Pediatric patellar dislocation.

Acute patellar dislocation affects approximately 1:1000 healthy children 9-15 years of age, and up to 50% are at risk for recurrent dislocations. In adults the condition is associated with long-term complications, such as osteoarthritis and impairment of knee function. However, literature describing the outcome in a pediatric population is sparse. The present review article evaluates the long-term effects on knee function and cartilage quality after traumatic patellar dislocation in childhood, and also to evaluate the reliability of two clinical tests of medio-lateral knee position, in healthy

Adolescent patellar osteochondral fracture following patellar dislocation.

PURPOSE: Patellar dislocations in adolescents may cause osteochondral fractures of the patella. The aim of this study was to review the outcomes of adolescent patients who underwent surgical intervention for patellar osteochondral fracture following patellar dislocation. METHODS: Nine patients who underwent surgery for osteochondral fracture of the patella following dislocation were identified retrospectively. Following arthroscopic examination, if the fragment was large enough to support fixation, headless screws or bioabsorbable pins were used. Otherwise, the loose body was excised, and the

Locked Superior Patellar Dislocation.

Superior dislocation of the patella is a rare cause of knee locking, with fewer than 20 cases reported in the English literature. The vast majority of traumatic or even atraumatic knee locking cases are due to such varied conditions as meniscal tears, osteochondral lesions, osteoarthritis, and ligamentous tears. A careful examination with proper radiographic imaging must be undertaken to properly evaluate patients who present with a locked knee, as they may have interdigitating patellofemoral osteophytes. An unusual case of a locked knee secondary to interlocking osteophytes between the femora

Fixed (Congenital) Patellar Dislocation.

Congenital dislocation of the patella is a rare condition characterized by lateral dislocation of the patella that is irreducible without surgical correction. Although there is no clear inheritance pattern, it is associated with several congenital syndromes. Patients often demonstrate flexion contracture, loss of active knee extension, increased tibial external rotation, and absent patella in the trochlea. Treatment requires surgical management and is comprised of lateral release, medial stabilization, quadriceps lengthening, and distal realignment. Results are generally favorable after treatm

Acute traumatic patellar dislocation.

Inaugural traumatic patellar dislocation is most often due to trauma sustained during physical or sports activity. Two-thirds of acute patellar dislocations occur in young active patients (less than 20 years old). Non-contact knee sprain in flexion and valgus is the leading mechanism in patellar dislocation, accounting for as many as 93% of all cases. The strong displacement of the patella tears the medial stabilizing structures, and notably the medial patellofemoral ligament (MPFL), which is almost always injured in acute patellar dislocation, most frequently at its femoral attachment. Latera

Patellar dislocation in achondroplasia.

The knee was assessed by normal plain radiographs in 20 achondroplasia patients (40 lower extremities), whose age ranged from 10.5 to 23 years. Five patients (seven knees) had permanent patellar dislocation. There was a significant difference between the achondroplasia group and control group (50 knees without skeletal dysplasia) for femoral sulcus angle but not for femorotibial angle, Insall-Salvati index and Q angle. Patellar dislocation for achondroplasia may be caused not only by the abnormalities of soft tissue but also a small groove of patellofemoral joint as a bony malformation.

Patellar dislocation: arthroscopic patellar stabilization with anchor sutures.

There are many procedures to treat recurrent patellar subluxations and dislocations. Most of these procedures are open repairs for acute conditions or open reconstructions for chronic conditions, and both surgeries need large skin incisions. Although there are many arthroscopic procedures for meniscal and ligament injuries, there are very few arthroscopic procedures described for patellofemoral instability. In this report, we describe a new arthroscopic method to stabilize the patella for acute dislocation. Our technique includes an arthroscopic lateral release and a medial structure repair. W

[Study of the patellar apex in objective patellar dislocation].

PURPOSE OF THE STUDY: The purpose of this work was to study the morphology of the patella in search of dysplasia typical of objective patellar dislocation. MATERIAL AND METHODS: One hundred forty patients (190 knees), 80 women and 60 men, who underwent surgery between 1988 and 1999 were included in this study. Mean age was 22.6 years (range 13-47). Patellar morphology was analyzed on the preoperative lateral x-rays (n=190) taken at 30 degrees flexion. The apex of the patella was measured as well as the length of the patellar tendon. The width of the patella was measured on the computed tomogr

Locked lateral patellar dislocation: a rare case of irreducible patellar dislocation requiring open reduction.

Irreducible patellar dislocations are rare injuries, but those that do occur are mainly directed intra-articularly. In this case, a 53-year-old woman sustained a locked lateral patellar dislocation when falling from a chair. A preoperative CT-scan revealed bony avulsions at the insertion of the vastus medialis muscle, the medical retinaculum, and partial disruption of the ligamentum patellae from the apex patellae. Open reduction was necessary and the torn structures were reattached with anchor systems and sutures. Postoperative management included intensive physiotherapy. At 1 year after surg

Neglected locked vertical patellar dislocation.

Patellar dislocations occurring about the vertical and horizontal axis are rare and irreducible. The neglected patellar dislocation is still rarer. We describe the clinical presentation and management of a case of neglected vertical patellar dislocation in a 6 year-old boy who sustained an external rotational strain with a laterally directed force to his knee. Initially the diagnosis was missed and 2 months later open reduction was done. The increased tension generated by the rotation of the lateral extensor retinaculum kept the patella locked in the lateral gutter even with the knee in full e

Patellofemoral osteoarthritis after patellar dislocation.

Clinical and radiographic studies were done for 85 patients treated conservatively for acute primary patellar dislocation occurring an average 13 years (range, 6-26 years) previously. Osteoarthritic changes in the patellofemoral joint were evaluated with special reference to the primary conservative treatment and treatment of redislocations (operative or conservative) or treatment for other subsequent problems such as pain and/or subluxations (late surgery). The patients were divided into two groups on the basis of findings in the unaffected knee and other joints. There were 56 patients (66%)

Permanent post-traumatic patellar dislocation.

Permanent post-traumatic dislocation of the patella is an extremely rare condition, and is often confused with congenital dislocation. The anatomic abnormalities differ in the two conditions: in the former there is an adaptive flattening of the patella, in the latter there is a propensity for flexion contractures and incongruity of the patella and trochlea. Treatment options include observation, patellar realignment, and patellectomy. In the patient reported, surgical correction appeared to result in patellar alignment.

[Patellar dislocation : Causes and treatment].

The causes of patellofemoral instability (PFI) are complex. In accordance with the current literature a classification was developed which clearly weights all entities and derives diagnostic and therapeutic consequences. It considers patellar instability and patellar maltracking or the complete loss of patellar tracking and differentiates into 5 types. Type 1: patellar dislocation without maltracking or instability with a low risk of redislocation. Type 2: high risk of redislocation, no maltracking. Type 3: instability and maltracking; reasons for maltracking are a) soft tissue contracture, b)

[Old unreduced traumatic patellar dislocation].

The old unreduced traumatic patellar dislocation is very rare and the results of the treatment are not always satisfactory. We report a 11 year-old boy with an unreduced patellar dislocation and a healed ipsilateral femoral fracture. The dislocation was reduced by an operative procedure. A long leg cast was applied for 6 weeks then the knee exercises were started. One year after surgery, he still has satisfactory results. This case suggests that if the dislocation is not of long duration and degenerative changes are absent in the patella and the knee joint, open reduction may be helpful in the

Patellar dislocation in army conscripts.

Between 1990 and 1996, 119 Finnish male conscripts underwent operative treatment for patellar dislocation. There were 68 conscripts (58%) with primary and 51 conscripts (42%) with recurrent patellar dislocation. Sixty-five (55%) dislocations occurred during military service, 40 (34%) occurred during sports activities, and 14 (11%) occurred during leisure time. The most common cause of injury was military training at battle exercises (n = 30). The typical injury mechanism was knee valgus rotation on fixed foot and tibia (97 conscripts, 82%). Surgical procedures performed were open in 75 conscri
