The peritoneovenous shunt: expectations and reality.

Since the introduction of the LeVeen modification of the peritoneovenous shunt (PVS) in 1974, these devices have been placed in a relatively large number of patients. The most common indication has been for medically intractable ascites in the setting of chronic liver disease. A review of a series of studies shows that we can expect approximately an 18% perioperative overall mortality rate, a 46% survival rate at 21 months, and loss of ascites in 59% of the survivors at 18 months. The PVS has not been shown by prospective trials to prolong survival significantly in patients with either intract

Complications of Denver peritoneovenous shunting.

The records of 49 consecutive patients treated with Denver peritoneovenous shunts were reviewed to determine the incidence of complications and length of patient survival. Thirty-eight complications (16 types) were identified in 22 (45%) of the patients and, in 10 instances, contributed to subsequent death. Ten percent of shunts failed during long-term follow-up. The 54% one-year survival by life-table analysis of patients with cirrhosis treated with the Denver shunt matched results previously reported for similar patients treated with the LeVeen shunt. Patients with intra-abdominal malignant

Hepatorenal syndrome: reversal by peritoneovenous shunt.

Two patients with hepatorenal syndrome were treated by insertion of a peritoneovenous shunt. The renal deficit was corrected rapidly in both cases. A 62-year-old woman with a slow onset syndrome with urine output of 100 to 150 ml/day and urinary sodium excretion of 1 mEq/day responded with large volume urinary output and sodium excretion. She is alive with minimal ascites 18 months after shunt. A 53-year-old man with severe nutritional cirrhosis, alcoholic hepatitis, and eventual massive necrosis was treated for bleeding esophageal varices by portacaval shunt. Postoperative massive ascites pro

Hepatorenal syndrome. Recovery after peritoneovenous shunt.

The renal failure in the hepatorenal syndrome is unusual because the kidneys are histologically normal and the renal failure may be "functional." Hemodynamic studies indicate that increased renal vascular resistance and decreased renal blood flow may be the primary abnormalities leading to renal failure in some cases. This report describes a patient whose renal failure resolved after placement of a peritoneovenous shunt. A major advantage of this device is that it can be inserted with the patient under local anesthesia with minimal surgical risk. Further studies are needed to define the role o

Peritoneovenous shunt occlusion. Etiology, diagnosis, therapy.

Electronic pressure testing of every LeVeen valve has practically eliminated mechanical malfunction as a cause of shunt failure. Nonetheless, failures do occur and in a series of 240 cases, early or late shunt failure occurred in 29 patients. Thirty-five additional cases of failures were either referred by other physicians over a period of 6 years or information and x-rays were accumulated by direct contact. Shunt failure becomes manifest by a sudden reaccumulation of ascites in patients with a previously functioning shunt. In immediate failure, the ascites may fail to disappear after surgery

Radiological insertion and management of peritoneovenous shunt.

The purpose of the study was to report our experience of the management of complications following the insertion of a peritoneovenous shunt for intractable malignant ascites. From June 1999 to January 2006, 26 patients underwent insertion of a peritoneovenous shunt for ascites by interventional radiologists. We have used ultrasound and shuntography to assist in the diagnosis of the cause of shunt blockage. Successful techniques for the restoration of the shunt function include port- pumping, stripping of any fibrin sheath, and revision of either the venous or peritoneal catheter. The procedure

Chylous ascites: resolution after Denver peritoneovenous shunt.

Chylous ascites complicating a Warren shunt was successfully treated with a Denver peritoneovenous shunt. There was spontaneous closure of the chylous peritoneal fistula seven months after the Denver shunt was inserted. This approach obviates the nutritional and septic complications of repeated paracentesis in the management of patients with chylous ascites.

Acute respiratory distress syndrome after peritoneovenous shunt.

A case is presented in which a fatal acute respiratory distress syndrome, associated with disseminated intravascular coagulation, developed immediately after the insertion of a peritoneovenous shunt for management of refractory ascites. The absence of left-sided heart failure or fluid overload was established by (a) lack of diuresis from intravenous furosemide; (b) repeatedly normal pulmonary wedge pressures; and (c) autopsy findings. The nature of the toxic effect of this patient's ascites upon the alveolar membrane remains obscure.

Complications of peritoneovenous shunt for malignant ascites.

A peritoneovenous (PV) shunt was placed in eight patients with intractable malignant ascites. The shunt successfully controlled the ascites in six patients. The median survival time of the entire group was 2 months, with one patient alive at 22 months. Two of seven patients had secondary shunt failure: one from an unknown cause which could not be corrected by revision and another which was corrected by revision following removal of psammoma bodies in the valve. The complications of the shunt included transient edema (four patients), transient intravascular coagulation (four patients), and feve

Peritoneovenous shunt in the management of the hepatorenal syndrome.

The hepatorenal syndrome (HRS) is a terminal complication of severe liver disease associated with a mortality of 80 to 90%. Although the renal functional abnormalities in the HRS suggest prerenal azotemia, volume expansion with saline, albumin or ascitic fluid rarely results in reversal of the HRS because fluid redistributes from the vascular space. Since the peritoneovenous (PV) shunt causes sustained central volume expansion, it has been advocated for the treatment of the HRS. We prospectively compared the PV shunt (N = 10) to Medical Therapy (MED) (N = 10) on renal function and mortality in

Percutaneous peritoneovenous shunt positioning: technique and preliminary results.

Nine peritoneovenous shunts were positioned by percutaneous technique in seven patients with advanced malignancy causing severe refractory ascites, and in two patients with hepatic cirrhosis (one with hepatocarcinoma). In all patients the shunts were percutaneously placed through the subclavian vein in the angiographic suite under digital fluoroscopic guide. No complications directly related to the procedure occurred. The shunt was successfully positioned in all patients in 60 min average time. No patient showed symptoms related to pulmonary overload or to disseminated intravascular coagulatio

Peritoneovenous shunt for palliation of gynecologic malignant ascites.

BACKGROUND: Ascites is a common sequela of advanced or recurrent gynecologic malignancies, such as carcinoma of the ovary, fallopian tube, or endometrium. Symptomatic treatment with repeated paracentesis is the initial management after failure of chemotherapy. STUDY DESIGN: This study was done to evaluate the safety and effectiveness of a peritoneovenous shunt (PVS) in the palliation of these patients with recurrent ascites. A retrospective review of 25 patients having a PVS between 1982 and 1992 was performed. RESULTS: The 25 patients consisted of 21 patients with carcinoma of the ovary, tw

Use of Peritoneovenous Shunt for the Management of Refractory Ascites.

BACKGROUND: Guidelines for the management of refractory ascites (RA) recommend transjugular intrahepatic portosystemic shunting (TIPS), diuretics, and paracentesis as the main strategies, discouraging use of surgical peritoneovenous shunts (PVSs). However, PVSs, including both Denver (DS) or saphenoperitoneal (SPS) modalities, may still have indications. Herein we report our experience with PVSs in the context of modern surgical and anesthetic management. METHODS: In our unit, PVSs are offered to patients with ascites refractory to diuretics in which TIPS are contraindicated. Heart function a

Management of refractory chylous ascites with peritoneovenous shunts.

OBJECTIVE: The purpose of this study was to define outcome of treatments of refractory chylous ascites using peritoneovenous shunts (PVSs). METHODS: Clinical data of patients with refractory chylous ascites treated with PVSs between 1992 and 2015 were retrospectively reviewed. The primary end point was clinical benefit, defined as cured, improved, or poor results; secondary end points were complications and reinterventions. RESULTS: Seventeen patients (eight female [47%]; median age, 47 years [range, 19-78 years]) with refractory chylous ascites were studied. This group represented 6% of 284

Central venous thrombosis and embolism associated with peritoneovenous shunts.

During a five-year period from Aug 1, 1977 through Aug 1, 1982, 36 patients required 47 peritoneovenous shunt procedures (36 initial and 11 revisions) for the management of their intractable ascites. The results at six months showed 23 (63.9%) of 36 patients were dead, but in those living, 12 (92.3%) of 13, the ascites was satisfactorily controlled. Patency was measurably prolonged by appropriate revision of the shunt. The early and late complication rates were surprisingly high, 38.3% and 40.4%, respectively. The most serious complication was central venous thrombosis, 11 (23.4%) of 47 proced
