[Anteromedial thigh perforator flap an ideal alternative flap for anterolateral thigh flap without suitable perforator].

PURPOSE: To find a reliable alternative flap with minimal donor site morbidity for anterolateral thigh flap (ALT) without suitable perforator. METHODS: From December 2007 to February 2011, 2 of 64 patients underwent tongue and floor of mouth reconstruction with an anteromedial thigh flap with the initial intent of using an anterolateral thigh flap. One was a perforator flap and the size was 7cm×4cm , the other was a perforator-based chimeric flap including a slender of rectus femoris island and a 14cm×6cm skin paddle. Both of the AMT flap pedicles were medial branch of descending branch of la

Perforator patterns of the ulnar artery perforator flap.

BACKGROUND: Flaps based on the ulnar artery have never gained the same popularity as the radial forearm flap, despite several potential advantages. In this article, the authors describe a true ulnar artery perforator flap with perforator mapping. METHODS: Thirty-eight consecutive patients who underwent ulnar artery perforator flap surgery were included in the study. The size, number, and location of perforators were recorded intraoperatively. Preoperative and postoperative grip strength was tested and compared. RESULTS: One to three cutaneous perforators from the ulnar artery were identified

The sternocleidomastoid perforator flap.

The conventional pedicled sternocleidomastoid (SCM) flap has a poor arc of rotation, limited volume and precarious vascularity. This report describes a new technique for raising a SCM flap based on the perforating vessels of the superior thyroid vascular pedicle. The upper and lower attachments of the sternocleidomastoid muscle are divided. Four medically and/or surgically compromised patients have successfully undergone reconstruction of hemiglossectomy (1), partial glossectomy (1) and rim of mandible (2) defects for malignancy. The arc of rotation of the SCM flap is greatly increased and the

Radial artery perforator flap.

Soft tissue defects in the hand and wrist can be challenging problems for the hand surgeon. The retrograde radial forearm fasciocutaneous flap has emerged in recent years as the workhorse flap to cover many hand and wrist defects. However, recognition of the intrinsic limitations of this flap has led to the development of other alternative flaps to provide soft tissue coverage for this region. The radial artery perforator flap has many of the benefits of the radial forearm flap but minimizes the disadvantages, such as the need to sacrifice the radial artery, color and bulk mismatch of the flap

Perforator flap breast reconstruction.

A perforator flap consists of skin and fat harvested from a donor site nourished by myocutaneous perforators while sparing the donor muscle for function and strength. This flap type has revolutionized microvascular free tissue transfer and the technique has been successfully applied to well-established donor sites for autologous breast reconstruction namely the lower abdomen, upper and lower buttock, the upper back, and the lateral thigh. Although these flaps are technically more demanding than conventional myocutaneous free flaps, their minimal impact on the donor site muscle function signifi

Perforator topography of the thoracodorsal artery perforator flap.

BACKGROUND: The anatomical topography of the perforators that pierce the latissimus dorsi muscle fascia determines the location and direction of the skin paddle and thus the ultimate design of the thoracodorsal artery perforator flap. The full scope of the thoracodorsal artery perforator topography has not been provided in detail, and the usefulness of perforators from the transverse branch has not been elucidated. METHODS: While harvesting the thoracodorsal artery perforator flap, every reliable musculocutaneous perforator that derives from the thoracodorsal artery was documented. The number

[The infragluteal perforator flap].

PURPOSE: Pressure sores of the ischial tuberosities are mostly covered with fascio- or myocutaneous flaps of the posterior thigh. In doing so, vascular territories of adjacent flaps are violated, which are not available any more in case of ulcer recurrences. In consideration of the high recurrence rates of pressure sores in paraplegics, we developed an infragluteal perforator flap that spares adjacent vascular territories. PATIENTS/MATERIAL AND METHOD: Infragluteal perforator flaps were dissected in five fresh human cadavers to investigate the anatomic relations of the cutaneous branches of t

Posterior Arm Artery Perforator Flap: An Ultrafast and Reproducible Perforator Flap Dissection Technique.

Axillary reconstruction after wide excision for hidradenitis suppurativa (HS) remains challenging, with no consensus regarding the optimal technique. Although multiple reconstructive options exist, many are limited by prolonged operative time, imperfect tissue match, functional restriction, or significant donor-site morbidity. The posterior arm artery perforator (PAAP) flap offers a reliable and efficient alternative due to its constant anatomy, thin and pliable tissue, and low donor-site morbidity. We present a standardized, landmark-based, Doppler-free, ultrafast harvesting technique, design

Lateral thoracic perforator flap: additional perforator flap option from the lateral thoracic region.

BACKGROUND: Perforator flaps from the lateral thoracic region have not been as popular as other donor sites because of the misconception that the vascular anatomy in this region is less than predictable. However, the skin over the lateral thoracic region is vascularised by three rows of perforators of varied vascular dominance. Two perforator flaps from this region based on the middle and the posterior row of perforators from the thoracodorsal artery have been described. The lateral thoracic perforator flap based on the anterior row of perforators is another useful option. PATIENTS AND RESULT

Another option of perforator flap in the lateral thoracic area: lateral thoracic perforator flap.

BACKGROUND: The lateral thoracic donor site provides two types of perforator flap; the latissimus dorsi perforator flap based on the musculocutaneous perforator, and the thoracodorsal perforator flap based on the septocutaneous perforators from the thoracodorsal artery. In this article, we introduce a direct cutaneous perforator derived from the lateral thoracic artery, which provides another option for harvesting a perforator flap from the lateral thoracic region. PATIENTS AND METHODS: Overall 17 patients underwent reconstructions using the lateral thoracic perforator flap for defects result

Superficial ulnar artery perforator flap.

Superficial ulnar artery is a rare finding but shows significant surgical implications. Its thinness and pliability make this flap an excellent solution for soft tissue reconstruction, especially in the head and neck region. We hereby report a successful free superficial ulnar artery perforator forearm flap transfer for tongue reconstruction. A 64-year-old man presenting with a squamous cell carcinoma of the left tongue underwent a wide resection of the tumor, left radical neck dissection, and reconstruction of the tongue and the left tonsillar pillar with the mentioned flap. No complications

Medial sural artery perforator flap.

In the last decade, the medial sural artery perforator flap (MSAP) has emerged as one of the most popular reconstructive options for multiple body sites. The versatility of the flap hinges on the ability to harvest multiple tissue components in various combinations but from the same wound. The flap can be used as a pedicled, free, or chimeric design, or even may be used in sequential order. Anatomic anomalies of these perforators are rare compared with other perforator flaps, but well-prepared preoperative planning and the identification of perforators remain the cornerstone of successful flap

Innervated digital artery perforator flap.

PURPOSE: To describe a technique for covering defects of the fingertips: the innervated digital artery perforator (IDAP) flap. METHODS: A total of 17 patients were treated with an IDAP flap. The size of the flaps varied between 2 ×1 cm and 3.5 × 2 cm. Postoperative evaluation of the patients consisted of the Semmes-Weinstein Monofilament test, static 2-point discrimination, patient satisfaction, extension loss, and an investigation into complications. RESULTS: All IDAP flaps survived completely, and no patients required secondary interventions. The mean follow-up period was 7 months (range,

Tensor fasciae latae perforator flap.

The tensor fasciae latae perforator flap and a new technique, named microdissection, for one-stage accurate formation of a thin flap were presented. Microdissection enables the perforator in the adipose tissue to be used as a lengthened pedicle, and the flap can be transferred without adding and reducing excess fat tissue to the recipient and donor sites. This microdissected thin tensor fasciae latae perforator flap can be used conveniently in many aspects of reconstructive surgery, especially in cases of severe bum scar contracture of the extremities.

Development of the pectoral perforator flap and the deltopectoral perforator flap pedicled with the pectoralis major muscle flap.

PURPOSE: Because thinning of the pectoralis major myocutaneous flap is impossible due to blood circulation, it is difficult to produce thin flaps. Although the pectoral flap and the deltopectoral flap are the best flaps that provide a highly desirable color-texture match to facial skin, their reach is restricted and they require resection in 2 stages. The purpose of this paper is to develop a new method of elevating a flap and to resolve these problems. METHODS: First, include the third intercostal perforating branch of the internal thoracic artery in the skin paddle and, outward therefrom, d

Pre-expanded Paraumbilical Perforator Flap.

The paraumbilical perforator flap is the first and the most famous perforator flap. Pre-expansion increases the flap dimension and reduces the flap thickness and donor site morbidities, making the paraumbilical perforator flap a more effective option for upper extremity reconstruction. Pre-expanded pedicled paraumbilical perforator flaps can achieve excellent function and aesthetic outcomes in patients with extensive scar contracture and giant melanocytic nevi in the upper extremity. Although this technique requires multiple procures, each operation is relatively simple and has a low complicat

Perforator-based forearm island flap.

Reconstruction of soft tissue defects of the hand with exposed tendons, nerves, joints and bones could be quite a challenge for the surgeon. Only adequate coverage can save their functions. In our case we present a patient with adduction contracture of the right thumb after II degree burns, treated with perforator-based forearm flap. This flap relies only on perforating branches (5 to 11 in number) arising from the radial artery and located proximal to the radial styloid (2 to 7 cm). The main advantages of this method are that it is a single-stage operative technique, it is applicable in emerg

Free gluteal artery perforator flap with a short, small perforator.

The introduction of supermicrosurgery has led to the development of a new gluteal perforator flap nourished only by a musculocutaneous perforator of the superficial gluteal artery system. This flap has a perforator that is short (3-4 cm in length) and small (less than 1 mm). The successful transference of a free gluteal perforator flap for the coverage of soft-tissue defects in the foot and face in two patients is described in this article. With this flap, deeper and longer dissection for a pedicle vessel is unnecessary, flap elevation time is shorter, thinning of the flap with primary defatti
