Advanced Imaging Techniques for Radiotherapy Planning of Gliomas.

The accuracy of target delineation in radiation treatment (RT) planning of cerebral gliomas is crucial to achieve high tumor control, while minimizing treatment-related toxicity. Conventional magnetic resonance imaging (MRI), including contrast-enhanced T1-weighted and fluid-attenuated inversion recovery (FLAIR) sequences, represents the current standard imaging modality for target volume delineation of gliomas. However, conventional sequences have limited capability to discriminate treatment-related changes from viable tumors, owing to the low specificity of increased blood-brain barrier perm

Radiation treatment planning techniques for lymphoma of the stomach.

PURPOSE: Involved-field radiation therapy of the stomach is often used in the curative treatment of gastric lymphoma. Yet, the optimal technique to irradiate the stomach with minimal morbidity has not been well established. This study was designed to evaluate treatment planning alternatives for stomach irradiation, including intensity-modulated radiation therapy (IMRT), to determine which approach resulted in improved dose distribution and to identify patient-specific anatomic factors that might influence a treatment planning choice. METHODS AND MATERIALS: Fifteen patients with lymphoma of th

The Evolving Role of Novel Imaging Techniques for Radiotherapy Planning.

The ability to visualise cancer with imaging has been crucial to the evolution of modern radiotherapy (RT) planning and delivery. And as evolving RT technologies deliver increasingly precise treatment, the importance of accurate identification and delineation of disease assumes ever greater significance. However, innovation in imaging technology has matched that seen with RT delivery platforms, and novel imaging techniques are a focus of much research activity. How these imaging modalities may alter and improve the diagnosis and staging of cancer is an important question, but already well serv

Orchiopexy: planned 2-stage technique.

Planned unilateral 2-stage orchiopexy was undertaken in 14 of 919 primary inguinal explorations for an undescended or nonpalpable testis. In 9 of the 14 cases a silicone sheath was placed around the spermatic cord to protect the cord structures from injury at the time of the second stage. An adequate-sized scrotal testis resulted in 83 per cent of these planned 2-stage orchiopexies. Our impression is that the silicone sheath greatly facilitates the second stage.

Analysis of computer-aided techniques for virtual planning in nasoalveolar moulding.

We compared two methods of planning virtual alveolar moulding as the first step in nasoalveolar moulding to provide the basis for an automated process to fabricate nasoalveolar moulding appliances by using computer-assisted design and computer-aided manufacturing (CAD/CAM). First, the initial intraoral casts taken from seven newborn babies with complete unilateral cleft lip and palate were digitised. This was repeated for the target models after conventional nasoalveolar moulding had been completed. The initial digital model for each patient was then virtually modified by two different modelli

Towards practical radiopharmaceutical treatment planning: a review of dosimetry simplification techniques.

Radiopharmaceutical therapy (RPT) plays an important role in modern precision oncology. However, treatment activity is still prescribed by fixed-dose protocols rather than adjusted to planned patient-specific absorbed dose (AD). Individualized dosimetry holds promise for improving treatment planning but is not yet standard. One main reason is that present dosimetry workflows depend on repeated imaging at multiple time points and require kinetic modeling and computational modeling. This review covers recent approaches to make dosimetry more practical and easier to use in everyday RPT workflows.

Description of a novel technique for ultrasound-based planning for gynaecological 3D brachytherapy and comparison between plans of this technique and 2D with fluoroscopy.

With the purpose of implementing a way to obtain volumes from ultrasound axial images, a novel method for 3D gynaecologic brachytherapy was assessed, with a 3D-printed attachment for a commercial stepper (for prostate brachytherapy). It allowed the acquisition of a transabdominal axial image series by ultrasound; these images were uploaded to a treatment planning system where high-risk clinical tumour volume (HR-CTV) and risk organs were contoured. A dose administration plan was developed with orthogonal X-ray images (0° and 270° incidences), using International Commission on Radiation Units a

Advanced Preoperative Planning Techniques in the Management of Complex Proximal Humerus Fractures.

This review evaluates the current literature on the recent advances of preoperative planning in the management of complex proximal humerus fractures (PHF). PHFs can pose a considerable challenge for orthopaedic surgeons due to their diversity in presentation and complexity. Poor preoperative planning can lead to prolonged operations, increased blood loss, higher risk of complications, and increased stress on the surgical team. Recent advances have seen the evolution of preoperative planning from conventional methods to computer-assisted virtual surgical technology (CAVST) and three-dimensional

Principles and practice of high-dose rate penile brachytherapy: Planning and delivery techniques.

PURPOSE: To allow for organ preservation, high-dose rate (HDR) brachytherapy may be used in the treatment of localized penile cancer. Penile cancer is a rare malignancy that accounts for <1% of cancers in men in the United States. The standard treatment for localized disease is partial amputation of the penis. However, patients with T1b or T2 disease <4 cm in maximum dimension and confined to the glans penis may be treated with brachytherapy as an organ-sparing approach. Previous works have described the technique involved for low-dose rate brachytherapy; however, we detail the techniques invo

Forward treatment planning techniques to reduce the normalization effect in Gamma Knife radiosurgery.

In Gamma Knife forward treatment planning, normalization effect may be observed when multiple shots are used for treating large lesions. This effect can reduce the proportion of coverage of high-value isodose lines within targets. The aim of this study was to evaluate the performance of forward treatment planning techniques using the Leksell Gamma Knife for the normalization effect reduction. We adjusted the shot positions and weightings to optimize the dose distribution and reduce the overlap of high-value isodose lines from each shot, thereby mitigating the normalization effect during treatm

Surgical planning after sleep versus awake techniques in patients with obstructive sleep apnea.

OBJECTIVE/HYPOTHESIS: This study examined correlations between surgical recommendations based on either drug-induced sleep endoscopy (DISE) or common awake examination methods in patients with obstructive sleep apnea syndrome (OSAS). STUDY DESIGN: Prospective, blinded, clinical trial at a university hospital. METHODS: An otorhinolaryngologist designed surgical plans for patients with OSAS after clinical examination, lateral cephalometry, the Müller maneuver, and Friedman staging. A second otorhinolaryngologist blinded to the previous plans made surgical recommendations after DISE. A third pe

Extended trochanteric osteotomy: planning, surgical technique, and pitfalls.

The extended trochanteric osteotomy is appropriate for a number of surgical indications. It facilitates removal of well-fixed cement mantles with a loose or well-fixed stem and of extensively porous-coated or tapered cementless stems. This exposure is particularly valuable in the presence of varus remodeling of the proximal femur, permitting correction of proximal femoral deformity and reducing the risk of fracture of the greater trochanter. It is also indicated in the removal of a well-fixed cemented stem that is complicated by infection, where it is vital to extract all foreign material for

A computer-aided treatment planning technique for universal wedges.

Some modern accelerators provide only one wedge built into the head of the treatment machine. A continuous range of wedge angles can be generated using wedged and unwedged beams in different proportions. However, this may pose difficulty in treatment planning since the planners will have to work with twice as many beams to achieve a desirable dose distribution. We have developed a simple algorithm to solve the treatment planning problem involving wedged and open beam combinations. The algorithm is based on the assumption that the user has some knowledge of the dose distribution to be optimized
