Periodontal debridement: still the treatment of choice.

UNLABELLED: Periodontal debridement (PD) remains a gold standard for the treatment of inflammatory periodontitis. BACKGROUND/PURPOSE: The evidence base regarding the causal relationship between oral biofilm and the host inflammatory response to the etiology of periodontal disease has substantially increased over the years. What has not changed significantly during that time is the conservative manner in which the disease can be treated with periodontal debridement (PD). Since dental hygienists, in particular, specialize in providing these procedures it is important to evaluate the evidence th

Comparison of periodontal open flap debridement versus closed debridement with Er,Cr:YSGG laser.

BACKGROUND: Traditional periodontal open flap debridement (OFD) results in reduced pocket depth (PD), clinical attachment loss (CAL), gingival recession (GR) and postoperative pain and discomfort. The quest to overcome these shortcomings has led to research into Er,Cr:YSGG laser assisted pocket therapy (ELAPT). This study was designed to compare the clinical outcomes of ELAPT versus OFD. METHODS: Fifteen patients with a PD of ≥5 mm and ≤8 mm at two sites were selected. Test sites (Group 1) were treated by ELAPT and the control (Group 2) by OFD. Clinical parameters were recorded at baseline, 3

Labial piercing and localized periodontal destruction - partial periodontal regeneration following periodontal debridement and free gingival graft.

Localized periodontal destruction has been reported as a rare complication of intraoral piercings. The purpose of this case report was to illustrate the destructive nature of a lip stud and to describe the successful treatment of this case. The lip stud was removed and supra- and subgingival debridement was performed. Because of a shallow vestibule, the absence of keratinized gingiva, and the strong frenulum insertion at the gingival margins, a free gingival graft was placed. Subsequently the patient demonstrated a significant amount of osseous regeneration and partial coverage of the recessio

Laser treatment as an adjunct to debridement of periodontal pockets.

The aim of the study was to investigate 1) whether laser treatment would enhance periodontal healing after debridement, 2) if laser treatment would facilitate instrumentation, and 3) if laser treatment would provide sufficient analgesia. The used apparatus produced radiation equivalent to a Nd:YAG-laser and was equipped with a water spray. 27 individuals with periodontitis took part in the study. Single-rooted teeth in one quadrant were assigned test teeth by lot with contralateral teeth serving as controls. Test areas were first laser irradiated to produce analgesia. The root surfaces were tr

Intraoperative bleeding during open flap debridement and regenerative periodontal surgery.

BACKGROUND: The objective of this study was to measure the intraoperative bleeding during periodontal flap surgery. METHODS: Patients scheduled for periodontal surgery were recruited for this study. Data regarding smoking habits, general health, and medications were collected. The amount of the local anesthetic that was injected was then recorded, as well as the number of teeth in the operative field and the duration of the procedure. During surgery, the liquids from the oral cavity were suctioned and collected into a sterile empty vial. To calculate the net amount of blood volume in the liqu

Evaluation of a surgical debridement bur for use in periodontal surgery.

The Evian Debridement Bur facilitates two fundamental techniques in periodontal surgery--soft-tissue debridement and root planing. The bur debrides granulation and other soft tissues attached to the root or bone and removes hard deposits from root surfaces. It can also be used as an adjunct while performing crown-lengthening procedures and endodontic surgery. The bur is made with blunted flutes and, if used with a relatively light touch, will not cut into the alveolus and will not damage the tooth surface. This article reviews the use and advantages of this bur and shows its effect on the root

Topical metronidazole as an adjunct to subgingival debridement in the treatment of chronic periodontitis.

The purpose of this study was to evaluate the clinical effects of a single application of a 5% metronidazole collagen device in periodontal pockets deeper than 5 mm, in association with debridement and without reinforcement of home care and hygiene as practiced by the patient at any time. Pocket depth, attachment level, bleeding on probing, gingival index and plaque index were assessed at baseline, and on days 15, 30 and 90. Analysis of data from 28 patients indicated that both debridement and metronidazole therapy decreased pocket depth, bleeding on probing and gingival index, but results wer

Mechanical debridement combined with amoxicillin and metronidazole compared with mechanical debridement alone for the treatment of chronic periodontitis: An overview of systematic reviews.

Periodontal mechanical debridement is the most common therapy for the treatment of periodontitis. However, depending on the severity of the disease, mechanical debridement has been recommended in combination with systemic antibiotics. In this study, we performed an overview of systematic reviews using the Friendly Summaries of Body of Evidence using Epistemonikos (FRISBEE) methodology on the effectiveness and safety of mechanical debridement combined with amoxicillin and metronidazole compared to mechanical debridement alone for the treatment of chronic periodontitis. We conducted a systematic
