The serial changes of perfusion index in preterm infants with patent ductus arteriosus: is perfusion index clinically significant?

BACKGROUND: Perfusion Index (PI) which reflects the peripheral blood flow may help early detection and treatment decision of hemodynamically significant patent ductus arteriosus (hsPDA) in preterm infants. The present study is designed to analyze the usefulness of PI level in early detection of hsPDA in preterm infants. METHODS: Preterm infants born before 36 gestational weeks were assessed for PI and simultaneous echocardiography. Based on echocardiography, each infant is categorized into no-PDA (group 1), non-hsPDA (group 2) and hsPDA (group 3). Heart rate (HR), mean arterial pressure (MAP)

Application of the perfusion index in obstetric bleeding.

OBJECTIVE: We assessed the utility of the pulse oximeter perfusion index (PI) in maternal monitoring immediately after delivery. METHODS: We examined 30 pregnant women without any complications using the Rad7 device at delivery. The correlations between heart rate (HR), systolic blood pressure (BP), oxygen saturation SpO2, PI, Pleth variability index (PVI), shock index and blood loss were assessed. RESULTS: Blood loss at 20-min postpartum was not correlated with the difference in heart rate, systolic BP, SpO2, shock index or PVI taken immediately after delivery and at 20-min postpartum, but

Accuracy of Perfusion Index and Perfusion Index Ratio as a Predictor of a Successful Low Interscalene Brachial Plexus Block: A Prospective Observational Study.

BACKGROUND: The perfusion index has previously been used to predict the success of brachial plexus blocks but to our knowledge has not yet been studied for the low interscalene block approach. This study evaluated the accuracy of the perfusion index and perfusion index ratio in predicting the success of a low interscalene brachial plexus block in patients undergoing upper limb surgery. METHODS: The study included 70 patients undergoing upper limb surgeries under ultrasound-guided low interscalene brachial plexus block. After local anesthetic injection, patients' sensory and motor blocks were

Effects of different approaches of infraclavicular brachial plexus block on perfusion index and tissue perfusion.

BACKGROUND: Although both the lateral sagittal and costoclavicular approaches are applied at the cord level in the infraclavicular region, there is a major difference between the distributions of the two approaches. We aimed to investigate the effects of this different distribution on tissue perfusion and oxygenation. METHODS: Sixty patients undergoing elective elbow, forearm, wrist and hand surgery under infraclavicular brachial plexus block were included in the study. Patients were divided into two groups: group CC (costoclavicular approach, n = 30) and group LS (lateral sagittal approach,

Regional tissue perfusion index (RTPI): a new optical-based metric for quantifying regional tissue perfusion.

UNLABELLED: Accurate, continuous assessment of regional tissue perfusion remains a significant clinical challenge, as most existing modalities are invasive, indirect, or impractical for routine monitoring. Near-infrared spectroscopy (NIRS) has been widely adopted to assess tissue oxygenation; however, conventional NIRS-derived indices are insufficient surrogates for true perfusion and often fail to capture rapid hemodynamic changes. This study aimed to introduce and validate the Regional Tissue Perfusion Index (RTPI), a novel NIRS-derived metric that integrates multiple features of the NIRS si

Perfusion index and perfusion index ratio as predictive tools for block success: A prospective observational study on ultrasound-guided supraclavicular brachial plexus block.

BACKGROUND: The supraclavicular brachial plexus block is widely used for upper limb surgeries due to its effectiveness in providing anesthesia. Conventionally, success is gauged through sensory and motor assessments, which are not only subjective but also require active patient cooperation. This poses challenges for patients who are sedated or under general anesthesia. Additionally, repeated sensory testing can be uncomfortable. Therefore, an objective and noninvasive method to assess block success is needed. Perfusion index (PI), derived from pulse oximetry, reflects peripheral perfusion chan

Reliability of the hepatic perfusion index for the detection of liver metastases.

The reliability of the technique for measuring the hepatic perfusion index (HPI) for the detection of liver metastases has been examined. Dynamic images from 173 patients with primary and recurrent colorectal cancer have been analysed by two operators using the peak values of both the right and left kidney time-activity curves. In 14.4% of analyses the right kidney HPI method failed, while only 5.7% of analyses failed using the left kidney method. The results showed (a) that disagreement between the operators HPI values was small but statistically significant (p less than 0.05) and (b) that di

Plethysmographic Peripheral Perfusion Index: Could It Be a New Vital Sign?

The plethysmographic peripheral perfusion index (PPI) is a very useful parameter with various emerging utilities in medical practice. The PPI represents the ratio between pulsatile and non-pulsatile portions in peripheral circulation and is mainly affected by two main determinants: cardiac output and balance between sympathetic and parasympathetic nervous systems. The PPI decreases in cases of sympathetic predominance and/or low cardiac output states; therefore, it is a useful predictor of patient outcomes in critical care units. The PPI could be a surrogate for cardiac output in tests for flu

Perfusion index assessment during transition period of newborns: an observational study.

BACKGROUND: Perfusion index (PI) is becoming a part of clinical practice in neonatology to monitor peripheral perfusion noninvasively. Hemodynamic and respiratory changes occur in newborns during the transition period after birth in which peripheral perfusion may be affected. Tachypnea is a frequent symptom during this period. While some tachypneic newborns get well in less than 6 h and diagnosed as "delayed transition", others get admitted to intensive care unit which transient tachypnea of newborn (TTN) being the most common diagnosis among them. We aimed to compare PI of neonates with TTN a

The perfusion index of healthy term infants during transition at birth.

UNLABELLED: Perfusion index is a continuous parameter provided by pulse oximetry and might be useful for evaluating hemodynamic changes at birth and identifying transitional problems. The objective was to describe perfusion index values in term infants immediately after birth. Perfusion index of 71 healthy term born infants were recorded during the first 10 min after birth, using a pulse oximetry sensor placed preductally. A Wilcoxon signed-rank test was used to compare between time points. No significant trend in perfusion index could be observed in term-delivered infants. There was a signifi

Hepatic perfusion index in the diagnosis of overt metastatic colorectal cancer.

The hepatic perfusion index (HPI) was measured in 180 patients with colorectal cancer: 109 with primary colorectal cancer, 38 with suspected recurrent colorectal cancer and 33 following curative resection of colorectal cancer. In 21 patients with proven metastatic disease serial imaging studies were performed. HPI was determined using the peak of the left kidney time-activity curve to define the division of arterial and portal blood flow. HPI was elevated (greater than 0.37) in 54 of 115 patients (47%) with no evidence of hepatic metastases, 17 of 27 patients (63%) with hepatic metastases at i

The perfusion index as a noninvasive method for monitoring revascularized and replanted digits.

BACKGROUND: Monitoring replanted digits remains a challenging task in microsurgical units. Previous studies have indicated the perfusion index (PI), a parameter directly proportional to the blood flow, might be a useful tool. OBJECTIVE: The PI is evaluated as a monitoring tool in patients with replanted digits. METHODS: This prospective, non-interventional study includes 31 patients with revascularized or replanted digits. After successful revascularization or replantation, the PI and peripheral oxygen saturation of the affected finger and its contralateral equivalent were measured simultane

Relationship between perfusion index and patent ductus arteriosus in preterm infants.

BACKGROUND: Perfusion index (PI) is a noninvasive measure of perfusion. ΔPI (difference between pre- and postductal PI) may identify hemodynamically significant PDA. However, studies are limited to brief and intermittent ΔPI sampling. Our objective is to assess the value of continuous high resolution ΔPI monitoring in the diagnosis of PDA. METHODS: Continuous ΔPI monitoring in preterm infants was prospectively performed using two high-resolution pulse oximeters. Perfusion Index measures (ΔPI mean and variability, pre- and postductal PI) were analyzed over a 4-h period prior to echocardiograph

[The advances in the application of peripheral perfusion index in patients with septic shock].

Septic shock, a prevalent critical condition in intensive care units (ICU) and a major cause of patient mortality, is fundamentally attributed to microcirculatory dysfunction. Traditional macrocirculatory parameters are often insufficiently sensitive to reflect microcirculatory status. Consequently monitoring peripheral microcirculatory function holds crucial significance for assessing disease progression and evaluating therapeutic efficacy in septic shock. The peripheral perfusion index (PPI), obtained from a standard pulse oximeter, is based on photoplethysmography (PPG). It calculates the d
