[In Nigeria: sexual permissiveness and sex education].

Considering the population explosion in Nigeria which is due both to medical advances and traditional beliefs in large families as well as the recent trend of sexual permissiveness which involves the teenagers population, the author of the article considers that sex education is imperative in Nigeria. However, he sees many obstacles standing in the way of general acceptance of sex education. The husband-wife relationship does not encourage free communication on the subject and colonization has removed the traditional forms of sex education through initiation rites and pre-marital counseling by

Permissive hypercapnia.

Although lifesaving, mechanical ventilation can result in lung injury and contribute to the development of bronchopulmonary dysplasia. The most critical determinants of lung injury are tidal volume and end-inspiratory lung volume. Permissive hypercapnia offers to maintain gas exchange with lower tidal volumes and thus decrease lung injury. Further physiologic benefits include improved oxygen delivery and neuroprotection, the latter through both avoidance of accidental hypocapnia, which is associated with a poor neurologic outcome, and direct cellular effects. Clinical trials in adults with acu

Permissive hypofiltration.

Acute kidney injury (AKI) is a syndrome with a multitude of causes and is associated with high mortality and a permanent loss of renal function. Our current understanding of the most common causes of AKI is limited, and thus a silver bullet therapy remains elusive. A change in the approach to AKI that shifts away from the primary composite endpoint of death/dialysis, and instead focuses on improving survival and mitigating permanent renal damage, is likely to be more fruitful. We suggest that the current approach of augmenting renal function by increasing the renal blood flow or glomerular fil

Permissive hypercapnia.

Mechanical ventilation using high tidal volume (VT) and transpulmonary pressure can damage the lung, causing ventilator-induced lung injury. Permissive hypercapnia, a ventilatory strategy for acute respiratory failure in which the lungs are ventilated with a low inspiratory volume and pressure, has been accepted progressively in critical care for adult, pediatric, and neonatal patients requiring mechanical ventilation and is one of the central components of current protective ventilatory strategies.

Permissive hypercapnia.

The term permissive hypercapnia defines a ventilatory strategy for acute respiratory failure in which the lungs are ventilated with a low inspiratory volume and pressure. The aim of permissive hypercapnia is to minimize lung damage during mechanical ventilation; its limitation is the resulting hypoventilation and carbon dioxide (CO2) retention. In this article we discuss the rationale, physiologic implications, and implementation of permissive hypercapnia. We then review recent clinical studies that tested the effect of various approaches to permissive hypercapnia on the outcome of patients wi

A Gut Commensal Bacterium Promotes Mosquito Permissiveness to Arboviruses.

Mosquitoes are hematophagous vectors that can acquire human viruses in their intestinal tract. Here, we define a mosquito gut commensal bacterium that promotes permissiveness to arboviruses. Antibiotic depletion of gut bacteria impaired arboviral infection of a lab-adapted Aedes aegypti mosquito strain. Reconstitution of individual cultivable gut bacteria in antibiotic-treated mosquitoes identified Serratia marcescens as a commensal bacterium critical for efficient arboviral acquisition. S. marcescens facilitates arboviral infection through a secreted protein named SmEnhancin, which digests me

Examining the tsetse teneral phenomenon and permissiveness to trypanosome infection.

Tsetse flies are the most important vectors of African trypanosomiasis but, surprisingly, are highly refractory to trypanosome parasite infection. In populations of wild caught flies, it is rare to find mature salivarian and mouthpart parasite infection rates exceeding 1 and 15%, respectively. This inherent refractoriness persists throughout the lifespan of the fly, although extreme starvation and suboptimal environmental conditions can cause a reversion to the susceptible phenotype. The teneral phenomenon is a phenotype unique to newly emerged, previously unfed tsetse, and is evidenced by a p

Innate airway responses shape permissiveness to human respiratory syncytial virus.

The interaction between human respiratory syncytial virus (HRSV) and the innate immune system has been demonstrated both in vitro and in vivo. Disruption of interferon (IFN) signaling pathways increases susceptibility and permissiveness to HRSV infection, whereas pretreatment of cells with IFN confers (partial) resistance. This suggests that HRSV disease severity is likely influenced by a pre-existing antiviral state of the respiratory epithelium, driven by baseline or primed expression of type I and type III IFNs. Here, we investigated whether prior exposure to respiratory bacteria or viruses

[Epilepsy and permission to drive].

INTRODUCTION: At the present time it has become a social necessity for most people to be able to drive a motor vehicle. For this reason, obtention of a driving license is continuously regulated so as to be adapted to the psychophysical aptitudes required at any given moment, since these vary with medical and social advances. DEVELOPMENT: We review the doctrine on this subject in some of the major countries, finding that far from having similar criteria, there is disparity even between different jurisdictions of the same country. In Spain epilepsy is regulated regarding the difficulty in obtai

Matrix permissiveness regulates 3D confinement of fibroblasts in biosynthetic hydrogel microenvironments.

The extracellular matrix (ECM) is a major regulator of cellular behavior, fate, and various mechanisms underlying homeostasis, development, and disease. Biophysical and biochemical properties of the ECM are known to affect three-dimensional (3D) cellular behavior and phenotype that regulate a wide range of pathological conditions. Tunable biomimetic hydrogels are extensively employed to investigate cell-matrix interactions in defined 3D microenvironments. However, the dynamics of these interactions in complex coupled multiparametric microenvironments has been relatively less studied. This stud

Assessment of hepatitis C virus permissiveness in iteratively genetically humanized mice.

Hepatitis C virus (HCV) is an enveloped, positive-sense single-stranded RNA virus causing chronic infections in over 50 million people who are at risk of developing severe liver disease. Greater understanding of HCV pathogenesis and vaccine development has been hampered by the lack of a fully immunocompetent small-animal model permissive to infection. Rodents are resistant to HCV infection due to a variety of factors at the levels of entry and replication, many of which have been discovered within the past decade. We hypothesized that genetically altering these factors in mice would provide a

Permissive hypercapnia in trauma patients.

The use of a normal tidal volume in patients with progressive loss of alveolar airspace may increase inspiratory pressure and overdistend remaining functional alveoli. Permissive hypercapnia (PH) is a ventilator management technique that emphasizes control of alveolar pressure, rather than PCO2. The purpose of this study was to determine if the use of PH is associated with an improved outcome from adult respiratory distress syndrome (ARDS). Over a 2-year period, 39 trauma patients were treated for ARDS. Permissive hypercapnia was used in 11, and the remaining patients were treated conventional

Permissive Cardiotoxicity: An Evolving Paradigm.

Permissive cardiotoxicity is an evolving clinical practice pattern that balances the management of cancer treatment-associated cardiotoxicities with the risk of interrupting lifesaving or life-extending cancer treatments. This paradigm shift has been catalyzed by advancements in the diagnostic and therapeutic tools available to treating clinicians, as well as a recognition of the consequences of withholding or delaying cancer therapy. Permissive cardiotoxicity has become a central tenet in the practice of cardio-oncologists; however, recognition and confidence in managing cardiotoxicity is als

Permissive hypercapnia: what to remember.

PURPOSE OF REVIEW: Hypercapnia is a central component of diverse respiratory disorders, while 'permissive hypercapnia' is frequently used in ventilatory strategies for patients with severe respiratory failure. This review will present data from recent studies relating to hypercapnia, focusing on issues that are of importance to anesthesiologists caring for the surgical and/or critically ill patient. RECENT FINDINGS: Protective ventilatory strategies involving permissive hypercapnia are widely used in patients with severe respiratory failure, particularly in acute respiratory distress syndrome

Permissive hypofiltration: an alternative view.

If permissive hypercapnia is used in the context of protective ventilation for patients with acute respiratory distress syndrome, it must be highlighted that the alveoli in these patients are still exposed to significant stress. Similarly, early renal replacement therapy is not necessarily a protective therapy for acute kidney injury and loop diuretics are not necessarily harmful. It is conceivable that early initiation of 'protective' low-dose (10 ml/kg/h) continuous renal replacement therapy with zero balanced ultrafiltration in association with administration of (high dose) diuretics may he
