The placebo effect in agoraphobia.

This paper presents two sets of data that suggests a weak but specific placebo response in agoraphobia. First, analyses in 20 agoraphobic patients given single-blind placebo over a 2-week period, without the customary confound of instructions for exposure to phobic situations, revealed a statistically significant reduction in panic and phobic symptoms. However, symptoms remained in the moderate to severe range and functioning was virtually unchanged. Second, comparisons between six agoraphobic patients receiving double-blind placebo and six others receiving "no pills," matched for age, sex and

Justice for Placebo: Placebo Effect in Clinical Trials and Everyday Practice.

The placebo effect has been widely documented across various medical conditions, demonstrating its ability to influence both subjective and objective outcomes. Placebo responses can significantly improve symptoms in these different conditions, such as pain, Parkinson's disease, depression, anxiety, and addiction. Psychological mechanisms, particularly the power of patient expectations, appear to play a central role, with neurobiological evidence supporting the activation of dopamine, endogenous opioids, and endocannabinoids in response to placebo interventions. Studies have demonstrated that p

The placebo effect in agoraphobia--II.

Analyses in 44 agoraphobic patients given single-blind placebo over a two-week period, without the customary confound of instructions of exposure to phobic situations, replicated previous findings of a weak placebo response in that there were statistically, but not clinically, significant reductions in panic and phobic symptoms. Further analyses of a representative subsample of 10 patients who continued to receive placebo revealed that the placebo response was maintained and even increased at the end of 10 weeks when 20% to 30% of the patients could be classified as marked responders on key pa

The placebo effect in psoriatic arthritis.

The placebo effect represents a clinically relevant and underexplored phenomenon in modern medicine with significant implications for treatments, particularly in rheumatological conditions, as placebo groups in clinical trial commonly achieve some degree of subjective and objective response, with an unclear correlation from the routine clinical observations. Emerging evidence highlights that the placebo effect extends beyond symptom perception, modulating markers of inflammation through neuroimmune pathways; however, these mechanisms remain poorly understood. In psoriatic arthritis (PsA), the

The powerful placebo effect in osteoarthritis.

Osteoarthritis (OA) is the most common form of arthritis. Pain and its related function and stiffness are currently the major symptoms and primary outcomes for treatment. However, the treatment in the past has been primarily targeting on the peripheral changes in the joint that has led to suboptimal outcomes. Recently, we find that people with OA respond better to treatment which targets on both peripheral and central pain abnormalities. We also find that placebo per se is very effective for OA. On average 75% pain reduction, 71% functional improvement and 83% stiffness improvement in the trea

Placebo effect in canine epilepsy trials.

BACKGROUND: The placebo effect is a well-recognized phenomenon in human medicine; in contrast, little information exists on the effect of placebo administration in veterinary patients. HYPOTHESIS: Nonpharmacologic therapeutic effects play a role in response rates identified in canine epilepsy trials. ANIMALS: Thirty-four dogs with epilepsy. METHODS: Meta-analysis of the 3 known prospective, placebo-controlled canine epilepsy trials. The number of seizures per week was compiled for each dog throughout their participation in the trial. Log-linear models were developed to evaluate seizure freq

Placebo Responses and Placebo Effects in Functional Gastrointestinal Disorders.

Much has been written about the placebo effects in functional gastrointestinal disorders (FGD), especially in irritable bowel syndrome (IBS), driven by the early hypothesis that in randomized controlled trials (RCTs) of IBS, the placebo effect might be specifically high and thus, corrupts the efficacy of novel drugs developed for this condition. This narrative review is based on a specific search method, a database (www.jips.online) developed since 2004 containing more than 4,500 papers (data papers, meta-analyses, systematic reviews, reviews) pertinent to the topic placebo effects/placebo res

Placebo responses and placebo effects in functional bowel disorders.

The nature and determinants of the placebo response are widely unknown, as are the underlying psychological and biological mechanisms. Placebo response rates in functional bowel disorders (functional dyspepsia, irritable bowel syndrome) trials are similar to those in nonintestinal pain conditions and are comparable with other organic gastrointestinal diseases (duodenal ulcer, inflammatory bowel diseases). In this narrative review, different methodologies (meta-analyses, reanalyses, and experimental setups) are discussed that have been applied to the study of the placebo response in functional

[Nocebo effect: the other side of placebo].

Administration of drugs is often followed by beneficial (placebo effects) and harmful (nocebo effects) effects that are not always related to their mechanism of action. Nocebo effects are rather unknown even when may be the source of many adverse reactions which could be erroneously attributed to drug therapy. Some mechanisms have been postulated which might be associated with the development of nocebo effects. Expectancy, learning and classical conditioning are probably important in the psychological domain. The neuropharmacological substrate is much less known yet an opioid peptide-cholecyst

Placebo Effect in Obsessive-Compulsive Disorder (OCD). Placebo Response and Placebo Responders in OCD: The Trend Over Time.

BACKGROUND: Placebo response appears to be increasing in antidepressant, antipsychotic and various internal medicine trials. A similar trend has been reported for OCD during 1989-1999. Placebo response is generally considered as the extent to which placebo treatment is associated with core symptom improvement. In this analysis, we used Joinpoint regression to assess the time trend of both placebo response and placebo responder rates according to the year of publication with no time restriction in OCD drug trials. METHODS: We included drug and/or psychotherapy trials vs. placebo from PubMed, E

Placebo effect in subjects with cognitive impairment.

Understanding both the placebo response and the underlying disease progression is crucial to designing clinical trials and interpreting results in Alzheimer's disease (AD) research. The disease severity at different stage of disease (e.g., mild cognitive impairment (MCI), early or late AD) is related to the rate of disease progression, which make it even difficult for AD researchers to understand the clinical trial results. A model-based meta-analysis approach using all available historical data provides quantitative understanding of placebo effect and disease progression in AD and offers a us

Placebo and nocebo effect: a mini-review.

It is well-known that placebo is a substance without medical effects, which benefits the health status because of the patient's belief that the substance is effective and that the nocebo is defined as a substance without medical effects but which worsenes the health status of the person taking it by the negative beliefs and expectations of the patient. Starting with the history of the placebo effect and giving a review of the most significant studies reporting about the placebo effect from 1939-2013 it was our intention to give the all-around look on this phenomena discussing the neurobiologic

Naltrexone may block euphoria-like placebo effect.

Naltrexone is a first-line treatment for alcohol use disorders in North America and Europe. It was prescribed to a 63-year-old patient in order to help control amounts of alcohol consumed per drinking occasion. The patient experienced a paradoxical, but consistent side effect of feeling inebriated each time he took naltrexone. In order to investigate this phenomenon we administered naltrexone and a placebo in a randomised double-blind fashion. The patient exhibited a 'high-like' response to all placebo capsules and a decrease in the subjectively perceived euphoria shortly after ingestion of na

[The placebo effect in inflammatory bowel disease].

Since time immemorial, placebos have been used in the treatment of patients with very different diseases. Interest in these substances has recently been renewed, since the use of placebo-controlled clinical trials has become the most useful tool to demonstrate the efficacy of a particular drug in patients with specific diseases. In inflammatory bowel disease (IBD), both in Crohn's disease and in ulcerative colitis, highly significant results have been obtained in patients in the placebo arm of clinical trials, surpassing even those achieved with drugs in other studies. Patients with IBD show c

Visual placebo and nocebo effects.

Placebo and nocebo effects modulate symptom perception through expectations and learning processes in various domains. Predominantly, their impact has been investigated on pain and physical performance. However, the influence of placebos and nocebos on visual system functionality has yet to be explored. The present study aimed to test whether placebo and nocebo effects can intervene in altering participants' performance outcomes during a novel visual accuracy task and to examine the underlying neural mechanisms through EEG. After performing a baseline session, visual accuracy was said to be en

Effects of aspirin, naloxone and placebo.

This study was designed to examine the effects of aspirin, naloxone and placebo treatment on serum beta-endorphin concentration and joint pain in patients with rheumatoid arthritis (RA). Ten patients with definite or classical RA were studied. All treatments were administered in a randomized sequence. On each study day, the following measurements were carried out at specified time intervals: serum beta-endorphin concentration, serum salicylate concentration and joint pain score on a visual analogue horizontal scale. We conclude that in patients with rheumatoid arthritis suffering from chronic

Placebo and nocebo effects in headaches.

As in the treatment of other pain conditions, the placebo and nocebo effects play an important role in the treatment of primary headache disorders, which are the most prevalent neurologic conditions worldwide with enormous personal and societal burden. Meta-analyses of randomized controlled trials (RCTs) for migraine prophylaxis revealed that 8 out of 20 participants treated with placebo experienced adverse events (AEs) and 1 out of 20 participants treated with placebo withdrew treatment because of AEs. Notably, the AEs in placebo groups mirrored the AEs expected of the medication in test, con
