Peroneal nerve decompression.

Peroneal neuropathy is a common pathology encountered by neurosurgeons. Symptoms include pain, numbness, and foot drop. When secondary to compression of the nerve at the fibular head, peroneal (fibular) nerve release is a low-risk procedure that can provide excellent results with pain relief and return of function. In this video, the authors highlight key operative techniques to ensure adequate decompression of the nerve while protecting the 3 major branches, including the superficial peroneal nerve, deep peroneal nerve, and recurrent genicular (articular) branches. Key steps include positioni

[Peroneal nerve schwannoma presenting with a peroneal palsy].

Peroneal nerve injury in the lateral aspect of the knee is frequent, commonly dominated by traumatic or compressive etiologies. Tumors, generally synovial cysts, are exceptional causes. We report a case of a 70-year-old women referred for assessment of peroneal palsy syndrome found to be related to peroneal schwannoma in the lateral aspect of the knee. Schwannoma is the most frequent peripheral nerve benign tumor. It is commonly limited to cranial and upper limb nerves. Localization on the lower limbs (sciatic nerve) has been reported in 1 percent of cases. The peroneal nerve localizaton has n

Common peroneal nerve decompression.

BACKGROUND: Common peroneal neuropathy occurs as a result of compression of the common peroneal nerve (CPN) in the fibro-osseous tunnel between the fibular neck and the tendinous arch of peroneus longus. High rates of spontaneous recovery have been reported. However, there is a subset of patients who require decompression of the CPN at the fibular neck. METHODS: We performed a retrospective analysis of patients that underwent decompressive surgery of the CPN. A total of 22 operations performed on 20 patients were analysed. Pre-operative workup included clinical examination, electrophysiologic

Localizing peroneal nerve lesions.

The main result of a peroneal nerve lesion, foot drop, is so obvious that few patients can ignore its presence. However, the detailed knowledge of peroneal nerve anatomy needed to localize the lesion may tax even a seasoned clinician. Examination of the strength of five muscles--the tibialis anterior, peroneus longus and brevis, posterior tibial, gluteus medius and quadriceps--provides information necessary to localize the site of the neuropathy. This tentative localization can be confirmed by electromyography. Treatment is directed to the cause of the lesion and stabilization of the ankle if

Intraseptal superficial peroneal nerve.

Knowledge of the anatomy of the superficial peroneal nerve (SPN) is necessary for surgeons caring for patients with lower extremity pain after ankle, leg, or knee injuries, for athletes with exertional compartment syndrome, and those having reconstructive microsurgery with either soft tissue or vascularized fibular flaps. The anatomy of the SPN is known to be that of a peripheral nerve traveling in the lateral compartment of the lower leg. Recently, clearer descriptions of its variability have documented that between 27 and 43% of patients have the SPN in either the anterior compartment or bot

The possibility of deep peroneal nerve neurotisation by the superficial peroneal nerve: an anatomical approach.

Neurotisation involves transfer of nerves for the restoration of function following injury. A number of nerves have been used in different part of the peripheral nervous system. This study was undertaken to develop a practical and relatively safe surgical approach to the treatment of L4 root lesion's. We examined the effectiveness and safety of neurotisation of the deep peroneal nerve and its branches by the superficial peroneal nerve. Twelve legs of dissected cadavers provided for teaching purposes in the anatomy laboratory were used to display the common peroneal nerve and its branches. Each

Compound nerve action potential of common peroneal nerve and sural nerve action potential in common peroneal neuropathy.

To enhance the accuracy for determining the precise localization, the findings of the compound nerve action potentials (CNAPs) of the common peroneal nerve (CPN) were investigated in patients with common peroneal mononeuropathy (CPM) in the knee, and the sural sensory nerve action potentials (SNAPs) were also analyzed. Twenty-five patients with CPM in the knee were retrospectively reviewed. The findings of the CNAPs of the CPN recorded at the fibular neck and the sural SNAPs were analyzed. The lesion was localized at the fibular head (abnormal CNAPs) and at or distal to the fibular head (norma

Involvement of superficial peroneal sensory nerve in common peroneal neuropathy.

Motor involvement in common peroneal neuropathy (CPN) frequently shows a selective pattern with regard to deep and superficial divisions of the peroneal nerve, by clinical examination and needle electromyography. The involvement of the sensory branch of the superficial peroneal nerve (SPN) has not been well established using nerve conduction studies. Among 42 cases of electrophysiologically defined CPN in 35 patients, 37 (88%) had clinical or electrophysiologic evidence for SPN involvement, but only 20 had diminished superficial peroneal sensory response (SPSR) amplitudes. Many of the cases in

Sciatic and peroneal nerve injuries.

In the past, the attitude toward surgical repair of sciatic and peroneal nerve lesions was excessively pessimistic. Refinements of microsurgical technique, nerve conduction studies, and the establishment of indications and optimal timings for surgical interventions have led to significant improvements in outcomes, making repair worthwhile in most cases. Tendon transfers can augment functional outcomes, and are performed as secondary procedures or (in selected cases) in combination with the initial repair. Developments in the basic sciences, bioengineering, and medical imaging may further impro

Peroneal nerve entrapment in runners.

In a practice involving large groups of athletes, seven runners and one soccer player with peroneal nerve compression neuropathy secondary to exercise have been found. Running incited pain, numbness and tingling to varying degrees in all patients, and examination after running revealed muscle weakness and a positive percussion test as the nerve winds around the fibular neck. Nerve conduction velocity studies confirmed the diagnosis in the five patients on whom the test was performed; other studies served primarily to exclude other causes of pain. All patients were treated surgically by neuroly

Peroneal nerve entrapment in athletes.

Peroneal nerve entrapment is one of the less common causes of exercise-induced leg pain in competitive athletes. This type of lower extremity peripheral nerve dysfunction is usually associated with activities that subject the nerve to constant compression or repetitive trauma. Herein, we present our experience with 12 competitive athletes treated for peroneal nerve entrapment. Diagnostic electromyography results, intraoperative findings, and the results of cadaveric dissections are discussed.

Peroneal nerve repair. Surgical results.

Eleven patients had either repair (two) or nerve graft reconstruction (nine) of the common peroneal nerve or its superficial or deep divisions about the knee. The average follow-up period was 29.1 months. A good or excellent result was achieved in six (54.5%) patients. The results appeared best when direct nerve repair was possible or when a defect of 6 cm or less required grafting. Peroneal nerve exploration and repair or reconstruction as required seem indicated for known nerve disruption or unknown nerve continuity if there is no suggestion of continuing recovery by three to six months afte

Decompression for peroneal nerve entrapment.

I reviewed 24 patients after decompression for peroneal entrapment neuropathy; in 3 cases the lesion was bilateral. There were 15 males and 9 females; mean age 44 (12-72) years. The etiology was an operation around the knee in 12, a tibial fracture in 2, a slight compression in 1, an ankle sprain in 2, excessive climbing in 2, sitting in a cross-legged position in 4, and in 4 cases no reason was found. There was foot drop in 15 and ankle instability in 12 cases. The nerve was decompressed after an average period of 17 months (4 days-8 years). Immediate relief of symptoms was achieved in 14 cas

Superficial Peroneal Nerve to Deep Peroneal Nerve Transfer With Allograft Conduit for Neuroma in Continuity.

The anterior approach to the ankle for surgery can result in injury to the superficial peroneal nerve, resulting in a painful neuroma and significant patient morbidity. A paucity of data is available evaluating the role of the superficial peroneal nerve to deep peroneal nerve transfer as a method of treatment of neuromas in continuity after ankle arthrodesis. We describe 11 patients who underwent nerve transfer with nerve allograft and conduit repair to treat recalcitrant painful neuromas after ankle arthrodesis. At a mean follow-up period of 31 months, the mean visual analog pain scale score

Compression of the peroneal nerve by a neurofibroma originating from collaterals of the peroneal nerve: a case report.

BACKGROUND: Paralysis of the external popliteal sciatic nerve is a frequent pathological condition that occurs after trauma. However, etiologies other than trauma, such as tumors, are also possible. The sensory collaterals of the external popliteal sciatic nerve have a small territory of innervation at the knee, and tumors involving these nerves become symptomatic after compression of the motor nerves. We here describe the first reported case of this phenomenon. CASE PRESENTATION: This case involved a lesion compressing the origin of the external popliteal sciatic nerve of a 13-year-old Moroc

Minimally invasive peripheral nerve surgery: peroneal nerve neurolysis.

BACKGROUND: Both patients and surgeons recognize the value of procedures that minimize scarring and tissue dissection. No previous reports have described a minimally invasive technique for peroneal nerve neurolysis, or evaluated its safety. METHODS: The senior author's technique for a minimally invasive approach to neurolysis of the common, superficial, and deep peroneal nerves is presented. Safety of the technique was determined by review of records of all patients undergoing this procedure from 2003-2011, looking for major complications. RESULTS: Using the minimally invasive approach to pe
