Noise levels in physicians' offices.

The development of hand-held auditory screening devices has led to increased hearing screening by physicians. Ambient noise levels were assessed in examination rooms of 20 physician offices. Measurements provided Ln, Leq, Lmax, and octave band data for specialty types: pediatrics, internal medicine, family practice and otolaryngology. Such data should not replace the need for individually monitoring potential screening sites.

Distribution of private practice offices of physicians with specified characteristics among urban neighborhoods.

The office location of New York County physicians in private practice is determined for a verified subpopulation of the "Medical Directory of New York State" listing for the year 1970-71. Physicians are categorized by specialty status, medical schools attended, dates of graduation, and hospital affiliations. Multiple regression analysis is performed to relate numbers of physicians per Health Area (average population: 20,000) to demographic, economic, and medical care characteristics of the areas. There is considerable unevenness in the number of private physician offices among Health Areas (me

Assessing dalfampridine efficacy in the physician's office.

Dalfampridine (extended release 4-aminopyridine) is shown in three recent randomised controlled trials to improve walking speed in people with multiple sclerosis; however, the trial literature makes it clear that dalfampridine is effective in only a subset of patients. For the neurologist working in an everyday physician's office, a key question arises: How to distinguish the few who experience a meaningful clinical benefit, from the many who do not? This question has not yet been adequately addressed in the available literature.

Incidence of Accommodations for Patients With Significant Vision Limitations in Physicians' Offices in the US.

IMPORTANCE: Despite documented disparities in health care for patients with significant vision impairments and legal mandates that patients with disability receive equitable care, little is known about the extent to which physicians practicing in the US accommodate these patients in outpatient clinical settings. OBJECTIVE: To empirically explore the extent of basic accommodations physicians practicing in the US provide to patients with significant vision limitations in outpatient care. DESIGN, SETTING, AND PARTICIPANTS: In this physician survey study, randomly selected physicians were survey

[Diabetic Neuropathy. Diagnosis and Treatment in the Physician's Office].

The most common form of diabetic neuropathy is the distal symmetrical sensorimotor variety. It is all the more likely to develop, the longer diabetes mellitus has been clinically manifest. Clinical signs of this neuropathy include the loss of the Achilles tendon reflex, sensory disorders beginning distally, distal muscle paresis and such signs of autonomic nerve involvement as erectile disorders, disordered bladder emptying, diaphoretic disorders or orthostatic hypotension. The diagnostic work-up should also include a number of laboratory investigations to exclude other causes of neuropathy. T

Urinalysis: its proper role in the physician's office.

Urine study is an important aspect of physician's office laboratories. The contributions that urine study can make are important for both the patient and the physician. State-of-the-art technology makes it possible to have urine test results available at the time the patient is being examined without inconvenience to the patient, the physician, or the laboratory. The cost-containment contributions of urine study are significant for both the patient and the physician. The analytes currently studied provide a broad spectrum of information that relates to many of the problems presented by the typ

[Management of community-acquired pneumonia in the physician's office].

The classical form of a community-acquired pneumonia is that caused by infection with pneumococcus, and differs clinically from atypical pneumonia in particular by fever-related differences. The diagnosis is based on the five cardinal symptoms: fever, cough, sputum production, pleuritic chest pain and a pulmonary infiltrate. Depending on whether there are further risk factors involved, a more or less comprehensive clinical and laboratory diagnostic work-up is needed. As a rule, oral antibiotic treatment with aminopenicillin, macrolides, fluorchinolones, ketolides or cephalosporins is applied,

Utilization of physician offices by adolescents in the United States.

UNLABELLED: Recent guidelines for adolescent primary care call for the specification of clinical services by three adolescent age subgroups. Yet analyses of office visits have either merged adolescence into one stage or divided it at age 15 years. OBJECTIVE: To explore the utilization of physician offices in the United States by early (11-14 years), middle (15-17 years), and late (18-21 years) adolescents. DESIGN: Secondary analysis of the 1994 National Ambulatory Medical Care Survey, focusing on visits made by the three adolescent age groups. SETTING: Nationally representative sample of 24

Infection control in physicians' offices. Academy of Pediatrics. The American Occupational Safety and Health Administration (OSHA).

Infection control is an integral part of pediatric practice in outpatient settings as well as in hospitals. All employees should be educated regarding the routes of transmission and techniques used to prevent transmission of infectious agents. Policies for infection control and prevention should be written, readily available, updated annually, and enforced. The Centers for Disease Control and Prevention standard precautions for hospitalized patients with modifications from the American Academy of Pediatrics are appropriate for most patient encounters. As employers, pediatricians are required b

Prevalence and epidemiology of onychomycosis in patients visiting physicians' offices: a multicenter canadian survey of 15,000 patients.

A prospective, multicenter study to determine the epidemiology of onychomycosis was performed in the offices of 3 dermatologists and 1 family physician in Ontario, Canada. In the sample of 15,000 patients, abnormal-appearing nails were observed in 2505 persons (16. 7%). There were 1199 patients (8%) with toenail or fingernail onychomycosis confirmed on mycologic examination, with 1137 patients (7.6%) who had only pedal onychomycosis, 40 patients with toenail and fingernail onychomycosis (0.27%), and 22 patients (0.15%) with only fingernail onychomycosis. The condition was more common in male p

Accommodating hearing loss in outpatient physician offices in the U.S.

BACKGROUND: Approximately 16.2 million Americans ages 18 and older (5.7%) report being deaf or having serious difficulty hearing. Hearing loss impedes effective communication during clinical encounters putting patients' safety at risk. A large fraction of Americans with Disabilities Act lawsuits addresses whether providers offered auxiliary aids and services required to ensure effective communication. OBJECTIVE: Examine use of different hearing accommodations for deaf or hard of hearing patients by U.S. physicians. METHODS: We surveyed randomly selected physicians nationwide representing 7 s

Antibiotic recommendations of office-based physicians, 2007-2011.

OBJECTIVE: To describe patterns of antibiotic recommendations by office-based physicians from five regions in Canada between 2007 and 2011. METHODS: Values were estimated based on quarterly data from a sample of approximately 652 physicians stratified by region and specialty. For four consecutive quarters, each physician maintained a practice diary describing information on every patient visit during a randomly selected 48-hour period. This information was then extrapolated using a projection factor to estimate prescriptions by all physicians across Canada. RESULTS: Over the five-year survei
