Animal-assisted therapy in pediatric urodynamics.

INTRODUCTION/BACKGROUND: Urodynamics (UDS) is associated with high levels of patient anxiety/discomfort. Children are often unable to complete UDS, with anesthesia needed to place catheters. Animal-assisted therapy (AAT) has been used in a variety of settings, but it has not been studied for UDS before. OBJECTIVE: To determine if AAT can increase success of completing UDS testing without anesthesia in children who were previously unable to perform UDS, along with decreasing distress levels. STUDY DESIGN: We performed a pilot case series of 7 patients (2 female, 5 male) aged 4-16 (mean 9.7) y

Pediatric Robotic Assisted Laparoscopic Pyeloplasty.

Ureteropelvic junction obstruction (UPJO) is a common cause of hydronephrosis in the pediatric population. Management with an open dismembered pyeloplasty was first described by Anderson-Hynes in 1949; minimally invasive approaches have been increasingly utilized. In the subsequent text and accompanying video, we review the technique for dismembered robotic assisted laparoscopic pyeloplasty in the pediatric population. Large retrospective series demonstrate an over 90% success rate for robotic assisted laparoscopic pyeloplasty. Given success and complication rates are similar to an open approa

Recent Advances in Pediatric Ventilatory Assistance.

In this review on respiratory assistance, we aim to discuss the following recent advances: the optimization and customization of mechanical ventilation, the use of high-frequency oscillatory ventilation, and the role of noninvasive ventilation. The prevention of ventilator-induced lung injury and diaphragmatic dysfunction is now a key aspect in the management of mechanical ventilation, since these complications may lead to higher mortality and prolonged length of stay in intensive care units. Different physiological measurements, such as esophageal pressure, electrical activity of the diaphrag

Robot-assisted procedures in pediatric neurosurgery.

OBJECTIVE During the last 3 decades, robotic technology has rapidly spread across several surgical fields due to the continuous evolution of its versatility, stability, dexterity, and haptic properties. Neurosurgery pioneered the development of robotics, with the aim of improving the quality of several procedures requiring a high degree of accuracy and safety. Moreover, robot-guided approaches are of special interest in pediatric patients, who often have altered anatomy and challenging relationships between the diseased and eloquent structures. Nevertheless, the use of robots has been rarely r

A computer model of the pediatric circulatory system for testing pediatric assist devices.

Lumped parameter computer models of the pediatric circulatory systems for 1- and 4-year-olds were developed to predict hemodynamic responses to mechanical circulatory support devices. Model parameters, including resistance, compliance and volume, were adjusted to match hemodynamic pressure and flow waveforms, pressure-volume loops, percent systole, and heart rate of pediatric patients (n = 6) with normal ventricles. Left ventricular failure was modeled by adjusting the time-varying compliance curve of the left heart to produce aortic pressures and cardiac outputs consistent with those observed

Video-Assisted Thoracoscopy in Pediatric Thoracic Trauma.

Pediatric thoracic trauma is a major cause of pediatric morbidity and mortality. Pediatric patients have unique anatomy and physiology that account for differences in injury patterns when compared to adult patients. These differences warrant special consideration in injury recognition and intraoperative management. The initial management of pediatric thoracic trauma should follow the principles of the primary survey in that life-threatening injuries should be ruled out. Hemodynamically unstable patients with serious thoracic injury warrant thoracotomy for expeditious management of life-threate

Developments in Pediatric Ventricular Assist Device Support.

Mechanical support devices have revolutionized the management of circulatory failure. This has been met by a dramatic increase in ventricular assist device (VAD) utilization in children over the last two decades. Previous wide gaps in experience and knowledge have significantly narrowed in the recent years. As we continue to gain experience with this technology, we face new challenges such as complex congenital circulations and small patient size. The emergence of new pharmacologic therapies and device technology offers more opportunities and requires constant adjustment in practice. As we con

Pulsatile mechanical cardiac assistance in pediatric patients with the Berlin heart ventricular assist device.

Mechanical cardiac assistance for neonates, infants, children and adolescents may be accomplished with pulsatile ventricular assist devices (VAD) instead of extracorporeal membrane oxygenation or centrifugal pumps. The Berlin Heart VAD consists of extracorporeal, pneumatically driven blood pumps for pulsatile univentricular or biventricular assistance for patients of all age groups. The blood pumps are heparin-coated. The stationary driving unit (IKUS) has the required enhanced compressor performance for pediatric pump sizes. The Berlin Heart VAD was used in a total number of 424 patients from

Homelessness in Pediatric Populations: Strategies for Prevention, Assistance, and Advocacy.

Recent data indicate that homelessness among pediatric and adolescent populations is significantly higher than previous studies and point-in-time counts indicate. Pediatricians and other health care providers often see children and youth who are at risk of or are currently experiencing homelessness, but may not be aware of their status. This article summarizes current definitions of homelessness and data on common health issues for pediatric patients. Information on how to recognize and help those experiencing homelessness as well as areas for continued advocacy is shared.
