MR Imaging of Pleural Neoplasms.

The pleura may be affected by primary tumors or metastatic spread of intrathoracic or extrathoracic neoplasms. Primary pleural neoplasms represent ∼10% of all pleural tumors, and malignant lesions are more common than benign lesions. The most common primary tumors include malignant pleural mesothelioma and solitary fibrous tumor. Although pleural neoplasms may initially be evaluated with computed tomography (CT) and/or fluorodeoxyglucose positron emission tomography (PET)/CT, magnetic resonance (MR) imaging is complementary to these other imaging modalities for disease staging and evaluation o

Pleural Neoplasms-What Could MRI Change?

The primary pleural neoplasms constitute around 10% of the pleural tumors. The currently recommended method for their imaging is CT which has been shown to have certain limitations. Strong development of the MRI within the last two decades has provided us with a number of sequences that could potentially be superior to CT when it comes to the pleural malignancies' detection and characterization. This literature review discusses the possible applications of the MRI as a diagnostic tool in patients with pleural neoplasms. Although selected MRI techniques have been shown to have a number of advan

Diffuse neoplasms of the pleural serosa.

This study describes the diffuse neoplastic conditions that may affect pleural membranes. These include mesothelioma, the most important and common malignancy of pleural origin, and metastatic involvement by carcinomas, lymphomas and thymomas. On the basis of diagnostic imaging, we identify the distinctive features of pleural involvement by each of these conditions and provide elements enabling accurate differential diagnosis. Finally, we discuss the best diagnostic approach in the case of suspected primary or secondary neoplastic involvement of pleural membranes.

Histomorphological and Immunohistochemical Analysis of Pleural Neoplasms.

BACKGROUND & OBJECTIVE: Primary pleural neoplasms are rare entities compared with the pleural involvement by metastatic carcinoma. The current study aimed at investigating the complete spectrum of pleural neoplasms and differentiating between them with the aid of immunohistochemistry (IHC). METHODS: Consecutive pleural biopsy specimens positive for a neoplasm, both primary and metastatic, were included in the study. Diagnosis or a differential diagnosis was suggested on histopathology confirmed by a panel of IHC markers such ascytokeratin(AE1/AE3), epithelial membrane antigen (EMA),vimentin, c

[Primary malignant pleural neoplasms. Diagnostic code 163.09].

The code 163.09 indicates a primary malignant pleural neoplasma-malignant mesothelioma. We have reinvestigated 173 patients, who were discharged from a department of pulmonary medicine with this code number during a 8 1/2 year period. A revision during which repeated biopsies, course of the disease and, in some cases, autopsy were considered, revealed that 63 were confirmed as having a malignant mesothelioma, 94 had neoplastic disease secondary to malignancy elsewhere, 13 had benign changes and in three cases the records were missing. Malignant mesothelioma entitles the patient to compensation

Application of Immunohistochemistry in the Diagnosis of Pulmonary and Pleural Neoplasms.

CONTEXT: - A vast majority of neoplasms arising from lung or pleura are initially diagnosed based on the histologic evaluation of small transbronchial, endobronchial, or needle core biopsies. Although most diagnoses can be determined by morphology alone, immunohistochemistry can be a valuable diagnostic tool in the workup of problematic cases. OBJECTIVE: - To provide a practical approach in the interpretation and immunohistochemical selection of lung/pleura-based neoplasms obtained from small biopsy samples. DATA SOURCES: - A literature review of previously published articles and the persona

Immunohistochemistry for assessment of pulmonary and pleural neoplasms: a review and update.

Diagnosis and classification of lung and pleural neoplasms are complex due to diverse histopathology and tumor heterogeneity. A large number of immunohistochemical markers have recently become available to facilitate accurate diagnosis and classification of pulmonary and pleural neoplasms. The purpose of this article is to review the current data available on these markers and to provide a practical approach to evaluate pulmonary and pleural neoplasms immunohistochemically. Current literature of immunohistochemical markers related to pulmonary and pleural neoplasms was collected and reviewed.

Pulmonary and pleural involvements at initial diagnosis in children with malignant neoplasm.

Seventeen cases of pediatric malignant neoplasm with pulmonary and/or pleural lesions shown by chest radiography at initial diagnosis were reviewed and analyzed. Respiratory symptoms such as dyspnea, tachypnea, and chest pain were observed on admission in approximately one-half of them. The initial chest radiography showed pleural lesions in 7 of the 17, pulmonary lesions in 8, and both pulmonary and pleural lesions in 2. Unilateral or bilateral pleural fluids were observed in all of the seven patients with pleural lesions, and malignant cells were confirmed in the pleural fluid of all patient

Unusual Cause of Diffuse Pleural Thickening (Malignant Pleural Mesothelioma Mimic): Undifferentiated Epithelioid Malignant Neoplasm.

The symptoms of malignant pleural mesothelioma can be mimicked by some other malignancies, clinically and radiologically. We describe a case of a 57-year-old man whose clinical and radiological findings were consistent with malignant pleural mesothelioma; however, a pleural biopsy showed undifferentiated epithelioid malignant neoplasm.

Pleural desmoid tumor mimicking a myofibroblastic neoplasm: Imaging-pathologic correlation.

Desmoid tumors are rare, locally aggressive soft-tissue neoplasms that may present a diagnostic challenge due to overlapping imaging and histopathologic features with other myofibroblastic lesions. We report the case of a 41-year-old woman presenting with a 2-year history of right-sided chest pain and progressive numbness of the right upper extremity. Contrast-enhanced computed tomography (CT) demonstrated a large pleural-based mass with chest wall extension. Initial biopsy suggested an inflammatory myofibroblastic tumor; however, further characterization with magnetic resonance imaging (MRI)

Reduction of lung parenchyma compliance as a factor in the genesis of pleural effusion in neoplasms.

It is well known that bronchial obstruction causes an increase in negative pressure in the pleural cavity and is considered to be one of the factors giving rise to pleural fluid. We suggest that a reduction in the compliance of the lung parenchyma, whatever its cause (apart from bronchial obstruction), may also lead to an increase in negative pressure in the pleural cavity and may contribute, together with the other known factors, as an additional mechanism in the appearance of pleural fluid. We believe that this factor is valid in neoplastic pleurisy and may also apply in pleurisy, and possib

Myopericytoma: a pleural-based spindle cell neoplasm off the beaten path.

Myopericytoma is a recently described hemangiopericytoma-like neoplasm with myoid differentiation. These tumors are typically located in the subcutaneous and soft tissues of the extremities. The authors report a rare pleural-based pulmonary myopericytoma in a 58-year-old woman. The lesion was grossly homogeneous and well circumscribed. Microscopically, it was composed of densely packed spindle cells organized as whorls and short interlacing fascicles with a concentric perivascular distribution. Immunohistochemical reactions were positive for vimentin, smooth muscle actin (SMA), muscle-specific

A Massive Haemothorax Due to Pleural Extramedullary Haematopoiesis in a Patient With Myelodysplastic Syndrome/Myeloproliferative Neoplasm.

Extramedullary haematopoiesis (EMH) refers to blood cell production outside the bone marrow, typically in reticuloendothelial organs such as spleen, liver, and lymph nodes, as a compensatory response to haematopoietic stress. We describe a rare case of recurrent haemothorax caused by pleural EMH in a female patient in her 60s with myelodysplastic/myeloproliferative neoplasm (MDS/MPN) not otherwise specified (NOS). She had recurrent presentations with transfusion-dependent anaemia, fevers, splenomegaly, and bone marrow morphology consistent with MDS/MPN. Over the course of her disease, she deve

Update on gene fusions and the emerging clinicopathological landscape of peritoneal and pleural mesotheliomas and other neoplasms.

BACKGROUND: Mesothelioma is a rare and aggressive malignant neoplasm arising from mesothelial cells, which occasionally manifests recurrent fusions. EWSR1/FUS-CREB, YY1, MAP3K8, NR4A3, and ALK-rearranged proliferations have been reported in limited series with no clear histological or clinical correlations, limiting clinicians' ability to assess prognosis and integrate these new entities into therapeutic decisions. The aim of this study was to better characterize these rearranged proliferations histologically, molecularly, and clinically. METHODS: Clinical, pathological, and comprehensive tra

Malignant pleural neoplasm with both differentiation of epithelioid mesothelioma and squamous-cell carcinoma, a rare phenomena.

Malignant mesothelioma, a neoplasm arising within the serosal surfaces, has been linked closely to asbestos exposure. We present a case of 72-year-old male with a 27 year work-related history of asbestos exposure who presented with dyspnea. Chest computed tomography scan showed a large, right pleural effusion with compressive right lung atelectasis. Biopsies, subsequent pleurectomy and lung wedge resections revealed epithelioid malignant mesothelioma with associated focal non-keratinizing squamous-cell carcinoma, supported by extensive immunohistochemical stains and molecular studies. The pati

Metastatic salivary gland neoplasms to pleural effusion: diagnostic challenges and prognostic significance in a series with 9 patients.

INTRODUCTION: Pleural effusions can present a diagnostic challenge as they are not always caused by malignancy in patients with a history of typical visceral primaries. MATERIAL AND METHODS: At 2 major academic medical centers, we have identified several cases in which salivary gland neoplasms metastasized to pleural effusions in patients who have been aggressively managed with various treatment modalities including chemotherapy, radiation, and/or surgical excision. RESULTS: Herein, we present a range of primary salivary gland tumors that metastasized to serous effusions and characterize the
