Partial weight bearing of the tibia.

INTRODUCTION: Partial weight bearing is part of treatment schemes in orthopedic surgery and traumatology. The aim of the present study was to explore to what degree ground reaction forces during partial weight bearing of the lower leg are related to given instructions and to tibia deformation. MATERIALS AND METHODS: Tibia deformation (torsion, medio-lateral and anterio-posterior bending) was measured for rear foot and forefoot loading, 10kg, 20kg and half body weight instructions compared to full loading in five healthy male subjects using the "Optical Segment Tracking" approach, a motion-cap

Walking cane designed to assist partial weight bearing.

The device described indicates accurately the amount of weight borne on an affected leg. This device consists of two major elements: a weight measuring system incorporated in the walking cane's body, and a feedback mechanism situated in the cane's handle. By means of vibration or lights the patient and the therapist can determine whether or not the affected leg is bearing the prescribed weight. Thirty patients (10 with fractures, 10 with vascular disorders, and 10 below-knee amputees) and 10 healthy people participated in the study. The study concluded that young patients used the feedback inf

Orthopedic inpatients’ ability to accurately reproduce partial weight bearing orders.

Partial weight bearing is often prescribed for patients with orthopedic injuries. Patients’ ability to accurately reproduce partial weight bearing orders is variable, and its impact on clinical outcomes is unknown. This observational study measured patients’ ability to reproduce partial weight bearing orders, factors influencing this, patients’ and physiotherapists’ ability to gauge partial weight bearing accuracy, and the effect of partial weight bearing accuracy on long-term clinical outcomes. Fifty-one orthopedic inpatients prescribed partial weight bearing were included. All received stand

Haptic biofeedback for improving compliance with lower-extremity partial weight bearing.

After lower-extremity orthopedic trauma and surgery, patients are often advised to restrict weight bearing on the affected limb. Conventional training methods are not effective at enabling patients to comply with recommendations for partial weight bearing. The current study assessed a novel method of using real-time haptic (vibratory/vibrotactile) biofeedback to improve compliance with instructions for partial weight bearing. Thirty healthy, asymptomatic participants were randomized into 1 of 3 groups: verbal instruction, bathroom scale training, and haptic biofeedback. Participants were instr

Partial weight-bearing following ankle fracture: what's the actual load in early recovery?

PURPOSE: This study investigates the learning efficacy for partial weight load before discharge as well as the impact of biofeedback during the learning process. METHODS: We monitored weight-bearing in 57 patients who had surgery for ankle fractures. Continuous measurements without and with biofeedback were performed in the early postoperative stage in order to, first, assess how well these patients could apply what they have learned before being discharged, and second, to examine the influence of biofeedback. RESULTS: Using conventional teaching methods, only about one-third of patients (36

Load Monitoring System for Partial Weight Bearing Therapy for rehabilitation of lower extremity fractures.

Partial Weight Bearing Therapy (PWB) is used during in rehabilitation of lower-extremity fractures, but optimal outcomes are limited by the paucity of data supporting the current standard of care, the inability of clinicians to assess patients' compliance to the prescribed therapy, and by the patient's inability to apply clinical loading guidelines. To address these needs a Load Monitoring System (LMS) was developed using a novel long-term load measuring technology coupled with consumer electronics for storing and reporting of patient limb loading data. The project was planned as a staged effo

Design of a Portable Biofeedback System for Monitoring Femoral Load During Partial Weight-Bearing Walking.

Patients with femoral fractures are typically advised to undergo partial weight-bearing (PWB) gait training during the postoperative rehabilitation period to facilitate bone healing and restore lower limb function. Various current portable biofeedback devices monitor ground reaction force (GRF) to assess the femoral loading of patients with fractures during PWB walking. However, due to the influence of muscle forces and the complexity of load transmission in the lower limbs, GRF may not accurately reflect the internal forces in the femur during walking. In this study, we developed an innovativ

Healing of partial flap necrosis in ankle disarticulation amputation by débridement and continued weight-bearing.

Five patients with partial tissue loss of the weight-bearing surface of the heel pad following ankle disarticulation were treated with residual-limb debridement and continued end-weight-bearing using a total-contact cast. The patients, ranging in age from 53 to 76 years, had insulin-requiring diabetes and insensate heel pads and were low-demand, limited-activity, community walkers before amputation surgery. Each underwent amputation surgery as a consequence of peripheral vascular disease. All patients progressed to complete wound healing over 3 to 6 months and were able to return to their prev

Partial Weight-Bearing after Lower Extremity Surgery: A Review of Current Standards, Innovations, and Patient Compliance.

Partial weight bearing (PWB) is a cornerstone of post-operative rehabilitation after lower extremity surgery (LES), balancing mechanical stimulation needed for bone, tendon, and cartilage healing with protection against excessive load that precipitates malunion, non-union, or joint dehiscence. This narrative review synthesizes literature to clarify current indications for PWB, evaluate traditional and emerging methods for prescribing and monitoring load, and identify factors influencing patient adherence. Hip, knee, ankle, and foot procedures commonly warrant PWB, yet optimal targets and progr

[Serious foot injury: consider partial amputation designed to preserve leg-length and a weight-bearing stump].

Three patients with physically demanding jobs, a 25-year-old farmer, a 22-year-old market trader and a 32-year-old house painter, each suffered a traumatic injury of one foot. All three underwent amputation in which part of their foot was salvaged and were able to return to their physically demanding jobs. The first patient underwent a Syme's amputation, the second a Pirogoff's amputation and the third a Chopart's amputation. A partial amputation of the foot has several advantages: patients are able to walk without walking aids, particularly in and around the house, due to the presence of a di

Can elderly individuals perform partial weight bearing on their lower limbs? A prospective cohort study using ambulatory real-time biofeedback.

BACKGROUND: Partial weight bearing in an orthosis and with forearm crutches is a widespread and well-accepted therapeutic principle after an injury of the lower extremity during early rehabilitation. Complying may be challenging to do under these circumstances, especially for elderly people. This study compares the spatiotemporal parameters and peak loads performed by a group of older participants before and after activating real-time biofeedback (BF) to determine whether they benefit from a biofeedback. METHODS: Twenty-four healthy subjects between 61 and 80 years learned how to walk using f

Validation of a Sensorized Forearm Crutch for Quantifying Partial Weight-Bearing During Assisted Gait Using Optical Motion Capture and Instrumented Treadmill.

Human gait analysis is a key component of rehabilitation medicine, enabling objective assessment of patient recovery. In crutch-assisted locomotion, however, conventional forearm crutches operate as passive devices, providing no quantitative information on load distribution or patient adherence to partial weight-bearing (PWB) prescriptions. This work presents the design and dynamic validation of a sensorized forearm crutch system for biomechanical monitoring during assisted gait. The proposed device combines a force-sensing module based on a full Wheatstone bridge strain-gauge configuration wi

Fixation of the mobile fragment in periacetabular osteotomy: a clinical study of two- vs. three-screw fixation with 4-week partial weight bearing.

AIM: Fixation of periacetabular osteotomy (PAO) has been a matter of interest since development of the procedure. Despite the stability of the construct, no consensus regarding the minimum number of screws needed for fixation is present. The aim of this study was to compare two-screw and three-screw fixation techniques in the clinical setting. METHODS: The study included a consecutive series of 100 hips that had undergone PAO surgery by a single surgeon between January 2022 and July 2023 with complete radiographic follow-up. The relationship of the angle of immobilisation of the knee to the force applied to the extensor mechanism when partially weight-bearing. A gait-analysis study in normal volunteers.

We describe the influence of the angle of immobilisation during partial weight-bearing on the forces across the extensor mechanism of the knee. Gait analysis was performed on eight healthy male subjects with the right knee in an orthotic brace locked at 0 degree, 10 degrees, 20 degrees and 30 degrees, with the brace unlocked and also without a brace. The ground reaction force, the angle of the knee and the net external flexion movement about the knee were measured and the extensor mechanism force was calculated. The results showed a direct non-linear relationship between the angle of knee flex
