OBJECTIVES: We evaluated the results of treatment and follow-up of patients with petrous bone lesions.
PATIENTS AND METHODS: We retrospectively evaluated 13 patients (5 females, 8 males; mean age 33.8+/-17.4 years; range 4.5 to 65 years) who were treated and followed-up for petrous bone lesions between 1990 and 2004. The diagnoses were cholesterol granuloma (n=2; 15.4%), chondrosarcoma (n=1; 7.7%), and cholesteatoma (n=10; 76.9%).
RESULTS: Patients with cholesterol granuloma had complaints of cephalgia and diplopia while those with cholesteatoma complained of otorrhea and hearing loss. One p

Petrous bone cholesteatoma is a rare pathologic entity and may be a difficult surgical challenge because of potential involvement of the facial nerve, carotid artery, dura mater, otic capsule, and risk of cerebrospinal fluid leak. The objective of this article is to present a personal classification of petrous bone cholesteatomas, a survey of recent surgical attitudes, and our present surgical strategy based on our experience with 54 operations between 1978 and 1990. Radical petromastoid exenteration with marsupialization and the middle cranial fossa approach were used only for small pure infr

BACKGROUND: The temporal bone is one of the most complex bones in the human body. Fractures require significant force and are usually associated with extensive damage to adjacent structures.
OBJECTIVE: Which imaging modality facilitates the assessment of petrous bone fractures, and which anatomical landmarks may be used for orientation?
METHODS: A literature search on PubMed.gov was conducted to determine the current state of research.
RESULTS: Injuries to the petrous part of the temporal bone often involve the facial nerve and the vestibulocochlear nerve, as well as the cochlea and the ves

Floating petrous bone fracture appears to occur when the elastic pediatric skull is subjected to large compressive forces. Unlike a longitudinal fracture, a floating petrous bone fracture does not involve the ear canal and squamous cortex. Rather, the fracture line runs through the middle ear, separating the petrous apex from its lateral and inferior bony attachments. Characteristic clinical findings include immediate onset of conductive hearing loss, with ipsilateral sixth and seventh cranial nerve paralysis. This article details the third known occurrence of this type of fracture, and includ

Petrous bone cholesteatoma is a rare pathology which grows slowly and is often asymptomatic. This work presents a series of 25 cases of Petrous bone cholesteatoma and discusses the diagnostic impact, the surgical approach, the results and the complications. Currently our surgical orientation favors translabyrinthine and transcochlear approaches which, better than other ones, allow both radical excision of the lesion and preservation of the main neuro-vascular structures. Out of the 25 patients undergoing surgery, 22 were treated with the translabyrinthine (6 cases) or transcochlear (16 cases)

In most cases, inflammatory diseases of the petrous bone can be diagnosed clinically. However, for certain diseases, the extent of the findings must be assessed using cross-sectional imaging, or important complications such as cerebral complications must be excluded. The petrous bone is part of the temporal bone, consisting of the pars squamosa, pars tympanica, and pars mastoidea [1]. It encompasses the inner ear, the internal auditory canal, and the petrous bone itself. Anatomically, the middle ear and mastoid already belong to the pars tympanica and pars mastoidea [1]. Imaging should ideally

In the region of the petrous bone, inner acoustic canal and cerebellopontine angle, a variety of different tissues can be found, such as bony, epithelial, neural and vascular structures. Tumorous or tumor-like lesions, vascular or bony malformations or other pathologies can therefore be found in all of these areas. We discuss various frequently occurring tumorous or tumor-like pathologies including congential lesions, such as mucoceles, inflammatory disorders including osteomyelitis, pseudotumors and Wegener's granulomatosis. Benign non-neoplastic lesions, such as cholesteatoma, cholesterol gr

Fibrous dysplasia named after Jaffè and Lichtenstein is a benign, self-limiting disorder of the bone structure. The normal bone is replaced by cellular fibrous connective tissue. Fibrous dysplasia can be differentiated into monostotic and polyostotic forms and the McCune-Albright syndrome (combination of polyostotic areas in the bone with hyperpigmentation of the skin and endocrinological disturbances). The rare disease is predominantly found in the 1st to 3rd decade of life in the femur or humerus. In about 15% of all cases it affects the facial bones, and one-third of these is located in the

Diseases of the petrous bone should now be diagnosed by means of high-resolution multislice spiral computed tomography (MSCT) and/or magnetic resonance imaging (MRI). The first step in the process of diagnosis, however, must be conventional X-ray photographs (according to Schüller, Mayer, Stenvers) for screening purposes, because of the high cost of the other procedures mentioned. Because of the excellent imaging of bone structures with MSCT, this technique is especially suitable for the diagnosis both of acquired pathologies and of congenital abnormalities of the external auditory meatus, the

Gd-DTPA-enhanced MRI provided accurate demonstration of the site and extension of a haemangioma of the petrous bone. Enhanced-MRI could allow early detection and differentiation from the other more common tumours of the geniculate ganglion area, thereby permitting the surgeon to remove the tumour, while preserving the facial nerve. CT is, however, required to display the characteristic ossification in these tumours.

This paper has described an unusual case of osteoclastoma of the petrous temporal bone which presented with multiple cranial neuropathies. A radical course of radiotherapy reversed the neuropathies but it is too early to comment on possible cure.

The object of this retrospective study was to describe a series of patients with petrous bone cholesteatomas, paying particular attention to classification, diagnosis, surgical strategy, results, complications and recurrences. Furthermore, the study was designed to evaluate the impact of imaging techniques on an early diagnosis. Topographically, the petrous bone cholesteatomas of the present series were grouped using Sanna's classification and different surgical approaches were used. High resolution CT and/or MRI were used to follow-up the patients. The case notes of 52 patients with petrous b

The diagnosis and treatment of petrous bone cholesteatoma is a challenge to aural surgeons. Seven patients with extensive petrous bone cholesteatomas which invaded the labyrinth and fallopian canal are presented. These cholesteatomas originated as secondary to acquired lesions. The cases were evaluated according to the clinical features, the intraoperative findings, the radiological findings, and the surgical approaches. In this series, six patients presented with facial paralysis associated with profound or total deafness. The transtemporal lateral approach was used in all the cases. Acute fa

Sinonasal and petrous bone fungal infections are rare and life-threatening conditions that essentialy arise in immunocompromised patients. They are mainly due to Aspergillus in patients with hematological diseases and to Mucorales in diabetic. Invasive fungal sinusitis can be acute (invasive aspergillosis, rhino-orbito-cerebral mucormycosis) but chronic invasive sinusitis can also exist (invasive chronic fungal sinusitis and granulomatous invasive fungal sinusitis). Petrous bone fungal infections are similar to bacterial necrotizing otitis externa and are most often due to Aspergillus. The dia

UNLABELLED: Basilar skullfractures: the petrous bone.
OBJECTIVES: to provide suggestions for the management of three of the most dangerous or important lesions (internal carotid artery lesions, cerebrospinal fluid leaks and facial nerve paralysis) associated with the petrous part of basilar skull fractures, thereby trying to assess categories of evidence and determine strengths of recommendation.
METHODOLOGY: A PubMed-based literature review was carried out, as well as a consultation of online sources as encountered in the literature review. Also, a non-systematic search of chapters of well-

BACKGROUND: Extramedullary plasmocytoma of the temporal bone is a rare neoplastic disorder. Only a very few cases are described in the world literature.
CASE: We report on a patient who presented with the clinical signs of a glomus jugular tumor. Postoperative histological evaluation revealed an extramedullary plasmocytoma of the temporal bone.
CONCLUSION: If a glomus tumor is suspected, histological investigation appears necessary if surgery is not possible or is contraindicated.
