A non-ideal authenticity-based conceptualization of personal autonomy.

Respect for autonomy is a central moral principle in bioethics. The concept of autonomy can be construed in various ways. Under the non-ideal conceptualization proposed by Beauchamp and Childress, everyday choices of generally competent persons are autonomous to the extent that they are intentional and are made with understanding and without controlling influences. It is sometimes suggested that authenticity is important to personal autonomy, so that inauthenticity prevents otherwise autonomous persons from making autonomous decisions. Building from Beauchamp and Childress's theory, this artic

Respect for persons, autonomy and palliative care.

This paper explores some of the values that underpin health care and how these relate more specifically to the values and ethics of palliative care. The paper focuses on the concept of autonomy because autonomy has emerged as a foundational concept in contemporary health care ethics and because this is an opportunity to scratch the surface of this concept in order to reveal something of its complexity, a necessary precaution when applying the concept to the context of palliative care. The paper begins with a theoretical discussion of autonomy exploring an aspect of its contemporary meaning and

Authenticity and psychiatric disorder: does autonomy of personal preferences matter?

In healthcare ethics there is a discussion regarding whether autonomy of personal preferences, what sometimes is referred to as authenticity, is necessary for autonomous decision-making. It has been argued that patients' decisions that lack sufficient authenticity could be deemed as non-autonomous and be justifiably overruled by healthcare staff. The present paper discusses this issue in relation certain psychiatric disorders. It takes its starting point in recent qualitative studies of the experiences and thoughts of patients' with anorexia nervosa where issues related to authenticity seem pa

[Empowerment of children and adolescents--the role of personal and social resources and personal autonomy for subjective health].

AIM: Scientific research on empowerment so far is nearly exclusively focused on the adult population. Nevertheless, it is possible to show a link between empowerment and a) the developmental psychology concepts of resilience, b) autogenetic concepts and c) concepts of risks and resources. This paper aims to study the role of personal, familial and other social resources as well as personal autonomy for subjective health-ratings. METHODS: A secondary analysis of the health data of 7,000 children and adolescents aged 10-17 years of the German health behaviour in school-aged children (HBSC) stud

Psychometrics of the autonomy and relatedness inventory in persons with type 2 diabetes.

BACKGROUND: Social support is essential for the self-management of type 2 diabetes mellitus (T2DM); no specific measure of the quality of social support has been designed and tested for people with T2DM. Having a validated questionnaire that is culturally and linguistically appropriate in this field would nurture research in this area and contribute to better patient health outcomes. AIMS: This study investigated the psychometric features of the Autonomy in Relatedness Inventory (ARI) among individuals with T2DM in Jordan. METHODOLOGY: We used a secondary dataset on self-management and outco

A systematic review of personality-related factors associated with autonomy-connectedness in functional neurological disorder.

UNLABELLED: Preliminary evidence suggests that individuals with Functional Neurological Disorder (FND) may exhibit reduced autonomy-connectedness, a psychological construct referring to the capacity to remain connected with one's own needs and emotions while maintaining meaningful relationships with others. Autonomy-connectedness consists of three interrelated dimensions: self-awareness, sensitivity to others, and the capacity to manage new situations. Although autonomy-connectedness itself has not yet been directly investigated in FND populations, several psychological characteristics associa

How Should Autonomy of Persons Experiencing Homelessness Be Balanced With Public Health During a Pandemic?

The COVID-19 pandemic underscores long-standing challenges to the health of persons experiencing homelessness. This case commentary considers primary care clinicians' response to a patient who declines COVID-19 testing and isolation. This article also considers other outbreaks in the United States in which the autonomy of persons with low income or persons of color was neglected and calls for community engagement, policies that center interests of marginalized groups, and economic relief, including supportive housing.

Does Alice Live Here Anymore? Autonomy and Identity in Persons Living and Dying With Dementia.

Conventional scientific definitions of dementia, or its newer proposed alternate-neurocognitive disorders place emphasis upon cognitive function, particularly memory. The changes in thought, emotion, behavior, personality, and biological function are usually considered only of secondary importance. At the core of the illness, however, lies a progressive loss of self, and by extension, of personhood, identity, autonomy, and agency. The identity of the person living with dementia, and the deterioration of a sense of self assumes significance in planning end of life care, including palliative car

The association between autonomy-supportive coaching and athletes' personal best performance: indirect associations involving basic psychological needs and autonomous motivation.

INTRODUCTION: Although autonomy-supportive coaching has been widely recognized as beneficial for athletes' motivation and performance, associations between autonomy-supportive coaching and athletes' personal best performance remain insufficiently understood. Grounded in Self-Determination Theory (SDT), this study examined the associations among autonomy-supportive coaching, basic psychological need satisfaction, autonomous motivation, and athletes' personal best performance, within a theory-informed associative framework. METHODS: A total of 411 provincial-level athletes from multiple sports

[Support provided by third parties and the reaction of close female caregivers in the care of elderly persons with loss of autonomy].

This article is about the relation between, on the one hand, support provided by third parties and, on the other, the burden and the depression experienced by the main female supporters who care for the elderly suffering from physical or cognitive problems. The authors examine the hypothesis that social support is not homogeneous across the board and that the different types of support are likely to have different effects on elders with adverse reactions to the caring process. Data originates from a study that was conducted in the Montréal area in 1990 with 159 female supporters living with an

Emphasizing Choice and Autonomy in Personal Hygiene, Menstrual Health, and Sexual Health Product Distribution to People Experiencing Homelessness in Atlanta, Georgia During COVID-19.

The Dignity Pack Project is a small-scale, crisis-oriented supply chain in Atlanta, Georgia, designed to meet the acute personal hygiene, menstrual health, and sexual health needs of people experiencing homelessness (PEH). It was organized in response to conditions during the COVID-19 pandemic that continue to illuminate and exacerbate the distinct and complex challenges PEH face when trying to meet their basic needs and maintain their health. In addition to being particularly vulnerable to COVID-19 due to underlying conditions, crowding, and shared living spaces, the pandemic makes it harder f

A Mother's Borderline Personality Disorder and Her Sensitivity, Autonomy Support, Hostility, Fearful/Disoriented Behavior, and Role Reversal With Her Young Child.

There is some evidence that maternal borderline personality disorder (BPD) adversely affects parenting in infancy, resulting in disorganized attachment, which longitudinally predicts BPD symptoms in adulthood. We examined parenting related to disorganized attachment beyond infancy in offspring of mothers with BPD, when parenting becomes a goal-corrected partnership. We observed puzzle solving in a low socioeconomic status (SES) sample of mothers with BPD and their children ages 4-7, n = 36, and normative comparisons, n = 34. Compared with normative mothers and controlling for maternal mood dis

Defining Autonomy in Psychiatry.

Mental illness undermines a patient's personal autonomy: the capacities of a person that enables them to live a meaningful life of their own making. So far there has been very little attention given to personal autonomy within psychiatry. This is unfortunate as personal autonomy is disturbed in different ways in psychiatric disorders, and understanding how autonomy is affected by mental illness is crucial for differential diagnosis and treatment, and also for understanding personal recovery. We will argue that disturbance of personal autonomy is related to patient's diminished quality of life

Personal values, professional responsibilities and non-invasive prenatal testing: reflections of genetic counsellors in Australia.

The reproductive autonomy rationale endorses the provision of non-invasive prenatal testing (NIPT) to enable individuals to make decisions about their pregnancy in accordance with their personal values. While the personal values of healthcare professionals may promote autonomous reproductive decision-making in light of increasingly complex genetic information, they may also impinge upon reproductive autonomy through limiting patient access to care. This paper provides timely insight into genetic counsellors' perceptions and implementation of professional responsibilities as well as their views

Why Controlling Teaching Persists in Medical Education: A Self-Determination Theory Perspective.

Autonomy-supportive teaching is widely promoted in medical education for its benefits to learner motivation and well-being. Yet many learners continue to experience clinical education as controlling, even in environments that explicitly endorse learner-centred values. This persistent gap suggests that barriers to autonomy-supportive practice extend beyond lack of knowledge or skill. This paper offers a theory-informed analysis of why psychologically controlling teaching persists in high-stakes clinical learning environments. Using Self-Determination Theory (SDT) as its organizing framework and
