PURPOSE OF REVIEW: Since the start of the HIV pandemic, systemic infection with Penicillium marneffei has developed from a very rare diagnosis to the third most common opportunistic infection in HIV co-infected patients in South East Asia. Penicillium marneffei is endemic in Southeast Asia, the Guangxi province of China, and Hong Kong. Cases of patients infected with P. marneffei have been very rare, but the incidence has increased markedly during the past several years. This increase is exclusively due to infection among patients infected with human immunodeficiency virus (HIV). We report an outbreak investigation of fungemia due to Penicillium species after prolonged flooding of a Thai hospital. Contaminated rubber diaphragms of blood culture bottles were identified, and the pseudo-outbreak was resolved after environmental cleaning, use of high-efficiency particulate air filtration, and strict compliance with basic infection control practices for blood culture procurement.

CLINICAL CASE: We present a case of conjunctivitis in a diabetic patient in whom not improvement was observed after empiric treatment, and finally developed a conjunctival granuloma. Culture of the conjunctival exudate and histological examination of the conjunctival biopsy confirmed the infection by Penicillium sp. BACKGROUND/PURPOSE: Penicillium marneffei is an emerging opportunistic pathogen. The goal of this study was to study its clinical and radiographic presentation, and the diagnostic value of a cytological study of penicilliosis.
METHODS: A total of 24 patients with penicilliosis were found by culture in an 8-year period. Thirteen patients had cytological examination, which were retrospectively reviewed to analyze the morphological characteristics of P. marneffei and diagnostic yields of different cytological specimens.

We report a case of necrotizing pneumonia due to Penicillium chrysogenum in a 57-year-old woman operated on for lung cancer. The residual right lower pulmonary lobe was infiltrated by Penicillium chrysogenum. The patient underwent a second pulmonary right lobectomy and was successfully treated with oral itraconazole. To our knowledge, this is the first case of pneumonia due to P. chrysogenum.

Coremia of Penicillium claviforme develop in three stages; primordium formation, elongation, and sporulation. Primordium formation was induced by external nutrients, while starvation initiated the differentiation of primordia into coremia with sporeheads. There is strong evidence that external nutrients are not taken up during this differentiation. Continued sporulation by mature coremia again required an external nutrient supply.

A 36-year-old HIV-positive man presented with symptoms of redness, blurred vision and foreign body sensation in his right eye for 3 months. The slit lamp examination revealed deep stromal infiltration with a feathery margin in an otherwise minimal anterior chamber reaction. A corneal scraping was negative. Confocal microscopy demonstrated an abnormal large hyper-reflective oval shape in the corneal stroma.
