Pneumomediastinum: a diagnostic problem.

Pneumomediastinum has numerous etiologies and its pathways of spread are multiple and well defined. Knowledge of these anatomic pathways and possible etiologies is important in order to avoid extensive and unnecessary evaluations. For example, if there is a known reason for pneumomediastinum, and pneumoperitoneum is present without associated abdominal findings, further evaluation for perforated viscus is unnecessary. In a patient who is an asthmatic or a diabetic, the presence of pneumomediastinum should not lead to work-up with contrast studies unless there are specific clinical reasons to d

A diagnostic challenge of an unusual presentation of pneumomediastinum.

A 77-year-old man who had underwent orthopedic surgery 17 days ago due to his left femur fracture caused by a pedestrian-car accident came to our emergency department with the chief complaint of a 2 days history of sore throat and cough and also swelling of eyelids. He had no respiratory distress or any other life-threatening symptoms. Subsequent physical examination revealed remarkable edema and crepitus over the whole face, neck, proximal upper limbs and the anterior and posterior chest regions, and also bilateral hyperresonance was detected in pulmonary auscultation. The imaging studies sho

Pneumoretroperitoneum, Pneumomediastinum, Pneumothorax, and Subcutaneous Emphysema after Diagnostic Colonoscopy.

Colonoscopy is a commonly performed endoscopic procedure. Although it is generally considered to be safe, serious complications, such as colorectal perforation, can occur. Most colonic perforations are intraperitoneal and cause pneumoperitoneum with acute abdominal pain as the initial symptom. However, extraperitoneal perforations with pneumoretroperitoneum may happen, albeit rarely, with atypical initial symptoms. We report a rare case of rectosigmoid perforation occurring after diagnostic colonoscopy that developed into pneumoretroperitoneum, pneumomediastinum, pneumothorax, and subcutaneous

Elevated troponin in a patient with pneumomediastinum and pneumothorax: a diagnostic challenge.

We present the case of a middle-aged man who was admitted with acute chest pain and markedly elevated troponin levels, initially raising concerns for acute coronary syndrome. Diagnostic evaluations revealed an apical pneumothorax, and pneumomediastinum, without evidence of dynamic ischaemic ECG changes. Despite the concerning elevation in troponin levels, minimal variation in serial measurements (<10%) indicated a non-ischaemic aetiology. The troponin elevation was attributed to stress from pneumomediastinum, localised inflammation and myocardial strain. This case highlights the importance of

[A harmless form of pneumomediastinum: avoid unnecessary diagnostics and treatment].

BACKGROUND: Pneumomediastinum is a potentially life-threatening condition. Patients can present with a variety of symptoms at the emergency department. Pneumomediastinum can be spontaneous or secondary due to perforation of the esophagus or tracheobronchial tree. CASE DESCRIPTION: A 20-year old man was seen at the emergency department with the suspicion of esophageal perforation. He noticed subcutaneous 'crackles' arising after an episode of severe vomiting. In the emergency room a non-acute ill patient was seen with subcutaneous emphysema of the neck and chest. A CT-scan with oral contrast d

Spontaneous Pneumomediastinum: A Diagnostic Challenge in a Patient Presenting With Excessive Vomiting and Neck Swelling.

Hamman's syndrome, or spontaneous pneumomediastinum, refers to free air in the mediastinum without an apparent cause and typically affects young people. This case report describes a 33-year-old man who presented with neck swelling following excessive vomiting due to alcohol consumption. Clinical examination revealed subcutaneous emphysema of the neck, and imaging confirmed pneumomediastinum. Initial suspicion of Boerhaave syndrome led to aggressive supportive management, but further imaging ruled out oesophageal perforation, confirming Hamman's syndrome. The patient was treated conservatively

Predictive criteria for oesophageal perforation in patients with pneumomediastinum: can invasive diagnostic modalities be avoided?

BACKGROUND: The increased use of CT in emergency departments (ED) has led to a rise in incidental detection of pneumomediastinum (PNM). As this finding can be associated with oesophageal perforation (OP), clinicians face a diagnostic dilemma on whether further invasive investigation (such as endoscopy) is required. This study aimed to identify clinical, biochemical and radiological predictors of OP in patients with PNM, to develop a non-invasive diagnostic approach. METHODS: A retrospective analysis of adult patients with CT-confirmed PNM was conducted from January 2016 to December 2022 at a

Diagnostic Utility of Computed Tomography in Patients With Spontaneous Pneumomediastinum Evaluated for Suspected Esophageal Perforation.

BACKGROUND: Spontaneous pneumomediastinum (PM) is an uncommon radiographic finding that may indicate esophageal perforation and lead to further evaluation including transfer to a tertiary care center. We evaluated computed tomography (CT) findings and associated clinical features in patients with a suspected spontaneous esophageal perforation. METHODS: We retrospectively reviewed records of adults referred to a tertiary care center (January 2020-December 2024) for suspected spontaneous esophageal perforation with PM on CT. Patients with a history of recent surgery, endoscopy, or trauma were e

Pneumomediastinum Masquerading As Superior Vena Cava Obstruction in a Patient With Metastatic Colorectal Cancer: A Diagnostic Challenge.

Pneumomediastinum refers to the presence of free air within the mediastinum and is most commonly associated with trauma, instrumentation, or barotrauma, but may rarely occur spontaneously. In patients with thoracic malignancy, pneumomediastinum can mimic other oncologic emergencies such as superior vena cava obstruction (SVCO), creating diagnostic uncertainty. We present the case of a 57-year-old woman with metastatic recurrent colorectal carcinoma receiving palliative FOLFIRI (folinic acid, fluorouracil, and irinotecan) and panitumumab who was admitted with recurrent respiratory infections. D

Superior mediastinal shift in right-lower-lobe collapse: the "upper triangle sign".

In some patients with right-lower-lobe collapse a triangular shadow was seen in the right upper lung field, continuous with the mediastinum and with its apex pointing toward the right hilus. This represents a shift of the upper anterior mediastinum to the right and may be mistaken for right-upper-lobe collapse or an infiltrate by the unwary observer. This finding, which the authors call the "upper triangle sign," occurs frequently enough in cases of significant right-lower-lobe collapse to be a useful diagnostic sign.

A clinical review of spontaneous pneumomediastinum.

Primary spontaneous pneumomediastinum is a rare, often benign and self-limited condition defined by air within the mediastinum. However, correctly distinguishing primary spontaneous pneumomediastinum from secondary causes, especially esophageal perforation, remains a diagnostic challenge. There is significant debate regarding the balance of completing a thorough but not overly invasive and costly diagnostic workup. This clinical review aims to gather the limited data regarding spontaneous pneumomediastinum management from case series and retrospective cohort studies, and presents an evaluation

Management and Outcomes of Spontaneous Pneumomediastinum in Children.

OBJECTIVES: Management of spontaneous pneumomediastinum in the pediatric population is highly variable. There are limited data on the use of diagnostic tests and the need for admission. Our objectives were to characterize the management of pediatric spontaneous pneumomediastinum, determine the diagnostic yield of advanced imaging, and describe the patients' outcomes. METHODS: This is a retrospective cohort study of all patients presenting to a single tertiary pediatric emergency department between January 2008 and February 2015 diagnosed with pneumomediastinum. Patients were identified using

[Pneumomediastinum: intervention is not always necessary].

Pneumomediastinum is characterized by the presence of free air in the mediastinum. This can be a sign of a severe underlying disorder that requires urgent intervention. However, pneumomediastinum is also associated with other causes that can be treated conservatively. Here, we describe three patients who presented with a primary or spontaneous pneumomediastinum and a secondary pneumomediastinum due to traumatic or iatrogenic perforation. Recognition of the underlying cause of pneumomediastinum is essential for further diagnostic studies and patient management. Unnecessary investigations and in

BACKGROUND: Pneumomediastinum is an uncommon cause of neonatal respiratory distress. Clinical history and examination of the neonate may be uninformative in determining the aetiology of the respiratory distress. Chest x-ray can be diagnostic of pneumomediastinum however is often difficult to interpret. CASE PRESENTATION: A 36 week gestation newborn was delivered by emergency caesarean section, intubated and given intermittent positive pressure ventilation via Neopuff™ for apnoea before being extubated to CPAP. Chest radiography initially showed bilateral upper zone opacities, presenting a dia
