PURPOSE: The Joint Commission on Accreditation of Healthcare Organizations (JCAHO) defines a "handoff" as a contemporaneous, interactive process of passing patient-specific information from one caregiver to another for the purpose of ensuring the continuity and safety of patient care. The purpose of this study was to conduct a comprehensive investigation on the determinants of an effective handoff management system. Specifically, we sought to address the following null hypotheses: There is no difference before and after implementation of a new, low-cost, low-tech process for surgery patient ha

Anaesthesia handoffs are associated with negative outcomes (e.g. inappropriate treatments, post-operative complications, and in-hospital mortality). To minimise these adverse outcomes, federal bodies (e.g. Joint Commission) have mandated handoff standardisation. Due to the proliferation of handoff interventions and research, there is a need to meta-analyze anaesthesia handoffs. Therefore, we performed meta-analyses on the provider, patient, organisational, and handoff outcomes related to post-operative anaesthesia handoff protocols. We meta-analysed 41 articles with post-operative anaesthesia

While care transitions influence quality of care, less work studies transitions between hospital units. We studied care transitions from the operating room (OR) to pediatric and adult intensive critical care units (ICU) using Systems Engineering Initiative for Patient Safety (SEIPS)-based process modeling. We interviewed twenty-nine physicians (surgery, anesthesia, pediatric critical care) and nurses (OR, ICU) and administered the AHRQ Hospital Survey on Patient Safety Culture items about handoffs, care transitions and teamwork. Care transitions are complex, spatio-temporal processes and invol

PURPOSE: The I-PASS tool has been shown to decrease medical errors in patient handoffs in nonorthopaedic surgery fields. We prospectively studied the implementation of a version of this handoff tool modified for orthopaedic surgery patients in an academic practice at two level I trauma centers.
METHODS: This was a prospective study of a multicenter handoff improvement program. Handoffs were evaluated preintervention and at 1, 6, 9, and 18 months postintervention for key data elements defined by I-PASS. Rates of adverse clinical outcomes were compared before and after the handoff intervention.

OBJECTIVES: Handoffs of patient care are opportunities for promoting efficiency and safety. With the shift from paper to digital records, we examine changes in provider perceptions of efficiency, errors, and confidence on a labor and delivery service at an urban, academic hospital.
METHODS: An anonymous survey was sent during the use of a paper and digital handoff board. Descriptive statistics and χ2 analyses were used to compare provider perceptions.
RESULTS: Survey responders were primarily resident (50.0%-54.8%) and attending (35.5%-50.0%) physicians, as well as one certified nurse-midwif

INTRODUCTION: Patient handoffs, the communications required for the safe transfer of patient care, are known to be a common source of medical errors. Simulation exercises are effective techniques for teaching the procedures and patient interaction skills involved in a handoff. We developed a teaching tool that allows students to individually interact with a simulated patient, develop a treatment plan, and practice a handoff to another provider.
METHODS: The curriculum is a flexible instructional tool to teach patient handoffs in the context of a simulated obstetric emergency for learners at t

Improving physician handoffs is a patient safety priority. The authors hypothesize that standardized handoff interventions during care transitions improve patient-related outcome measures. PubMed, Cochrane, PsycINFO, CINAHL, Embase, and Web of Science were searched for publications from 2000 to May 2016. Eligible studies compared standardized handoff intervention(s) with no standardized handoff intervention and measured patient-related outcomes. Studies were evaluated independently for eligibility for inclusion by at least 2 authors in a 2-stage process; 14 articles met inclusion criteria. Onl

INTRODUCTION: Intraoperative anaesthesia handoffs represent a risk point in the care of surgical patients. Although often necessary to prevent fatigue, improve vigilance and optimise operational efficiency, critical information can be lost, potentially leading to postoperative complications. Structured handoffs can increase the transfer of knowledge during intraoperative anaesthesia handoffs, improving their quality. We therefore propose to test the primary hypothesis that a semi-structured intraoperative anaesthesia handoff cognitive aid reduces the number of serious 30-day complications in s

Critical patient care information is often omitted or misunderstood during handoffs, which can lead to inefficiencies, delays, and sometimes patient harm. We implemented an aviation-style post-anesthesia care unit (PACU) handoff checklist displayed on a tablet computer to improve PACU handoff communication. We developed an aviation-style computerized checklist system for use in procedural rooms and adapted it for tablet computers to facilitate the performance of PACU handoffs. We then compared the proportion of PACU handoff items communicated before and after the implementation of the PACU han

BACKGROUND AND OBJECTIVE: Handoff communication errors are a leading source of sentinel events. We sought to determine the impact of a handoff improvement programme for nurses.
METHODS: We conducted a prospective pre-post intervention study on a paediatric intensive care unit in 2011-2012. The I-PASS Nursing Handoff Bundle intervention consisted of educational training, verbal handoff I-PASS mnemonic implementation, and visual materials to provide reinforcement and sustainability. We developed handoff direct observation and time motion workflow assessment tools to measure: (1) quality of the

BACKGROUND: Nursing handoff is a critical process in which essential patient and treatment-related information, as well as responsibilities, are exchanged to ensure quality of care and continuity of treatment. Errors during this process may lead to adverse clinical outcomes and compromise patient safety. This study aimed to systematically review the overall trends of and factors contributing to nursing handoff errors in clinical settings to enhance nurses’ awareness and understanding. METHODS: Following a study protocol pre-registered with the International Prospective Register of Systematic R

INTRODUCTION: Emergency department (ED) to operating room (OR) handoffs for injured patients requiring operative intervention represent a critical junction in patient care. Numerous models and checklists have been developed to guide an effective handoff, but studying adherence is challenging secondary to a lack of available data sources. In 2024, our center initiated an intraoperative trauma video review program as part of our quality improvement efforts, effectively providing a granular source of data to study this issue. We sought to describe variability in the timing and content of ED-to-OR

AIMS AND OBJECTIVES: To qualitatively investigate the medical-surgical nurse shift handoff as a process within the workflow of the exchanging nurses. Specifically, this study sought to identify the ideal handoff, ways the handoff deviated from ideal, and subsequent effect on nursing care.
BACKGROUND: The functions as well as information content of the handoff have been studied. However, typical studies look at the handoff as an isolated activity utilising nurse perceptions as the primary measure of quality.
DESIGN: Semi-structured focus groups were conducted to discuss nurses' perspectives o

Shift-to-shift handoffs refer to the process of transferring role and responsibility for providing care from one person to another, thus insuring continuity of care. Through focus groups of residents and supervising physicians, we studied how physicians select patient cases to discuss during handoffs. We also compared the selection across level of experience. Understanding the patient selection criteria can give us insight into how to improve handoffs, in particular using supportive technologies that are integrated into the clinical information system. Studying the actual handoff process and n

OBJECTIVES: Interfacility transfers from frontline settings to the PICU can place children at risk for harm due to handoff miscommunication. We aimed to evaluate the effectiveness of a structured interfacility handoff program in improving communication between referring and receiving clinicians and across the PICU admitting team.
DESIGN: Prospective intervention study using a mixed-methods approach (surveys, chart review, and semistructured interviews) before and after I-PASS-to-PICU implementation.
SETTING: Academic tertiary referral PICU.

BACKGROUND: Hospital handoffs, including shift changes and transfers across units, are vulnerable points in care delivery where communication failures may lead to information loss and preventable adverse events. Although teamwork is a well‑established driver of handoffs, prior research suggests that its effectiveness varies across organizational contexts. Organizational Learning and Continuous Improvement (OL-CI)-defined by routine review of work processes, testing of changes, and prevention of recurring problems-may strengthen the degree to which teamwork translates into more reliable handoff
