Powering Value-Based Care with Clinician-Led, Patient-Centered Design: The MyHealth Bundles Program.

Value-based care has long been championed for its potential to align clinical outcomes with financial incentives, but many health systems struggle to implement truly transformative models. Fee-for-service constraints, payer fragmentation, and complex claims processing often limit progress to incremental gains rather than transformation to true value-based arrangements. Herein, the authors present an innovative approach to bundled payments that overcomes many of these hurdles by placing physicians and patients at the center of the redesign process while partnering with self-insured employers. Bundle care interventions for patients with perianal pain: A scoping review.

BACKGROUND: Perianal pain is influenced by a complex interplay of physiological and psychological factors. Current research focuses predominantly on surgical or pharmacological treatments, while studies on bundle care interventions remain insufficient, particularly regarding dynamic assessments and mechanistic explorations. OBJECTIVE: This scoping review aims to comprehensively analyze bundle care interventions for patients with perianal pain to provide a reference for precision management tailored to diverse patient profiles. By exploring individual characteristics and multicomponent interve

Implementation of an SSI care bundle for patients undergoing emergency laparotomy.

BACKGROUND: Surgical site infection (SSI) is common after emergency laparotomy (EL) due to contamination of the surgical field, yet most hospitals are unaware of their SSI rate. Most studies have focused on elective surgery where the surgical environment can be standardized easily. METHODS: This prospective quality improvement study, with continuous SSI surveillance over 4 years, aimed to establish a reliable method for measuring post-EL SSI and to evaluate the impact of a care bundle (2% chlorhexidine skin preparation, second antibiotic dose after 4 h, use of a wound protector, antibacterial

Evaluating a Fistula Self-Care Bundle's Effect on Hemodialysis Patients' Complications.

INTRODUCTION: Hemodialysis often leads to complications related to the arteriovenous fistula (AVF), the primary vascular access for dialysis. AVF complications, such as thrombosis and infection, are common and can disrupt treatment, increase morbidity, and diminish patients' quality of life. Effective self-care and management of AVF are essential to minimize these risks, but many patients lack adequate knowledge and skills to perform necessary self-care practices. OBJECTIVE: To evaluate a fistula self-care bundle's effect on hemodialysis patients' complications. METHODS: Quasi-experimental d

Influence of bundled care treatment on functional outcome in patients with intracerebral hemorrhage.

BACKGROUND AND AIMS: General guideline recommendations in patients with intracerebral hemorrhage (ICH) include blood pressure-, temperature- and glucose management. The therapeutic effect of such a "care bundle" (blood pressure lowering, glycemic control, and treatment of pyrexia) on clinical outcomes becomes increasingly established. For the present study, we aimed to investigate associations of strict bundled care treatment (BCT) with clinical outcomes and characterize associations with key outcome effectors such as hematoma enlargement (HE) and peak perihemorrhagic edema (PHE). METHODS: We

Implementation of a 'care bundle' improves the management of patients admitted to hospital with decompensated cirrhosis.

BACKGROUND: Since 1970, there has been a 400% increase in liver-related deaths due to the increasing prevalence of chronic liver disease in the United Kingdom (UK). The 2013 UK National Confidential Enquiry into Patient Outcome and Death report found that only 47% of patients who died from alcohol-related liver disease received 'good care' during their hospital stay. AIM: To develop a 'care bundle' for patients with decompensated cirrhosis, aiming to ensure that evidence-based treatments are delivered within the first 24 h of hospital admission. METHODS: This work gives practical advice abou

Care Bundle for Acute Kidney Injury in Cardiac Patients: A Cluster-Randomized Trial.

Detection and timely intervention of acute kidney injury (AKI) is a major challenge worldwide. Electronic alerts for AKI may improve process- and patient-related endpoints. The present study evaluated the efficacy of an AKI electronic alert system and care bundle. This is a two-arm, prospective, cluster-randomized, controlled trial enrolling patients with AKI (KDIGO criteria) and cardiac diseases. Patients were randomly assigned to a routine care group or intervention group (DRKS-IDDRKS00017751). Two hundred patients (age 79 years, 46% female) were enrolled, with 100 patients in each group. Application effect for a care bundle in optimizing nursing of patients with severe craniocerebral injury.

BACKGROUND: Severe craniocerebral injury (STBI) is a critical physical trauma caused by a sudden external force acting on the head. The condition is complex and changeable, and disability and mortality rates are high. Although the life of STBI patients can be saved through treatment, the sequelae of consciousness, speech, cognitive impairment, stiffness, spasm, pain and abnormal behavior in the early rehabilitation stage can be a heavy burden to a family. In the past, routine nursing was often used to treat/manage STBI; however, problems, such as improper cooperation and untimely communication
