Focal malakoplakia in chronic periapical periodontitis.

AIMS: Three cases of chronic periapical periodontitis including focal areas with malakoplakia changes are reported. METHODS AND RESULTS: These areas included both von Hansemann-type macrophages and periodic acid-Schiff-positive, iron- and calcium-containing concretions. Some concretions corresponded to spherules with a targetoid configuration, thus fitting the morphological criteria for classical Michaelis-Gutmann bodies. CONCLUSION: The vast majority of the cases of malakoplakia that have been reported in the literature corresponded to a characteristic, fairly homogeneous lesion, but a few

Decoding Cellular Landscapes: Unravelling Periodontitis and Periapical Periodontitis at the Single-Cell Frontier.

OBJECTIVE: Elucidate differences in key gene expression and cellular heterogeneity in alveolar bone between periodontitis (PD) and periapical periodontitis (PP) by analysing single-cell RNA sequencing data. METHODS: Single-cell RNA sequencing datasets for PD (GSE171213) and PP (GSE181688) were obtained from the Gene Expression Omnibus and analysed. Differentially expressed genes were identified in comparisons of PD versus healthy controls (HC), PP versus HC, and PD versus PP. Functional enrichment analyses were performed to explore heterogeneity. Key genes related to angiogenesis (ARGs), infl

[Histopathology and etiopathogenesis of chronic apical periodontitis--periapical granuloma].

Periapical lesions are among the most frequently diagnosed apical odontogenic pathologies in human teeth. The condition is generally described as apical periodontitis. Apical periodontitis is a sequel to endodontic infection and manifests itself as the host defense response to microbial challenge emanating from the root canal system to the periapical tissue. It is viewed as a dynamic encounter between microbial factors and host defenses at the interface between infected radicular pulp and periodontal ligament that results in local inflammation, resorption of hard tissues, destruction of other

[Endodontic treatment of chronic apical periodontitis without periapical medication].

The aim of this research was to evaluate the clinical course of chronic apical periodontitis (CAP), treated only endodontically, without periapical medication. Chronic apical periodontitis cases of pulpal origin were used as a material and treated endodontically as individual clinical cases. Clinical and statistical procedures were applied. The percentage of efficient or non-efficient cases was evaluated on the basis of radiographic comparison of treated CAP immediately after obturation, and six months later. Influences of various factors on therapy effect were analysed: radiographic condition

Determinants of Periodontal/Periapical Lesion Stability and Progression.

Periodontal and periapical lesions are infectious inflammatory osteolitytic conditions in which a complex inflammatory immune response mediates bone destruction. However, the uncertainty of a lesion's progressive or stable phenotype complicates understanding of the cellular and molecular mechanisms triggering lesion activity. Evidence from clinical and preclinical studies of both periodontal and periapical lesions points to a high receptor activator of NF-κB ligand/osteoprotegerin (RANKL/OPG) ratio as the primary determinant of osteolytic activity, while a low RANKL/OPG ratio is often observed

Periodontal repair of periapical lesions: the borderland between pulpal and periodontal disease.

A series of cases demonstrating the destruction of periapical periodontal structures, without pulpal involvement has been presented. Treatment using both surgery and antibiotics resulted in extensive healing without any concommitant endodontic therapy. The results suggest that lesions affecting the apical periodontium are either periodontal or pulpal in origin. Careful diagnosis allows the maintenance of pulp vitality in cases where apical destruction has a source other than an infected pulp. The commonly held belief that lateral and accessory canals are a significant source of pulpal contamin

Relationship between prostaglandin E2 concentrations in periapical exudates from root canals and clinical findings of periapical periodontitis.

In this study the relationship of prostaglandin E2 (PGE2) concentrations in periapical exudates with clinical findings of teeth with periapical periodontitis is discussed. Periapical exudate samples were obtained from root canals of 77 endodontically involved teeth during routine root canal treatment by the quantitative sampling method using paper points. PGE2 concentrations in periapical exudates (PE-PGE2) were determined by radioimmunoassay. Significantly higher levels of PGE2 were found in periapical exudates from teeth with radiolucent areas than from teeth without radiolucent areas (236.8

Periapical Lesion Grade and Sinus Mucosal Thickness in Severe Periodontitis.

INTRODUCTION AND AIMS: Severe periodontitis (Stage III-IV) frequently coexists with apical periodontitis, both of which can contribute to Schneiderian membrane thickening (SMT). This study quantifies the association between periapical lesion severity and SMT in a well-defined cohort of severe periodontitis, aiming to improve the preoperative risk assessment using cone-beam computed tomography (CBCT). METHODS: CBCT images from 330 patients diagnosed with Stage III-IV periodontitis complicated by apical periodontitis in maxillary posterior teeth were analysed. SMT was measured at standardised s

The transition from pulpitis to periapical periodontitis in dogs' teeth.

The purpose of the study was to histologically analyse transition from pulpitis to periapical periodontitis on dog's teeth. Pulps of mandibular premolars (37 roots) were exposed using a low-speed handpiece. Teeth were left open to the oral environment for 20, 35, 50 and 65 days. After the experimental period animals were sacrificed. Undemineralised teeth with surrounding bone, embedded in methylmetacrylate, were prepared for standard histological analysis. All teeth with pulpitis (five roots), regardless of the experimental period, had acute serose periapical periodontitis. All teeth (15 roots

The effect of periapical bone defects on stress distribution in teeth with periapical periodontitis: a finite element analysis.

BACKGROUND: Apical periodontitis directly affects the stress state of the affected tooth owing to the destruction of the periapical bone. Understanding the mechanical of periapical bone defects/tooth is clinically meaningful. In this study, we evaluate the effect of periapical bone defects on the stress distribution in teeth with periapical periodontitis using finite element analysis. METHODS: Finite element models of normal mandibular second premolars and those with periapical bone defects (spherical defects with diameters of 5, 10, 15, and 20 mm) were created using a digital model design so

[Re-evaluation of the etiology and diagnosis of persistent periapical periodontitis].

The professional terms "persistent apical periodontitis" and "refractory apical periodontitis" are both used to describe clinical cases that periapical lesions fail to heal after root canal therapy. Both share similarities and differences in terms of their connotations, pathogenesis, treatment methods, and prognosis. Using suitable term to describe this type of disease has always been a dilemma in clinical practice, scientific research, or academic exchanges. Based on extensive literature searches, clinical research findings and experience, this paper systematically analyzes the reasons for th

Magnetic resonance imaging in the diagnosis of periodontal and periapical disease.

OBJECTIVES: Early pre-clinical inflammatory changes in periodontal and/or periapical lesions, which typically precede bone loss, are challenging to diagnose using ionizing-radiation-based imaging modalities. MRI provides relevant additional diagnostic information of inflammatory processes in soft and hard tissues. The aim of the present study is to undertake a systematic review of the literature on MRI in the diagnosis of periodontal and/or periapical disease. METHODS AND MATERIALS: The PubMed/MEDLINE and Scopus bibliographic databases were searched (2000-2021) using the search string: ("MRI"

[Clinical studies on pulpitis and periapical periodontitis caused by traumatic occlusion].

89 cases including 100 teeth with pulpitis and apical periodontitis caused by traumatic occlusion were clinically observed. All teeth are caries-free, no any dental disease, and no pocket formation. But there is a distinct evidence of marked occlusal trauma, such as premature contact and occlusal interference. The single-root teeth are usually involved. When subjected to vitality testing the teeth associated with the periapical lesions sometimes yield positive response (about 30%). The result of investigation endodontic treatment demonstrated that the presence of predisposing factors of trauma
