Pilot demonstration for physician compensation incentives to improve diagnostic specificity in clinical documentation.

OBJECTIVES: Omissions in clinical documentation hinder diagnosis by obscuring important information and delaying communication among clinicians. Additionally, carefully implemented incentives can improve health care processes such as clinical documentation. Therefore, we created a pilot using $20,000 in compensation incentives to improve free text documentation for six common outpatient diagnoses (cerebrovascular accidents, chronic kidney disease, congestive heart failure, deep vein thrombosis and pulmonary embolism, chronic obstructive pulmonary disease, and smoking) in an urban health system

The effect of financial incentives on the quality of health care provided by primary care physicians.

BACKGROUND: The use of blended payment schemes in primary care, including the use of financial incentives to directly reward 'performance' and 'quality' is increasing in a number of countries. There are many examples in the US, and the Quality and Outcomes Framework (QoF) for general practitioners (GPs) in the UK is an example of a major system-wide reform. Despite the popularity of these schemes, there is currently little rigorous evidence of their success in improving the quality of primary health care, or of whether such an approach is cost-effective relative to other ways to improve the qu

Have we got a deal for you, doc. Small towns recruiting big time to lure physicians to rural Texas.

Ask most folks if they would trade the crime, high taxes, traffic nightmares, and general hassle of big cities for the pastoral setting of small-town life, and their first question may be what time the moving van arrives. Life in a small town can be peaceful, quiet, and relaxing: just the tonic for the headaches of modern urban life. Then reality in the form of illness intrudes, and the services of a doctor are needed. Unfortunately, in many rural areas of Texas, one may not be available.

Physician incentives and the timing of cesarean sections: evidence from California.

OBJECTIVES: The timing of cesarean sections is studied to examine how physician convenience and financial incentives play a role in the decision to perform a cesarean section. METHODS: Using birth certificate and hospital financial data from California, the likelihood of cesarean sections being performed at particular times of day was examined, controlling for maternal characteristics and the mother's insurance coverage. Two diagnoses associated with cesarean sections are examined separately: fetal distress and prolonged/dysfunctional labor. The hypotheses are that cesarean sections performed

A Payment Incentive to Improve Confirmatory Testing in Men With Prostate Cancer.

IMPORTANCE: Among men with favorable-risk (ie, low-risk or favorable intermediate-risk) prostate cancer, confirmatory testing substantially improves the detection of aggressive cancers that may merit treatment instead of conservative management. Despite guideline recommendations, confirmatory testing is inconsistently used, and more than half of men do not receive it. Value-based interventions and payment incentives may improve care quality by motivating adherence to guideline-concordant care. OBJECTIVE: To examine the use of confirmatory testing among men with low-risk prostate cancer, after

Evaluating the Consequences of a Hypertension Management Incentive.

IMPORTANCE: Quality metrics with financial incentives are widely used, but their impact on clinical care and patient health remains challenging to isolate. OBJECTIVE: To evaluate the association of a physician-facing quality metric and financial incentive for hypertension control (blood pressure <140/90 mm Hg) with clinical decisions and health outcomes. DESIGN, SETTING, AND PARTICIPANTS: This quasi-experimental difference-in-differences study in a large US health system compared changes in outcomes at practices that did vs those that did not adopt the financial incentive before (2021) vs af

Relationship between physician financial incentives and clinical pathway compliance: a cross-sectional study of 18 public hospitals in China.

OBJECTIVES: Many strategies have been either used or recommended to promote physician compliance with clinical practice guidelines and clinical pathways (CPs). This study examines the relationship between hospitals' use of financial incentives to encourage physician compliance with CPs and physician adherence to CPs. DESIGN: A retrospectively cross-sectional study of the relationship between the extent to which patient care was consistent with CPs and hospital's use of financial incentives to influence CP compliance. SETTING: Eighteen public hospitals in three provinces in China. PARTICIPAN

"Mind the gap!" Evaluation of the performance gap attributable to exception reporting and target thresholds in the new GMS contract: National database analysis.

BACKGROUND: The 2003 revision of the UK GMS contract rewards general practices for performance against clinical quality indicators. Practices can exempt patients from treatment, and can receive maximum payment for less than full coverage of eligible patients. This paper aims to estimate the gap between the percentage of maximum incentive gained and the percentage of patients receiving indicated care (the pay-performance gap), and to estimate how much of the gap is attributable respectively to thresholds and to exception reporting. METHODS: Analysis of Quality Outcomes Framework data in the Na

Paying for quality improvement: compliance with tobacco cessation guidelines.

BACKGROUND: Identification and treatment of routine tobacco use in medical practice is an effective intervention but is not used consistently. A study was conducted at HealthPartners, a large network-model health plan in Minnesota, to determine the effect of an outcomes recognition strategy that involved bonus funds and the rates at which network physicians document that tobacco users are identified and advised to quit. METHODS: Audits of 14,489 ambulatory patient records from 19-20 medical groups were conducted to determine the proportion of charts from each medical group that demonstrated i

Health-insurance products and plan options.

Trends in health insurance are discussed, with emphasis on insurers' efforts to offer an array of cost-effective plans tailored to the needs of employers and subscribers. Health-insurance companies, responding to employers' demands to curtail the rising costs of premiums, now offer a variety of insurance products. While indemnity plans, health maintenance organizations (HMOs), and preferred-provider organizations (PPOs) remain as the three basic types of plans, insurers are combining these elements in different ways, creating dual- and triple-option plans that consist of indemnity insurance an

Recruiting physicians without inviting trouble.

Many hospitals use physician recruitment strategies--generally assistance or employment strategies--to ensure medical staff loyalty. Although these strategies appeal to both hospitals and physicians, they are becoming increasingly problematic. Over the past three years, the government has issued pronouncements that question their legality. Thus any hospital considering physician recruitment strategies would be wise to evaluate them in light of various legal issues. such as reimbursement, nonprofit taxation, corporate practice of medicine, and certificate-of-need statutes. The consequences of f

Valuing the primary care patient base.

This analysis, albeit limited, can help clinics and integrated health systems with their medical staff planning, and it demonstrates the importance of the primary care patient base as a foundation for health services planning and economic and market strategies. It is important to note, however, that when primary, specialty, subspecialty, surgical, and hospital care are well planned and marketed, it is virtually impossible to identify the relative contribution of each to the overall success of the system. Consequently, primary care physicians should not attribute the value discussed here solely

[Recruitment of pediatric practices for an intervention study: strategies, implementation, and insights from the Intervention Study to Increase HPV Vaccination Coverage in Germany (InveSt HPV)].

INTRODUCTION: Recruiting healthcare facilities for intervention studies is a well-documented challenge, yet evidence on effective strategies remains limited. To strengthen this evidence base, we report and discuss the recruitment approach applied in the Intervention Study to Increase HPV Vaccination Coverage in Germany (InveSt HPV). METHODS: Pediatric practices in Bremen and selected districts in Bavaria were contacted by mail and through the German Professional Association of Pediatricians (BVKJ). Each participant received incentives of up to € 200. RESULTS: In Bremen, 14 of 39 practices (3

Making Sense: Treating Patients with Opioid Use Disorder through the Lens of Game Theory.

The opioid epidemic continues to influence the field of medicine, creating new challenges and obstacles to quality care. Patients with injection drug use are marginalized individuals who received poor quality of care and often discharged without safe recovery plan. Cooperation between physicians and patients allow the best outcomes for the patient, the physician, and society, however we often see patient-directed discharges and inadequate care. We believe that this result is due to an incentive model in the decision-making process that ultimately makes cooperation difficult. We use different g

Exploring physician specialist response rates to web-based surveys.

BACKGROUND: Survey research in healthcare is an important tool to collect information about healthcare delivery, service use and overall issues relating to quality of care. Unfortunately, physicians are often a group with low survey response rates and little research has looked at response rates among physician specialists. For these reasons, the purpose of this project was to explore survey response rates among physician specialists in a large metropolitan Canadian city. METHODS: As part of a larger project to look at physician payment plans, an online survey about medical billing practices

Fitness, knowledge, progress: assessing physician qualification.

The informed and empowered consumer is an ideal invoked by many would-be health care reformers. An actual consumer wishing to don the mantle of power may be hindered by the scarcity of information available, particularly with respect to choosing among physicians. How is one to know who is best qualified? This issue brief looks at the basics of physician qualification and the processes by which physicians are licensed, credentialed, and board-certified. It examines how the evolution of these processes (for example, the move from lifetime certification to ongoing maintenance of certification) af
