondary to ovarian carcinosarcoma: a cerebellar conundrum.

We present a case of an elderly female patient who presented with a 6-month history of progressive slurred speech, vertigo, unsteadiness and falls. She underwent an extensive battery of neurological and cardiovascular investigations, none of which demonstrated a diagnostic cause for her symptoms. She was referred to the stroke and neurology teams and was started on treatment for presumed anxiety. As her symptoms continued to progress, she was referred to the falls service. Following a multidisciplinary team discussion, she was reviewed by the consultant geriatrician who felt this may be due to

Deterioration of anti-Yo-associated paraneoplastic cerebellar degeneration.

OBJECTIVES: Usually, the course of paraneoplastic cerebellar degeneration(PCD) is stable or progresses only slowly. Sudden marked progression after several years, as in the following case, has not been reported. CASE REPORT: After a 57 year old female had developed diplopia, cerebellar signs, upper-limb weakness, and bilateral stocking-type hypoesthesia, and Yo-antibodies were positive, PCD and sensory polyneuropathy were diagnosed. Upon further diagnostic work-up ovarian cancer FIGO-IIC was detected and treated with ovarectomy, hysterectomy, omentectomy, and chemotherapy. Within the followin

Antineural autoantibodies in patients with paraneoplastic cerebellar degeneration.

We studied serum autoantibodies from six patients with paraneoplastic cerebellar degeneration using rat brains. Immunohistochemically, serum samples from four of six patients with uterine or ovarian tumors showed a similar staining pattern, labeling Purkinje cells and various other neurons distributed throughout the brain and spinal cord. The immunoblot study showed that these serum samples recognized the common 52-kd band. Serum from a patient with duct carcinoma of the breast reacted with the 46-kd band with a similar but weaker immunohistochemical reactivity. Serum from another patient with

Paraneoplastic cerebellar degeneration in a patient with ovarian cancer.

OBJECTIVE: Herein, we report a case of ovarian serous carcinoma with paraneoplastic cerebellar degeneration. CASE REPORT: A 44-year-old female presented to our hospital with dizziness, slurred speech, and ataxic gait. Brain magnetic resonance imaging was normal. A lumbar puncture revealed a normal cell count in the cerebrospinal fluid, but slightly elevated protein. Her serum cancer antigen -125 level was high (2126.4 U/mL), and abdominal computed tomography disclosed a pelvic mass measuring 11 cm in diameter. Exploratory laparotomy was then performed, and a frozen section of the tumor reveal
