Assessment of implementation of the caries management by risk assessment philosophy in a dental school.

OBJECTIVE: This cross-sectional study assessed the implementation of documenting a baseline caries risk assessment (CRA) of patients seen by predoctoral dental students and its association with the presence of caries risk management (CRM) treatment. METHODS: A convenience sample of 10,000 electronic axiUm patient records at Tufts University School of Dental Medicine was retrospectively assessed for the presence or absence of a completed CRA and CRM after IRB approval following predetermined inclusion and exclusion criteria. The CRM variables (nutrition counseling, sealant, fluoride) were iden

Microvascular reconstruction and dental rehabilitation of benign severely atrophic jaws and defects of the alveolar ridge: our philosophy in 2019.

We deal regularly with patients who present with severe atrophy of the jaws, compromised soft tissue, or penetrating defects of the alveolar ridge that are not the result of malignant disease. For these patients we use microvascular bony flaps together with dental implants and implant-supported prostheses. The purpose of this retrospective study was to present our current management and the lessons we have learned over a 16-year period while treating 86 patients for these indications with 87 microvascular bone flaps. We used a transoral approach for the anastomosis in 60 flaps, thereby avoidin

The success of restorative dentistry?

Direct statistical evidence of the value of routine, periodic dental examination and treatment is lacking and inferences can only be drawn from surveys of population samples and groups of people. Except perhaps in areas of exceptionally favourable dentist: population ratios the restorative philosophy makes too high a demand on scarce professional resources for it to answer the treatment needs of populations. However, it was originally developed to benefit individual patients who sought dental care and the evidence at present suggests that it may postpone the loss of teeth. It is hypothesized t

Tissue preservation strategies for fostering long-term soft and hard tissue stability.

As placement of functionally stable dental implants has become routine, concerns have shifted to maintenance of crestal bone and soft tissue stability. This article proposes the development of a tissue preservation philosophy to avoid crestal bone loss and gingival recession and thus foster long-term esthetics around implants. Pillars of this philosophy must include avoidance or minimization of an implant-abutment microgap and micromovement, use of platform switching, appropriate implant positioning relative to the bone crest, and preservation of the papillae when placing both single and multi

[Adherent gingiva: therapeutic approaches as a function of different clinical situations].

Mucogingival surgery has been indicated for many years in cases of insufficiency or absence of attached gingiva. With time results of clinical studies have led to reconsidering this concept. Some authors today believe that systematic increase of attached gingiva is a therapeutic abuse. It is necessary to find a satisfactory attitude in the midst of this debate. Only the careful analysis of the clinical situation considered with the whole patient's dental problem can guide our clinical choice. Our attitude will be different if we are confronted with a natural dentition, prosthetic requirements

Selective and intelligent use of antibiotics in endodontics.

Antibiotics have been used extensively for the management of odontogenic infections since their discovery early this century. Although they have been invaluable in the management of acute and severe infections, many members of the dental and medical professions have unfortunately used these drugs inappropriately for the management of dental pain when "Локально" dental treatment would have sufficed. This, and the general over-use of antibiotics in other situations such as for growth enhancement in animals for human consumption, is leading to the widespread development of strains of bacteria that a

Combined orthodontic and prosthetic therapy. Special considerations.

The adult patient may present with a complex medical and dental history. Comprehensive treatment planning with appropriate consultation among the restorative dentist and other dental specialists is a prerequisite for a successful treatment outcome. This article reviewed the role of orthodontic treatment in the restoration of the partially edentulous patient. The case illustrations presented show the value of collaborative efforts in a multidisciplinary team approach. Although there may be initial reluctance on the part of some patients to seek multiple specialty consultations, the rewards are

Management of Wide Alar Lobule Using Preservation Rhinoplasty Philosophy: Alar Coring Technique.

A broad alar base disrupts nasal harmony and facial aesthetic balance and is an aesthetic concern for many patients. This wide appearance may be related to excessive nostril sill, alar flare, wide alar lobule, or a combination. Many techniques are described in literature to address the alar base with resultant scars to a variable degree. Management of alar lobule thickness is particularly challenging. Our technique presented here introduces a novel method to reduce the thickness of the alar lobule with excellent aesthetic outcome.

Rationale for the use of antimicrobial agents in periodontal disease.

The traditional approach to treating dental decay and periodontal disease has often focused on caries, neglecting periodontal infection. The past 15 years have seen significant advances in the treatment of periodontal disease with antimicrobial therapy, both with and without more traditional debridement or surgery. This article presents an overview of the use of antimicrobials, including an examination of treatment philosophies and the diagnosis of periodontal infection.

Making the Case for Open Dental Education.

Dental education as a field and dentistry as a profession are facing significant challenges, including workforce shortages, ever-rising costs, unmet student and community needs, as well as quality of pedagogical practices. Open educational resources are learning objects that are freely accessible and are able to be retained, reused, revised, remixed, and redistributed under the philosophy of more openness and sharing. While some efforts are being made to address the aforementioned concerns, the prospects of more readily welcoming and adopting open education have not been discussed as a potenti

Non-Operative, Micro- and Minimally Invasive Methods for Caries Treatment-A Narrative Review.

The management of dental caries has evolved from the traditional mechanical approach of "extension for prevention" to a biologically oriented philosophy centered on preserving natural tooth structures. Minimally invasive dentistry (MID) emphasizes early detection, risk assessment, prevention, and conservative intervention based on the lesion's activity and depth. This review outlines current evidence on non-operative, micro-invasive, and minimally invasive strategies, including fluoride therapy, remineralizing agents such as casein phosphopeptide-amorphous calcium phosphate (CPP-ACP), self-ass

Evidence-Based Deep Carious Lesion Management: From Concept to Application in Everyday Clinical Practice.

This chapter aims to discuss the way dental practitioners manage deep carious lesions (DCL) in routine practice and the barriers and incentives/facilitators to changes in accepted practice. In concert with the philosophy of minimal intervention dentistry, the concept of quaternary prevention (actions taken to prevent overtreatment and reduce harm to the patient) emerges in dentistry. This can be applied to carious tissue removal (CTR) in DCL given the risks associated with this procedure (high risk of pulp exposure, pulpal complications, tooth substance loss, and unsuccessful invasive, expensi

Occlusal stability.

Occlusion is the foundation for clinical success in fixed, removable, and implant prosthodontic treatment. Understanding those principles is critical when restoring a patient's occlusion. Many philosophies, devices, and theories of occlusion have evolved based on anecdotal clinical observations and applied geometric perceptions. The literature has reported these classic and contemporary occlusal concepts. As evidence-based dentistry emerged, it championed scrutiny of previously held beliefs, resulting in the abandonment of many pragmatic, yet beneficial occlusal procedures. The impetus toward

21st century endodontics. Part 1.

Root canal treatment techniques probably develop and change more frequently than any other area of dental practice and it can be hard for the busy general dental practitioner to keep up to date. The aim of this series of five papers is to give a thorough review of current teaching and practice. This will encompass the rationale of root canal treatment, guidance on the latest technical procedures for preparation and obturation of the entire root canal system, and consideration of how these principles may contribute to successful treatment. The series will address the following issues: diagnosis

New philosophy for the treatment of avulsed teeth.

The success rate of replanted avulsed teeth has been low. This poor success rate results from the use of only one treatment regimen. This treatment regimen, replantation as quickly as possible regardless of circumstances, ignores much of the dental research that shows this regimen to not be efficacious. In addition, the profession has disregarded much of the research that shows many recommended storage media damaging to avulsed teeth. This article reviews the pertinent research on avulsed teeth and recommends a new treatment philosophy of avulsed teeth that delineates 10 treatment categories b

The dilemma of different dental degrees: DDS and DMD.

Although the Doctor of Dental Surgery (DDS) evolved from the surgical and the Doctor of Medicine in Dentistry (DMD) from the medical (DMD) roots of the dental profession, dental schools in the US currently award both degrees, verified as equivalent by finding no differences between them in standards of admissions, accreditation, or state licensure requirements while continuing to be subjectively perceived as different enough to create professional and public confusion. In contrast, Doctors of Osteopathy (DOs) and Doctors of Medicine (MDs) are both objectively and subjectively perceived as diff
