Prediction of axillary pathologic response with breast pathologic complete response after neoadjuvant chemotherapy.

PURPOSE: Many breast cancer patients receive sentinel lymph node biopsy after neoadjuvant chemotherapy (NAC). We evaluated the axillary pathologic complete response (pCR) in relation to achievement of breast pCR. METHODS: We evaluated 1044 patients who were diagnosed with invasive breast cancer and were treated with NAC followed by curative surgery at the Samsung Medical Center between January 2008 and December 2016. The pathologic node-positive rates in breast pCR patients were compared for different breast cancer subtypes and clinical nodal stages. RESULTS: Axillary pCR was achieved in 51.

Do not expect an endoluminal complete response to identify a pathologic complete response in rectal cancer!

PURPOSE: To examine the relationship between endoluminal and pathologic complete response after chemoradiotherapy for rectal cancer and identify predictors of a pathologic complete response. METHOD: The anatomic pathology reports of 102 consecutive patients with rectal cancer who underwent neoadjuvant chemoradiotherapy followed by proctectomy between 2013 and 2017 were reviewed for the presence or absence of endoluminal complete response. The presence of endoluminal complete response was compared with the anatomopathological stage. The residual lesion area was compared with the final patholog

The pathologic complete response open question in primary therapy.

Accurate pathological diagnosis of tumor mass before treatment and careful examination of specimens after treatment are two main objectives in the diagnostic process of neoadjuvant-treated breast cancer. To achieve the first objective, multiple core biopsies can be taken to assess intratumor heterogeneity and thus to address a more targeted therapeutic protocol. At the same time, fine needle aspiration cytology of lymph nodes found suspicious at ultrasound examination may be useful to plan surgical treatment. As for the second objective, a careful examination of specimens after primary systemi

Characteristics of pathologic complete response for locally advanced rectal cancer.

BACKGROUND: Neoadjuvant chemoradiation (NACRT) is the standard of care for locally advanced rectal cancers. The purpose of this study was to determine patient and tumor factors associated with a pathologic complete response (pCR). METHODS: The National Surgical Quality Improvement Program proctectomy-targeted database was utilized to identify all patients from 2016 to 2020 who underwent NACRT followed by proctectomy with curative intent for T3-4N0-2 rectal cancers. RESULTS: A total of 1891 patients were included, of which 253 (13.4%) demonstrated a pCR. Pretreatment N0 staging was associated

Multimodal AI Methods for Predicting Pathologic Complete Response in Breast Cancer.

Neoadjuvant chemotherapy (NAC) can eliminate all invasive cancer in some breast cancer patients, achieving a pathologic complete response (pCR) that is associated with a better prognosis. Prediction of pCR from pre-treatment radiology imaging is challenging but could provide immense value in the treatment planning. We propose a novel multimodal deep learning approach that combines pre-treatment dynamic contrast-enhanced MRI and clinical data for predicting pCR before chemotherapy. It is the first to integrate attention-based multiple instance learning technique for slice aggregation and a self

Long-term Oncologic Outcomes Following Pathologic Complete Response for Esophageal Cancer.

BACKGROUND: While multimodality therapy for locally advanced esophageal cancer evolves, neoadjuvant chemoradiation has been a longstanding treatment paradigm. Pathologic complete response (pCR) rates vary by histology: 25% for adenocarcinoma (AC) and up to 50% for squamous cell carcinoma (SCC). Long-term outcomes after pCR remain poorly understood but important to define as multiple treatment options emerge. PATIENTS AND METHODS: We analyzed patients with cT2+/N+M0 esophageal AC or SCC treated with neoadjuvant chemoradiation and resection from an institutional database. Recurrence-free surviv

Do Decreased Breast Microcalcifications After Neoadjuvant Chemotherapy Predict Pathologic Complete Response?

BACKGROUND: Clinical response evaluation by image examination after neoadjuvant chemotherapy for breast cancer is important for determining drug response and progression. Mammography is less correlated with pathologic response assessment than magnetic resonance imaging or ultrasonography. The present study clarified characteristics of breast cancer patients with decreased microcalcifications after neoadjuvant chemotherapy to increase the accuracy of mammographic assessment. PATIENTS AND METHODS: Consecutive patients who received neoadjuvant chemotherapy for breast cancer at our hospital from

Revisiting OPTIMIST trial imaging criteria for pathologic complete response to neoadjuvant therapy.

INTRODUCTION: Omission of breast surgery after neoadjuvant systemic therapy (NST) may reduce treatment burden in exceptional responders, but safe implementation requires reliable identification of pathologic complete response (pCR). This study assessed the performance of OPTIMIST imaging criteria for predicting pCR and explored whether refined thresholds could improve applicability without compromising performance. MATERIALS AND METHODS: We retrospectively reviewed patients who received NST followed by surgery at a single tertiary center between 2016 and 2020. Among 1659 patients, 499 met OPT

Predictors of Nodal Pathologic Complete Response After Neoadjuvant Chemotherapy in Breast Cancer.

BACKGROUND: Nodal pathologic complete response (npCR) after neoadjuvant treatment in patients with node-positive breast cancer (BC) avoids the morbidity of axillary dissection. In this study, we aimed to identify predictors of npCR. PATIENTS AND METHODS: Adult women with stage 2-3 BC and clinically positive nodes from 2011 to 2021 in the National Cancer Database who received neoadjuvant chemotherapy followed by surgery within 8 months were included. Predictors of npCR were modeled with multivariable logistic regression. RESULTS: In total, 47,483 patients were included: 18,978 (40.0%) with np

Axillary Pathologic Complete Response in Inflammatory Breast Cancer Patients: Implications for SLNB?

BACKGROUND: Sentinel lymph node biopsy (SLNB) is increasingly utilized after neoadjuvant chemotherapy (NAC) in responsive adenopathy, particularly with placement of a marking clip in the involved node(s). This may allow a subset of patients to avoid axillary lymph node dissection. SLNB is still discouraged in inflammatory breast cancer (IBC). The purpose of this study is to examine the axillary pathologic complete response (AXpCR) in IBC patients with clinical adenopathy. There may be an implication to approach a subset of IBC patients for SLNB after NAC. METHODS: A single-institution institu

Pathological complete response in pancreatic adenocarcinoma with FOLFIRINOX.

The report describes a patient who presented to our centre with abdominal pain and significant weight loss due to adenocarcinoma of the tail of the pancreas. The cancer was deemed as 'resectable disease associated with morbid surgical outcomes' due to the local involvement of the vessels and adjacent organs. Given the patient's excellent performance status, the patient underwent neoadjuvant chemotherapy with folinic acid, fluorouracil, irinotecan and oxaliplatin to downstage the tumour for less morbid surgical resection. The patient underwent 12 cycles of chemotherapy with serial imaging which

Complete Pathologic Response of Squamous Cell Carcinoma Abutting an Internal Mammary Bypass Graft.

We present a case of an advanced squamous cell carcinoma encroaching on a patient's left internal mammary artery bypass graft. Tumor board consensus was to proceed with 2 cycles of neoadjuvant chemotherapy followed by resection. Intraoperatively, the left internal mammary artery bypass could not be safely dissected from the adjacent pleura, but frozen sections were negative for malignant transformation. Final pathologic examination showed a complete pathologic response to neoadjuvant chemotherapy, and surveillance imaging is now negative for recurrence 5 years postoperatively. Although maligna

Gene signatures in patients with early breast cancer and relapse despite pathologic complete response.

A substantial minority of early breast cancer (EBC) patients relapse despite their tumors achieving pathologic complete response (pCR) after neoadjuvant therapy. We compared gene expression (BC360; nCounter® platform; NanoString) between primary tumors of patients with post-pCR relapse (N = 14) with: (i) matched recurrent tumors from same patient (intraindividual analysis); and (ii) primary tumors from matched controls with pCR and no relapse (N = 41; interindividual analysis). Intraindividual analysis showed lower estrogen receptor signaling signature expression in recurrent tumors versus pri

Laboratory indicators predict axillary nodal pathologic complete response after neoadjuvant therapy in breast cancer.

The purpose was to integrate clinicopathological and laboratory indicators to predict axillary nodal pathologic complete response (apCR) after neoadjuvant therapy (NAT). The pretreatment clinicopathological and laboratory indicators of 416 clinical nodal-positive breast cancer patients who underwent surgery after NAT were analyzed from April 2015 to 2020. Predictive factors of apCR were examined by logistic analysis. A nomogram was built according to logistic analysis. Among the 416 patients, 37.3% achieved apCR. Multivariate analysis showed that age, pathological grading, molecular subtype an
