[POSITIONING OF IMMOBALE NEUROLOGIC PATIENT].

Damage to the central nervous system leads to the loss of motor control, loss of consciousness, sensory, cognitive and perceptive dysfunction. Patients are immobile in the early phase of recovery, therefore therapeutic approach demands appropriate methods of patient positioning in bed. The positioning has to ensure conditions that will stimulate and promote functional rehabilitation and prevent complications of immobility. The positioning has to rely on functional assessment of the patient problem, while it should also be therapeutic and individually adjusted to the patient needs. In the metho

Proper positioning eliminates patient injury.

1. Pre-existing disorders such as poor nutritional state, diabetes, respiratory disorders arthritis, and cardiac status compound the effects of positioning. 2. Under anesthesia, the patient may be placed in a potentially injurious position that he could protect himself against otherwise. 3. Position influences respiration by mechanical restriction of the ribs and sternum, and by reduced ability of the diaphragm to push down against the abdominal contents. 4. Informed involvement of the operating room nurse will minimize risk to the patient who is defenseless under anesthesia.

Patient Positioning for Postless Hip Arthroscopy.

Hip arthroscopy has been on the rise since its inception. Initial descriptions of the procedure required skeletal traction of the operative extremity with countertraction provided by a perineal post. Perineal posts are associated with complications such as perineal nerve palsy, genital laceration, and hematoma formation. Postless traction has been developed to avoid complications related to use of perineal posts. A patient is positioned supine on a postless traction table (Guardian; Stryker, Greenwood Village, CO) with a disposable traction pad. The anterior superior iliac spine is positioned

Patient positioning for subclavian vein catheterization.

The subclavian vein is a commonly used venous access route. Percutaneous cannulation is associated with complications and malpositioning of the catheter. We studied the potential effect of various body positions on subclavian vein catheterization using gross anatomic dissection and magnetic resonance imaging. Findings show that traditional recommendations for patient positioning with the shoulders retracted and the head turned away can act to distort the subclavian vein anatomy and make successful cannulation more difficult. Positioning the patient flat with the head and shoulders in a neutral

[Necrotizing fasciitis: complication of patient positioning?].

Necrotizing fasciitis is a hypodermis, muscular fascia then dermis necrotizing infection. It disseminates along fascias with a mortality sometimes within 18 hours. The average mortality, reported in the literature, is about 30%. A 65-year-old man, with a history of Vaquez disease (under hydroxurea) and a smoke addiction, had an epidermoid carcinoma of the left vocal cord (T2 N0 M0). The cancer treatment consisted of a functional lymph node excision, followed by tracheotomy then by partial laryngectomy. At the end of the intervention, after removal of operative fields, it was noticed that the M

A comparison of fluoroscopically controlled patient positioning and conventional positioning, including comparative dosimetry.

The size of the area of exposure on the skin during excretory urography was determined by either technician-controlled fluoroscopy or conventional assessment in 2 comparable groups of patients. The mean skin exposure to radiation per patient was the same in each group. The time required for a urogram was 10% less when fluoroscopy was employed. Since accurate, tightly shuttered exposure areas improve radiographic quality, fluoroscopically controlled determination of patient positioning is justified.

Biomechanical study of patient positioning: influence of lower limb positioning on spinal geometry.

OBJECTIVE: The objective of this study was to use finite element model (FEM) simulations and experimental testing to study the relationship between lower limb positioning for surgeries of the spine and changes in sagittal curves. METHODS: Four volunteers underwent lower limb flexibility and range of motion testing before being placed prone on a new surgical frame where lateral radiographs of their spines were taken in positions of hip flexion (average 48 degrees) and extension (average 13 degrees). Personalized FEMs were created representing each volunteer's spine, rib cage, pelvis, and lower

Influence of tube and patient positioning in thoracoabdominal CT examinations on radiation exposure-towards a better patient positioning.

Although iso-centric patient positioning is enormously important in computed tomography (CT), it is complicated in thoracoabdominal imaging by the varying dimensions of the body. Patient positioning can affect the appearance of the patient on the localiser. Positioned too close to the x-ray tube, a patient appears considerably more voluminous. The goal of this study is to assess the difference in radiation exposure of combined chest and abdomen CT scans between scans with prior 0°- and 180°-localisers in conjunction with patient positioning. In this IRB-approved retrospective study, patients w

Pressure ulcer prevention, patient positioning and protective equipment.

Pressure ulcer (PU) prevention is a high priority for health professionals in all settings. While PU incidence is expected to rise given the ageing population in the UK and Europe, PUs can occur in people of any age. It is therefore imperative that all practitioners know about the causes and consequences of PUs and be aware of up-to-date guidance on the prevention and management in patients who have developed a PU or who are at risk of developing a PU. Health professionals should also be aware of preventive equipment highlighted in national and local guidance, prevention strategies and protoco

Positioning of a morbidly obese patient during retinal surgery.

A body mass index (BMI) ≥35 kg/m2 is classified as obese, and a BMI ≥40 kg/m2 is classified as morbidly obese. Obese people are at a higher risk for developing cardiovascular complications like ischemic heart diseases, congestive cardiac failure, hypertension, type 2 diabetes mellitus and obstructive sleep apnea (OSA) among other health issues. Central obesity can also increase the pleural pressure and cardiac filling pressures, thus increasing the intracranial (ICP) and intraocular pressure (IOP). These clinical co-morbidities can make retina surgeries, which require patient in supine positio

Prospective assessment of patient morbidity from prone sacral positioning.

OBJECT: Sacrectomy positioning must balance surgical exposure, localization, associated operative procedures, and patient safety. Poor positioning may increase hemorrhage, risk of blindness, and skin breakdown. METHODS: The authors prospectively identified positioning-related morbidity in 17 patients undergoing 19 prone sacral procedures from September 2008 to August 2009 following institution of a standardized positioning protocol. Key elements include skull traction/head suspension, an open radiolucent frame, and wide draping for associated closure and reconstructive procedures. RESULTS: T

Dose-guided patient positioning in proton radiotherapy using multicriteria-optimization.

Proton radiotherapy (PT) requires accurate target alignment before each treatment fraction, ideally utilizing 3D in-room X-ray computed tomography (CT) imaging. Typically, the optimal patient position is determined based on anatomical landmarks or implanted markers. In the presence of non-rigid anatomical changes, however, the planning scenario cannot be exactly reproduced and positioning should rather aim at finding the optimal position in terms of the actually applied dose. In this work, dose-guided patient alignment, implemented as multicriterial optimization (MCO) problem, was investigated
