The 2023 wearable photoplethysmography roadmap.

Photoplethysmography is a key sensing technology which is used in wearable devices such as smartwatches and fitness trackers. Currently, photoplethysmography sensors are used to monitor physiological parameters including heart rate and heart rhythm, and to track activities like sleep and exercise. Yet, wearable photoplethysmography has potential to provide much more information on health and wellbeing, which could inform clinical decision making. This Roadmap outlines directions for research and development to realise the full potential of wearable photoplethysmography. Experts discuss key top

Cardiac arrhythmia detection using photoplethysmography.

Cardiovascular Diseases (CVDs) cause a very large number of casualties around the world every year and cardiac arrhythmias contribute to significant proportion of CVD related deaths. Bedside cardiac activity monitors in hospitals are based on electrocardiogram (ECG) processing and are known to produce too many false alarms. Moving beyond bedside care, ECG is not very suitable for use in wearable devices. Photoplethysmography (PPG) on the other hand provides an inexpensive and more wearable device-friendly alternative. This work presents a technique to detect life threatening arrhythmias using

Identifying airway obstructions using photoplethysmography (PPG).

OBJECTIVE: Central and obstructive apneas are sources of morbidity and mortality associated with primary patient conditions as well as secondary to medical care such as sedation/analgesia in post-operative patients. This research investigates the predictive value of the respirophasic variation in the noninvasive photoplethysmography (PPG) waveform signal in detecting airway obstruction. METHODS: PPG data from 20 consenting healthy adults (12 male, 8 female) undergoing anesthesia were collected directly after surgery and before transfer to the Post Anesthesia Care Unit (PACU). Cardiac arrhythmias classification using photoplethysmography database.

Worldwide, Cardiovascular Diseases (CVDs) are the leading cause of death. Patients at high cardiovascular risk require long-term follow-up for early CVDs detection. Generally, cardiac arrhythmia detection through the electrocardiogram (ECG) signal has been the basis of many studies. This technique does not provide sufficient information in addition to a high false alarm potential. In addition, the electrodes used to record the ECG signal are not suitable for long-term monitoring. Recently, the photoplethysmogram (PPG) signal has attracted great interest among scientists as it provides a non-in

Determinants of photoplethysmography signal quality at the wrist.

Wrist photoplethysmogram (PPG) signals are widely used for physiological monitoring in consumer devices. However, the PPG is highly susceptible to noise, which can reduce the accuracy of monitored parameters. The aim of this study was to identify factors which influence PPG signal quality. Data from the Aurora-BP dataset were used, consisting of reflectance wrist PPG signals measured from 1,142 subjects of varying ages and health statuses. Measurements were acquired in supine, sitting, and standing postures, and with the sensor held at different heights. Three signal quality metrics were calcu

Identification of cerebral infarction using bilateral photoplethysmography.

BackgroundCerebral infarction is often associated with underlying cerebral vascular stenosis, such as carotid artery stenosis or cerebral artery stenosis due to arteriosclerosis. Existing imaging techniques, including carotid ultrasound, computed tomography angiography (CTA), and magnetic resonance angiography (MRA), are useful for diagnosis, but have limitations such as radiation exposure, contrast medium use side effects, and high cost. Therefore, the need for a simple, noninvasive, and cost-effective screening tool is emerging.ObjectiveIn this study, we propose a novel cerebral infarction s

Quantifying Movement in Preterm Infants Using Photoplethysmography.

Long-term recordings of movement in preterm infants might reveal important clinical information. However, measurement of movement is limited because of time-consuming and subjective analysis of video or reluctance to attach additional sensors to the infant. We evaluated whether photoplethysmogram (PPG), routinely used for oximetry in preterm infants in the neonatal intensive care unit (NICU), can provide reliable long-term measurements of movement. In 18 infants [mean post-conceptional age (PCA) 31.10 weeks, range 29-34.29 weeks], we designed and tested a wavelet-based algorithm that detects m

Non-Contact, Simple Neonatal Monitoring by Photoplethysmography.

This paper presents non-contact vital sign monitoring in neonates, based on image processing, where a standard color camera captures the plethysmographic signal and the heart and breathing rates are processed and estimated online. It is important that the measurements are taken in a non-invasive manner, which is imperceptible to the patient. Currently, many methods have been proposed for non-contact measurement. However, to the best of the authors' knowledge, it has not been possible to identify methods with low computational costs and a high tolerance to artifacts. With the aim of improving c

Intelligent Electrocardiogram Acquisition Via Ubiquitous Photoplethysmography Monitoring.

Recent advances in machine learning, particularly deep neural network architectures, have shown substantial promise in classifying and predicting cardiac abnormalities from electrocardiogram (ECG) data. Such data are rich in information content, typically in morphology and timing, due to the close correlation between cardiac function and the ECG. However, the ECG is usually not measured ubiquitously in a passive manner from consumer devices, and generally requires 'active' sampling whereby the user prompts a device to take an ECG measurement. Conversely, photoplethysmography (PPG) data are typ

Photoplethysmography wave morphology in patients with atrial fibrillation.

Objective. Most current algorithms for detecting atrial fibrillation (AF) rely on heart rate variability (HRV), and only a few studies analyse the variability of photopletysmography (PPG) waveform. This study aimed to compare morphological features of the PPG curve in patients with AF to those presenting a normal sinus rhythm (NSR) and evaluate their usefulness in AF detection.Approach.10 min PPG signals were obtained from patients with persistent/paroxysmal AF and NSR. Nine morphological parameters (1/ΔT), Pulse Width [PW], augmentation index [AI], b/a, e/a, [b-e]/a, crest time [CT], inflectio

[Ultrasonic dopplerography and photoplethysmography in the diagnosis of Raynaud's syndrome].

Data on the studies of the blood flow in finger vessels in 45 patients with Raynaud's disease by means of ultrasonic dopplerography and photoplethysmography are given. The control group consisted of 30 persons. The use of a cryotest showed significant differences in arterial segmental blood flow of the hand fingers in patients with Raynaud's disease and in those of the control group. The mentioned methods are noninvasive, give sufficiently informative results providing an objective assessment of the nature of vascular disorders in Raynaud's phenomenon of different genesis.

Noninvasive assessment of local nicotinate pharmacodynamics by photoplethysmography.

The local pharmacodynamics of a topical vasodilator (methyl nicotinate) has been followed noninvasively using photopulse plethysmography. This technique is sensitive to changes in blood flow through the cutaneous microcirculation and responds to the pharmacologic stimulus of the vasoactive agent employed. Five different application sites for the drug were studied and the time course of the local effect (i.e., onset, duration, and decay) was recorded. The applied amount of drug elicited, within a short period, a response which was saturable such that the observed increase in blood flow reached

Assessment of the healing potentials of ulcers of the skin by photoplethysmography.

An objective method of assessing the healing potentials of skin ulcers would eliminate the long waiting period necessary for visual evidence of healing or nonhealing to appear. This would permit the continued conservative treatment for healing ulcers and prompt surgical interventions for those ulcers which are nonhealing. In this preliminary study, the cutaneous blood flow immediately around the ulcer site was studied using the photoplethysmograph. Vigorous cutaneous blood flow shown by good pulsatile wave forms on the photoplethysmograph is shown to be indicative of an ongoing healing process

Photoplethysmography parameters in monitoring nociception during general anesthesia.

BACKGROUND: Validated monitoring methods for evaluating the balance of nociception and anti-nociception (BNAN) are needed in general anesthesia. This study assessed six photoplethysmography (PPG) parameters, computed from finger photoplethysmographic waveforms in patients undergoing gynecological surgery under general anesthesia. METHODS: A total of 20 participants were included, each undergoing general anesthesia with propofol and remifentanil. The same concentration of remifentanil was maintained throughout the experiment, four different intensities of electrical stimulation were administer

Robust Heart Rate Detection via Multi-Site Photoplethysmography.

Smartwatches have become popular for monitoring physiological parameters outside clinical settings. Using reflective photoplethysmography (PPG) sensors, such watches can noninvasively estimate heart rate (HR) in everyday environments and throughout a patient's day. However, achieving consistently high accuracy remains challenging, particularly during moments of increased motion or due to varying device placement. In this paper, we introduce a novel sensor fusion method for estimating HR that flexibly combines samples from multiple PPG sensors placed across the patient's body, including wrist,

Dual Wavelength Photoplethysmography Framework for Heart Rate Calculation.

The quality of heart rate (HR) measurements extracted from human photoplethysmography (PPG) signals are known to deteriorate under appreciable human motion. Auxiliary signals, such as accelerometer readings, are usually employed to detect and suppress motion artifacts. A 2019 study by Yifan Zhang and his coinvestigatorsused the noise components extracted from an infrared PPG signal to denoise a green PPG signal from which HR was extracted. Until now, this approach was only tested on "micro-motion" such as finger tapping. In this study, we extend this technique to allow accurate calculation of

Cuffless Single-Site Photoplethysmography for Blood Pressure Monitoring.

One in three adults worldwide has hypertension, which is associated with significant morbidity and mortality. Consequently, there is a global demand for continuous and non-invasive blood pressure (BP) measurements that are convenient, easy to use, and more accurate than the currently available methods for detecting hypertension. This could easily be achieved through the integration of single-site photoplethysmography (PPG) readings into wearable devices, although improved reliability and an understanding of BP estimation accuracy are essential. This review paper focuses on understanding the fe

Mathematical Modeling of Photoplethysmography: Model Assessment and Validation.

PURPOSE: Photoplethysmography (PPG) is a well-known technique employed to assess optically perfused bio-tissue volume changes. A PPG apparatus consists of a light emitter and a receptor. The analysis of the received light is used to infer properties of the illuminated tissues. This paper aims at presenting a novel distributed mathematical model for PPG signals, which combines a poroelastic model of tissue perfusion with a diffusion model for light absorption and scattering. METHODS: We assume that the tissues undergo small deformations, allowing for a linear poroelastic description of perfusi

Quantification of the Phenomena Affecting Reflective Arterial Photoplethysmography.

Photoplethysmography (PPG) is a widely emerging method to assess vascular health in humans. The origins of the signal of reflective PPG on peripheral arteries have not been thoroughly investigated. We aimed to identify and quantify the optical and biomechanical processes that influence the reflective PPG signal. We developed a theoretical model to describe the dependence of reflected light on the pressure, flow rate, and the hemorheological properties of erythrocytes. To verify the theory, we designed a silicone model of a human radial artery, inserted it in a mock circulatory circuit filled w

A Supervised Approach to Robust Photoplethysmography Quality Assessment.

Early detection of Atrial Fibrillation (AFib) is crucial to prevent stroke recurrence. New tools for monitoring cardiac rhythm are important for risk stratification and stroke prevention. As many of new approaches to long-term AFib detection are now based on photoplethysmogram (PPG) recordings from wearable devices, ensuring high PPG signal-to-noise ratios is a fundamental requirement for a robust detection of AFib episodes. Traditionally, signal quality assessment is often based on the evaluation of similarity between pulses to derive signal quality indices. There are limitations to using thi
