[Roles of Cardiology Pharmacists].

Inpatients with heart failure take a number of high-risk drugs such as anti-thrombotic and anti-arrhythmic drugs, in addition to other drug types, including diuretics and cardioprotective agents. Pharmacists should support the pharmacological management of these patients by evaluating their clinical condition and laboratory data, due to the frequency of renal failure. Furthermore, it is known that poor adherence to a drug regimen is one of the aggravating factors for heart failure. It is necessary to conduct regular patient education to facilitate early hospital discharge and to prevent the re

Prescribing by pharmacists in Alberta.

PURPOSE: The evolution of prescribing by pharmacists in Alberta is described. SUMMARY: The prescribing model for pharmacists registered with the Alberta College of Pharmacists is defined by three categories of prescribing a Schedule 1 drug, a drug available only by prescription in Alberta. Pharmacists are not authorized to prescribe narcotics and controlled drugs (i.e., opium and its derivatives, barbiturates, and benzodiazepines) that are federally regulated. The first two categories of prescribing, adapting a prescription and prescribing in an emergency, legitimize existing practices that w

Role of pharmacists in HIV prophylaxis: a scoping review of pharmacists' services, interventions and outcomes.

INTRODUCTION: HIV remains a major global public health challenge, with nearly 1.3 million new HIV acquisitions annually. Despite the effectiveness of pre-exposure (PrEP) and post-exposure prophylaxis (PEP), barriers to access, initiation and adherence persist. Pharmacists, as accessible healthcare providers with medication expertise, are increasingly involved in HIV prevention. However, the scope and impact of pharmacist-led prophylaxis services have not been comprehensively synthesized. This study aimed to map and characterize the existing evidence on pharmacists' services, interventions and

Pharmacists' Impact on Secondary Stroke Prevention.

BACKGROUND: Patients admitted to our institution with a cerebrovascular accident (stroke) or transient ischemic attack (TIA) are referred to the pharmacist-run stroke prevention clinic (SPC) for medication and risk factor management. OBJECTIVE: The objective was to determine if patients receiving care from the SPC have better outcomes than patients who received usual care. METHODS: This was a retrospective chart review of patients referred to the SPC. At the time of stroke/TIA, before initial visit, and after last SPC visit, risk factor data was collected. Hospital readmissions were reviewed

The ICU pharmacist's contribution to antimicrobial optimization through routine pharmacist interventions.

BACKGROUND: Antimicrobials are among the medication groups most frequently involved in medication errors and adverse drug events in the Intensive Care Unit (ICU). Despite growing evidence supporting the role of Critical Care Pharmacists (CCPs) in antimicrobial stewardship, their integration into ICU teams remains inconsistent across European healthcare systems. METHODS: A prospective observational study was conducted over five months in an 18-bed medical-surgical ICU at a tertiary-level hospital in Spain. All admitted patients were included. A specialist pharmacist reviewed daily antimicrobia

Pharmacist Competences and Impact of Pharmacist Intervention on Medication Adherence: an Observational Study.

BACKGROUND: The aim of this study is to determine the results of pharmacist interventions on patient's adherence to drugs in community settings. SUBJECTS AND METHODS: The study was designed using a standard EQ-SDQ questionnaire and a Culig questionnaire about the adherence to drug. General Level Framework (GLF) was used as a tool for competence assessment among community pharmacists. Participants were pharmacists that issued the medication on repeat prescription in the pharmacy. They interviewed the patient at the first visit to the pharmacy and the next time when the patient came to refill t

Neuromuscular monitoring: A tutorial for pharmacists.

PURPOSE: To describe neuromuscular monitoring modalities and highlight the importance of neuromuscular monitoring to clinical pharmacy practice. SUMMARY: A growing body of literature and clinical practice guidelines have highlighted the importance of neuromuscular monitoring practices to ensure patient safety during surgery and in the intensive care unit. Understanding neuromuscular monitoring modalities can allow pharmacists to enhance participation in institutional discussions and optimization of neuromuscular blocker administration and reversal practices. We have described the various moda

AIMS: We aimed to explore pharmacists' attitudes and support toward medically assisted dying (MaiD) through the End of Life Choice Act 2019 (EOLC), their willingness to provide services in this area of practice, and the influences on their decisions. METHODS: The study was conducted via an anonymous, online QualtricsTM survey of pharmacists. Registered New Zealand pharmacists who agreed to receive surveys from the two Schools of Pharmacy as part of their Annual Practicing Certificate renewal were invited to participate through an email with a Qualtrics URL link. The survey contained questions

[Anti-doping Activities by Hospital Pharmacists].

With the hosting of the Tokyo Olympics, interest in anti-doping (AD) has increased, and at the same time, sports pharmacists (SPs) are expected to play active roles. However, despite the fact that it has been more than 10 years since the SP certification system was launched, the existence and roles of SPs are not well recognized by sports officials, and many SPs feel that there is no place or way for them to contribute. In my case, as a staff member of the federation, I have supported competitions as a referee and secretariat, so I have had opportunities to conduct AD activities such as semina

Knowledge of epilepsy among German pharmacists.

PROBLEM: Pharmacists can have a large impact on attitude, acceptance and adherence in patients with long-term conditions. They should be able to give appropriate advice. METHODS: We invited pharmacists in Mecklenburg-Vorpommern and Saxony, Germany, to complete a questionnaire mainly containing multiple-choice-questions. RESULTS: Overall, 238 pharmacists took part in the survey. 199 (84%) said they were insecure counselling people with epilepsy or had no experience at all. 205 (86%) rightly assumed a seizure can result in death. Concerning an acute seizure, administration of a rescue medicati

Cystatin C: A Primer for Pharmacists.

Pharmacists are at the forefront of dosing and monitoring medications eliminated by or toxic to the kidney. To evaluate the effectiveness and safety of these medications, accurate measurement of kidney function is paramount. The mainstay of kidney assessment for drug dosing and monitoring is serum creatinine (SCr)-based estimation equations. Yet, SCr has known limitations including its insensitivity to underlying changes in kidney function and the numerous non-kidney factors that are incompletely accounted for in equations to estimate glomerular filtration rate (eGFR). Serum cystatin C (cysC)
