Phaeohyphomycosis.

Phaeohyphomycosis refers to infections due to a large group of heterogenous organisms called "dematiaceous" or "melanized" fungi. These fungi are distinguished by the predominance of melanin in their cell walls, which likely acts as a virulence factor. Virtually, everyone is exposed to dematiaceous fungi through inhalation, as they are ubiquitous in the environment, although the development of infection is extremely uncommon. Invasive disease is rare but remains important due to the ability to cause serious disease in immunocompetent and immunocompromised hosts, unlike other fungal infections

Intraoral phaeohyphomycosis.

Phaeohyphomycosis is an infection caused by pigment-producing saprophytic fungi. Systemic infections may occur in the immunocompromised patient. Infection in healthy individuals may result in subcutaneous abscess formation. Oral lesions appear to be rare. A case of intraoral phaeohyphomycosis presenting as a well-demarcated, painful nodule of the anterior hard palate in a 12-year-old healthy male is described. The mass was excised and the diagnosis was established following histopathologic examination of the tissue.

Cutaneous phaeohyphomycosis.

BACKGROUND: Phaeohyphomycosis is an infrequent infection in human beings. However, in recent years, its prevalence has augmented in immunosuppressed patients (mostly in solid organ transplanted patients). Infection can be mucocutaneous or disseminated. In the former, the fungus inoculation occurs mainly through traumatism. Lesions may be polymorphic and asymptomatic, isolated or multiple, and are usually localized in exposed areas of the limbs and head. Treatment is not standardized. When possible, surgical resection of the lesion is combined with systemic antifungals. METHODS: We communicate

Cerebral phaeohyphomycosis.

A 14-year-old girl with a rare form of fungal brain abscess involving the dura and brain parenchyma is reported. No portal of entry of the infection was found. Histological findings and fungal culture both indicated that the causative agent was Xylohypha bantiana, an uncommon dematiaceous fungus. A review of the literature suggests that this infection, which affects primarily young male patients, exhibits distinct neurotropism. Despite therapy, the prognosis is generally poor.

Experimental phaeohyphomycosis.

The authors performed an experimental infection of the rabbit eye with Wangiella dermatitidis which had been isolated from the corneal ulcer of a patient. The fungus was inoculated into the front chamber and the vitreous body. The disease showed a trend to spontaneous recovery. The individual phases of the experimental infection were followed by histology and electronmicroscopy, both TEM and SEM. Different stages of development of the polymorphous fungus, as sclerotic bodies, mycelial filaments and yeast-like cells, could be demonstrated.

Cerebral phaeohyphomycosis.

Multiple phaeohyphomycotic brain abscesses caused by Cladosporium species occurred in a 55 year old woman. No immunological abnormality could be detected. The disease ran a protracted course for a total of 20 months before she died from sudden rupture of an abscess loculus into the ventricular system. Course was characterised by spontaneous remissions and relapses totally independent of adequate doses and prolonged regimes of all the three available anti-fungal chemotherapeutic agents, namely amphotericin B, flucytosine and ketoconazole. Three surgical procedures were carried out; and surgical

Phaeohyphomycosis in China.

BACKGROUND: Due to more attentions paid to melanized fungi over the past few decades and under the background of the global coronavirus disease 2019 pandemic (COVID-19) the fact that the virus itself and the immunosuppressive agents such as glucocorticoids can further increase the risk of infections of deep mycoses, the number of patients with phaeohyphomycosis (PHM) has a substantial increase. Their spectrum is broad and the early diagnosis and treatments are extremely sticky. This study aims to more comprehensively understand the clinical features of phaeohyphomycosis in China over 35 years

[Pseudomycetomatous subcutaneous phaeohyphomycosis].

The authors report the first Nigerian case of cutaneous and sub-cutaneous phaeohyphomycosis. It was characterized by its original macroscopy: the lesion was pseudo-tumoral and provided with fistulae. So it looked like a mycetoma. The diagnosis was finally made by histology: it showed the typically pigmented mycelium. Cultures have not been made. Etiological agents of phaeohyphomycosis being opportunistic, this disease occurs often among immunosuppressed patients. The knowledge of unusual clinical forms may allow to suspect the diagnosis and to cultivate the exudate: this provides the only spec

[Treatment of chromoblastomycosis and phaeohyphomycosis].

Cases of chromoblastomycosis are frequent in certain parts of the world, especially in some developing countries. Clinical manifestations of chromoblastomycosis are typical. To a certain extent, pathogens causing chromoblastomycosis overlap with those causing phaeohyphomycosis. Although cases of phaeohyphomycosis are not very common, they may end fatally. Therefore early management of these life-threatening infections is rather important. Targeted antifungal therapy and surgery are effective in combating these infections. Recently, several triazole antifungals such as posaconazole and isavucon

Disseminated phaeohyphomycosis in a dog.

Phaeohyphomycosis is a rare but emerging disease caused by dematiaceous fungi. Here we describe the case of an immunosuppressed dog with disseminated phaeohyphomycosis secondary to Bipolaris spicifera infection. Regionally extensive infiltration of the paw pads, skin, myocardium, liver, renal interstitium and diaphragm was identified on histopathology. Candida glabrata and Fusarium oxysporum were also cultured from multiple sites post-mortem. The dog was treated with fluconazole, itraconazole, terbinafine and liposomal amphotericin B, but was euthanized due to its poor prognosis after 12 days

Cutaneous and subcutaneous phaeohyphomycosis.

UNLABELLED: Case 1: A 17-year-old male rural worker from Bolivia living in La Plata (Argentina) for the past year had a lesion on the flexor side of his right forearm (6 x 4 cm). The lesion was formed by several confluent nodular areas, wine-red in color, some fistulized, with hemopurulent drainage. The area was hot and painless (Figure 1). On physical examination, no regional adenomegalies were reported. The following analyses were requested and results reported. Soft tissue ultrasound: material of solid consistency with layered liquid areas, located in the subcutaneous cellular tissue, with

Cerebral phaeohyphomycosis: case report.

Cerebral phaeohyphomycosis is a rare and frequently fatal disease. This disease is often caused by hematogenous spread of pathogens that are inoculated in the skin of the extremities after slight or minor trauma, and its mortality rate is rather high despite aggressive treatment. Our patient presented with headache and pyrexia. She was diagnosed with fungal meningitis and treated by systemic administration of voriconazole (VRCZ). However, after initial improvement, meningitis recurred. MRI of the brain showed multiple small masses in the cerebral hemisphere and she was thus referred to our Dep

Cerebral phaeohyphomycosis in a dog.

Cerebral phaeohyphomycosis was diagnosed in a 9-year-old spayed dog that had a series of epileptic convulsions a day before death. About 6 weeks before her death, she had been treated for severe demodectic mange. During this period, persistent leukopenia, lymphocytopenia, and thrombocytopenia were found by blood analyses. At necropsy, multiple large pyogranulomatous lesions were found in the cerebrum and meninges. Dematiaceous fungi with brown, branching, septate hyphae and budding yeasts were found within tissue cells and in the necrotic areas.

Subcutaneous phaeohyphomycosis (mycotic cyst).

Phaeohyphomycosis designates fungal infections caused by pheoid or melanized fungi and characterized histopathologically by the presence of septate hyphae, pseudohyphae, and yeasts. Etiologic agents include Exophiala, Phoma, Bipolaris, Phialophora, Colletotrichum, Curvularia, Alternaria, Exserohilum, and Phialemonium sp. The most common are Exophiala jeanselmei and Wangiella dermatitidis. The clinical presentation depends on the immune status of the host: superficial (tinea nigra and black piedra); cutaneous (scytalidiosis) and corneal; subcutaneous (mycotic cyst); and systemic phaeohyphomycos

Molds: hyalohyphomycosis, phaeohyphomycosis, and zygomycosis.

Emerging fungi previously thought to be nonpathogenic are now recognized as playing a significant role in the increased incidence of invasive fungal disease. This change in the epidemiology of invasive fungal infections (IFIs) has occurred in the era of aggressive new therapies for hematopoietic stem cell transplantation and other malignancies that lead to profound immunosuppression for longer durations and has extended the survival of these critically ill patients. The significant morbidity and mortality associated with these infections is not only related to the host populations but to delay

Phaeohyphomycosis complicating compound skull fracture.

Intracranial infection is a well-recognized complication of compound skull fractures. In most cases various bacteria are identified as the etiologic agents. Fungal infection complicating open head trauma is unusual. We describe a patient who contracted fatal meningoencephalitis due to Drechslera spicifera, a rarely pathogenic soil saprophyte, after open head trauma.

Systemic phaeohyphomycosis in pregnancy and the puerperium.

Systemic fungal infections are rare. In pregnancy, treatment is problematic because of the risk of possible teratogenic effects of the antifungal drugs. We present the case of a 32 year-old woman who presented during pregnancy with a two-month history of painless subcutaneous nodules. Excision biopsy of one lesion showed dematiaceous fungal elements. Anti-fungal treatment was deferred and the pregnancy proceeded uneventfully. The remaining nodules were excised at the time of caesarean section delivery. Three weeks into the puerperium, she developed generalised seizures and investigations indic
