Comparison of an online self-referral tool with a physician-based referral strategy for early recognition of patients with a high probability of axial spa.

OBJECTIVES: The diagnostic delay in axial spondyloarthritis (axial SpA) remains unacceptably high, with one of the reasons being a late referral. Structured physician-based referral programs are able to improve early diagnosis, but lack of implementation is still an issue. The objective of this study was to evaluate an online self-referral (OSR) tool for patients with back pain and to compare it to an established physician-based referral tool. METHODS: Patients with back pain were included if they either fulfilled the requirements of the OSR tool or were referred by a physician using the Berl

Physician self-referral: frequency of negative findings at MR imaging of the knee as a marker of appropriate utilization.

PURPOSE: To determine whether ownership of magnetic resonance (MR) imaging equipment by ordering physicians affects the likelihood of positive findings at MR imaging of the knee and to evaluate rates of knee abnormalities seen at MR imaging as a metric for comparison of utilization. MATERIALS AND METHODS: The institutional review board approved this retrospective HIPAA-compliant study and waived the need for informed consent. A retrospective review was performed of consecutive diagnostic MR images of the knee interpreted by one radiology practice between January and April 2009 for patients wh

Comparing the self-reported referral and management preferences of pediatricians and family physicians for children with juvenile rheumatoid arthritis.

OBJECTIVE: The symptoms of juvenile rheumatoid arthritis (JRA) are often first recognized by primary care physicians. Little is known about the determinants of the initial management and referral patterns of these physicians for children with JRA. We compared the self-reported preferences and practices of pediatricians (PD) and family physicians (FP) in the diagnosis and management of children with JRA. METHODS: Surveys were mailed to a national random sample of 700 PD and 867 FP. Questions included prior experience with JRA, usual patterns in the diagnosis and management of JRA, perception o

Screening, brief intervention, and referral to treatment training for Nigerian primary care physicians: A pilot evaluation of knowledge, attitudes, self-efficacy, and barriers to implementation.

Substance use disorder (SUD) poses a significant public health challenge in sub-Saharan Africa, where evidence-based approaches such as Screening, Brief Intervention, and Referral to Treatment (SBIRT) are rarely integrated into primary healthcare. In Nigeria, primary care providers often lack substance use prevention training, limiting their ability to identify and manage individuals with risky substance use patterns. This pilot study is the first documented evaluation of structured SBIRT training delivered to physicians within Nigeria's primary healthcare system, assessing changes in self-rep

Mammography screening and the self-referred woman.

There are no easy answers to the question of whether a mammography screening practice should accept self-referred women. The major reason for doing so is the fact that many women are not being screened either because their primary care physicians do not initiate mammography screening referrals or because they do not have a physician at all. However, this compelling argument is at least partially countered by the numerous problems encountered when practices accept self-referred women, including potential adverse medicolegal consequences, tedious and time-consuming reporting requirements, the po

Physicians' tacit and stated policies for determining patient benefit and referral to cardiac rehabilitation.

BACKGROUND / PURPOSE: The benefits of prescribing cardiac rehabilitation (CR) for patients following heart surgery is well documented; however, physicians continue to underuse CR programs, and disparities in the referral of women are common. Previous research into the causes of these problems has relied on self-report methods, which presume that physicians have insight into their referral behavior and can describe it accurately. In contrast, the research presented here used clinical judgment analysis (CJA) to discover the tacit judgment and referral policies of individual physicians. The speci

Compliance with the screening mammography program of British Columbia: will she return?

OBJECTIVE: To identify factors associated with compliance in the Screening Mammography Program of British Columbia (SMPBC). METHOD: Factors associated with rescreening within 18 months (annual compliers) and between 18 to 36 months later (late compliers) were identified in a cohort of SMPBC screenees using a self-administered questionnaire. RESULTS: Fewer than half of women initially screened within the SMPBC were annual compliers, nearly 40% not returning by 3 years. In women age 50+ years, annual compliers tended to have no prior mammography, no prior breast pain, a physician referral to S

Effectiveness of smoking cessation interventions integrated into primary care practice.

Using a complete factorial design, we tested three interventions for smoking cessation in routine primary care practice. The interventions tested were 1) physician counseling, 2) mailed letters and educational materials designed by the National Cancer Institute (NCI), and 3) referral to smoking cessation classes. Thirty-seven family practice physicians at three of Group Health's outpatient facilities participated. Patient participation rates were 95%, and follow-up was complete for 92% of those participating. None of the interventions had any effect on point prevalence of quitting as determine

The Characteristics and Outcomes of Patients Who Make an Emergency Department Visit for Hypertension After Use of a Home or Pharmacy Blood Pressure Device.

STUDY OBJECTIVE: Emergency department (ED) visits for hypertension are rapidly increasing. Hypertension guidelines began recommending use of self-measurement blood pressure (BP) devices in the early 2000s, which could be contributing to the increase. We aim to examine the proportion of ED visits for hypertension that occurred after a BP measurement with a self-measuring device, and the associated outcomes. METHODS: This retrospective observational study included adults who made an ED visit and received a primary diagnosis of hypertension at 1 of 5 community and tertiary hospitals in Ontario,

Radiology in family practice: experience in community practice.

This study examined the utilization and interpretation of office x-rays in community practice. Analysis of physician and practice descriptive data obtained by survey from the Minnesota Academy of Family Physicians Research Network, a representative sample of family physicians in Minnesota, leads to these conclusions: (1) Most (87.3%) of the study physicians have on-site x-ray facilities. (2) An extensive variety of x-rays is obtained in the office. (3) Most (54.2%) family physicians interpret their own x-rays and self-select those they want re-read by a radiologist; 43.8% referred all x-rays t

Association between a structured training programme and self-reported clinical inertia in type 2 diabetes management in primary care.

This non-randomised, post-intervention–only controlled cross-sectional study examined the association between participation in the Turkish Diabetes Foundation’s face-to-face training and patient counselling programme (Diabetes 01) and self-reported clinical decision-making among primary care physicians managing type 2 diabetes mellitus (T2DM). The programme was implemented in Adana, southern Turkey, between November 2021 and June 2022. After programme completion, a standardised questionnaire evaluating treatment initiation, treatment modification, and referral practices was administered to 118

The self-referred mammography patient: a new responsibility for radiologists.

A mammography screening program was initiated in which self-referred women were accepted for examination. Two views of each breast were obtained, and no physical examination was performed. Reports were sent to each patient and to the patient's primary care physician, if she had one. The program was successful in that the number of examinations performed per day increased from 36 to 80 within 6 months. Approximately 50% of the women who came for screening did so at their own request. Self-referred women with abnormal findings on mammograms who did not have a primary care physician were contacte

Sexual dysfunction.

Sexual dysfunction may adversely affect a woman's self-esteem and her overall sense of well-being. The obstetrician-gynecologist can play an important role by asking the patient about any problems with sexual function. Physiologic changes, disease, surgery, or medical therapy may result in sexual dysfunction. The physician should recognize the potential effects of these conditions on a patient's sexuality and provide the patient with appropriate education and counseling. Patients should also be advised about alternatives for sexual expression, particularly after pelvic surgery. Specific suppor

Clinical evaluation of high-risk HPV detection on self-samples using the indicating FTA-elute solid-carrier cartridge.

BACKGROUND: High-risk human papillomavirus (hrHPV) testing in cervical screening is usually performed on physician-taken cervical smears in liquid-based medium. However, solid-state specimen carriers allow easy, non-hazardous storage and transportation and might be suitable for self-collection by non-responders in screening and in low-resource settings. OBJECTIVES: We evaluated the adequacy of self-collected cervicovaginal (c/v) samples using a Viba-brush stored on an Indicating FTA-elute cartridge (FTA-based self-sampling) for hrHPV testing in women referred to a gynecology clinic due to an
