Patient harm from cardiovascular medications.

BACKGROUND: Medication harm can lead to hospital admission, prolonged hospital stay and poor patient outcomes. Reducing medication harm is a priority for healthcare organisations worldwide. Recent Australian studies demonstrate cardiovascular (CV) medications are a leading cause of harm. However, they appear to receive less recognition as 'high risk' medications compared with those classified by the medication safety acronym, 'APINCH' (antimicrobials, potassium, insulin, narcotics, chemotherapeutics, heparin). Our aim was to determine the scale and type of medication harm caused by CV medicati

Medication-related patient harm in New Zealand hospitals.

AIM: The purpose of this study is to identify patterns of medication-related harm from a national perspective, and to use this information to inform decisions on where to focus medication safety efforts. This study updates a 2013 study using the same methodology. METHOD: District health boards (DHBs) still actively using either the Adverse Drug Event (ADE) Trigger Tool (TT) or the Global Trigger Tool (GTT), submitted two years of anonymised ADE data (1 July 2013-30 June 2015) to the Health Quality & Safety Commission (the Commission) using a standard template. Analyses were conducted using ag

Invisible harm in patient safety: a framework and definition for preventable psychological harm in cancer care.

BACKGROUND: While patient safety is receiving increasing attention in healthcare services research and policies, it is mainly centred around prevention of physical harm. Preventable psychological harm (PPH) remains invisible in reports and quality measurements. As patients with cancer are particularly vulnerable due to the severity of their condition and therapies, they are exposed to risks such as non-physical adverse events. Recently, incidents of psychological harm have gained more attention in patient safety research, but a common and accepted definition and classification are missing. AI

Patient Harm From Orthodontic Care (Part II): Incident Analysis of Contributory Factors and Perceived Avoidability of Harm.

BACKGROUND: Patient safety, defined as the absence of avoidable harm, in orthodontics is still in its early stages of development. Knowledge on the subject is limited. AIM: (1) To conduct an incident analysis, (2) to measure avoidability, (3) to investigate the relationship between contributory factors and perceived avoidability of harm from orthodontic care, and (4) to suggest methods to mitigate potential patient harm incidents. MATERIALS AND METHODS: Data were retrospectively collected from consecutively filed orthodontic-related claims from the Danish Dental Compensation Association (DDC

Hospitable harm: When patient experience efforts compromise clinical integrity.

Efforts to improve patients' hospital experience can reduce uncertainty and indignity, but they can also create "hospitable harm" when pressure for smoother encounters encourages low-value care, preferential access, or compromises in safety, stewardship, or equity. This Perspective introduces the Experience-Integrity Matrix as a conceptual model for examining such initiatives. One axis asks whether an initiative makes care more reliable and dignified for patients; the other asks whether it aligns with clinical integrity. The four quadrants indicate what leaders should scale, repair, decline, o

Underdosed generic specialty medications: A prescription for patient harm?

The use of generic specialty medications amongst individuals with multiple sclerosis (MS) has expanded due to an increase in the number of available agents. We describe a woman who was denied continued use of brand name teriflunomide (AubagioⓇ), despite being clinically stable for 2.5 years, and switched to generic teriflunomide. She experienced a significant spinal cord exacerbation within a few months of starting treatment. We analyzed 3 generic teriflunomide agents, including the one used for treatment, in addition to AubagioⓇ. The generic teriflunomide used by our patient contained 55.5 %

Patient harm events: hospital-onset bacteremia vs CMS-reportable events.

National Healthcare Safety Network developed a hospital-onset bacteremia (HOB) reporting module which may be used for Centers for Medicare and Medicaid Services (CMS)-reimbursement in the future. This retrospective study compared CMS-reported, HOB, and Overlap events. There were twice as many HOB as CMS events, but >40% of HOB were due to infections that lack defined infection prevention practices.

First do no harm: integrating patient safety and quality improvement.

BACKGROUND: Lehigh Valley Hospital's (LVH's; Allentown, Penn) interdisciplinary quality improvement program Primum Non Nocere (PNN), or First Do No Harm, is composed of 12 quality improvement (QI) projects that are a combination of ongoing operations improvement projects and new projects in patient safety. The projects stress delivery of cost-effective medical care while reducing preventable adverse events through improved communication, process redesign, and evidence-based protocol use. EXAMPLE: WRONG-SITE SURGERY: In response to an initial alert warning in 1998, LVH developed a policy of ma

Deliberate self-harm in a patient with hyperthyroidism with acute psychosis.

A woman in her 30s with underlying Graves' disease, who recently completed radioactive iodine treatment, presented with 2 weeks of acutely altered behaviour associated with auditory hallucinations and religious preoccupations. Laboratory investigation demonstrated elevated free thyroxine levels and suppressed thyroid-stimulating hormone levels. Additionally, there was a presence of antithyroid peroxidase antibodies consistent with autoimmune thyroid disease. She responded to antipsychotics and achieved biochemical euthyroidism. Subsequently, antipsychotic was tapered off during outpatient foll

Patient-safe radiological language: Reducing nocebo harm in orthopedic imaging reports.

As patients increasingly access radiology reports through electronic portals, imaging reports are no longer private technical communications between clinicians. They have become patient-facing texts that can shape how people understand pain, ageing, bodily damage, activity, and treatment choices. In orthopedics, terms such as degeneration, tear, collapse, compression, severe narrowing, and metaphors such as bone-on-bone may be technically familiar to clinicians but frightening or misleading to patients. This discussion paper argues that orthopedic imaging language should be considered part of

An overlooked alliance: using human factors engineering to reduce patient harm.

BACKGROUND: Although human factors engineering (HFE) is considered only in relationship to the design of medical devices or information systems technology, human factors issues arise in many aspects of work in health care organizations. HFE ANALYSIS: In one scenario, the resuscitation stretcher would not pass through the ED door closest to radiology. Many clinical work spaces were never formally designed for the work currently being performed in them; instead, they were adapted from existing space originally designed for a different use. In a second scenario, infusion pump malfunction was not

Adjusting surgical mortality rates for patient comorbidities: more harm than good?

Background. Studies of medical admissions have questioned the validity of using claims data to adjust for preexisting medical conditions (comorbidities), but the impact of using comorbidities from claims data to risk-adjust mortality rates for high-risk surgery is not well characterized. The purpose of this study was to evaluate the relationship between comorbidities and mortality in administrative data in surgical populations and identify better risk-adjustment methods. Methods. Using the national Medicare database (1994-1997), we identified admissions for elective abdominal aortic aneurysm r

Patient privacy versus protecting the patient and the health system from harm: a case study.

A 71-year-old man who presented to hospital with chest pain and a history of cardiovascular disease was repeatedly hospitalised over the course of a month for care that included multiple investigations, intensive care, transfer to and from a metropolitan hospital, discharge, and readmissions for collapse, hemiparesis, and vision change. The medical team excluded underlying disease related to his initial chest pain and subsequent neurological symptoms. A search for (undisclosed) prior hospitalisations revealed multiple previous admissions and invasive investigations at hospitals across Australi

Opioid Overdose Harm Prevention: The Role of the Nurse in Patient Education.

Opioid overdose continues to affect thousands each year in the United States, with nearly 850,000 lives lost within the last 20 years. It will take a comprehensive and coordinated approach from all members of the health care team and health care institutions, in addition to governmental officials, public safety, and community organizations to mitigate this crisis. Nurses can be instrumental in educating patients, families, and community members about ways to combat this epidemic, instrumental in advocating for their patients, advocating for reform, as well as continuing to bring awareness to t

Between doubt and diagnosis: Patient experiences of emotional harm from diagnostic delays.

BACKGROUND: Diagnostic error, including delayed diagnosis, is a significant threat to patient safety. While much research has focused on clinical and system-level contributors to diagnostic delay, less is known about patient perspectives, particularly regarding the emotional and psychological impacts. METHODS: We conducted in-depth, narrative interviews with 23 patients who self-reported experiencing a delayed diagnosis in one of five medical conditions: preeclampsia, myocardial infarction, ankylosing spondylitis, sepsis, and lung cancer. Participants were recruited via online patient communi
