Necrotizing aspiration pneumonia.

Infectious disease is the most common etiology of a cavitary lung process, which can occur with necrotizing pneumonia, lung abscess and septic pulmonary embolism. Other infectious causes of pulmonary cavitation include cavitating fungal or mycobacterial granulomas and superinfection of preexisting spaces. Noninfectious causes of cavitation that should be considered in the differential diagnosis are infarction from thromboembolic disease or vasculitis, necrotic primary or metastatic tumor, rheumatoid nodules and air-filled congenital cysts. The clinical history combined with radiographic featur

[Necrotizing pneumonia due to Streptococcus pneumonia].

We present a patient with a necrotizing pneumonia due to Streptococcus pneumoniae. This complication often affects children and is relatively rare and unrecognized in adults. Its diagnosis is often difficult on chest-X-ray and need a chest computed tomography. No risk factors predisposing to necrosis are described in literature and its mortality is not different from pneumonia without necrosis. The serotype 3 is the most common type implicated in pneumococcal necrotizing pneumonia. Study of virulence factors of S. pneumoniae and various genetic polymorphisms of the host should allow a better u

[Necrotizing pneumonia - a rare complication].

Necrotizing pneumonia is a rare complication of community-acquired pneumonia associated with destruction of the lung tissue during the infection and rise of necrotic foci in consolidated areas. Staphylococcus aureus, Streptococcus pyogenes, Nocardia, Klebsiella pneumoniae and Streptococcus pneumoniae are the most common causative agents. Risk factors for developing necrotizing pneumonia include smoking, alcoholism, old age, diabetes mellitus, chronic lung diseases or liver disease. Diagnosis of necrotizing pneumonia requires supportive care, use of broad-spectrum antibiotics and monitoring for

Differences of clinical features and prognosis between Mycoplasma pneumoniae necrotizing pneumonia and non-Mycoplasma pneumoniae necrotizing pneumonia in children.

BACKGROUND: In the past few years, Mycoplasma pneumoniae (Shi et al. Lancet 390:946-958, 2017) infection has been reported more in China. However, there are few studies on the clinical characteristics and prognosis of necrotizing pneumonia (NP) (Griffiths et al. Nature 583:615-619, 2020) caused by different pathogens. METHODS: A retrospective analysis was performed, including 31 children with a clinical diagnosis of NP in the hospital from January 1, 2013 to January 31, 2020. A total of 11 children with MPNP were included in the observation group and the other 20 children with other pathogens

[Necrotizing community-acquired Staphylococcus aureus pneumonia].

Acute necrotizing pneumonia due to Panton-Valentine secreting Staphylococcus aureus was identified as a clinical entity by Gilet et al., in 2002. This severe acute necrotizing pneumonia occurring in previously healthy children and adolescents can lead to a rapid fatal outcome even if quickly diagnosed and treated. We report the case of a healthy 10-year-old girl presenting with hemorrhagic necrotizing pneumonia and septic shock. Bacteriological cultures yielded methicillin-susceptible Staphylococcus aureus. The course of the disease was characterized by recurrent uncontrolled hemoptysia leadin

Massive necrotizing pneumonia with pulmonary gangrene.

Pulmonary gangrene is an extremely rare and severe complication of pneumonia. It is very rarely seen in community-acquired pneumonia. A 49-year-old immunocompetent man was admitted with community-acquired pneumonia caused by Klebsiella pneumonia. His condition rapidly deteriorated with ensuing pulmonary gangrene and septic shock. He was successfully managed by emergency pneumonectomy.

Necrotizing staphylococcal pneumonia in a neonate.

Hospitalized neonates are commonly colonized soon after birth with Staphylococcus aureus. The majority of neonates do not develop infectious sequelae; however, premature neonates appear to be more susceptible to serious infections, such as pneumonia. We report a case of an extremely low birth weight infant who developed necrotizing pneumonia due to methicillin-resistant Staphylococcal aureus (MRSA). The MRSA isolate from this neonate is identical to the strains that have been causing primarily community-associated skin and soft tissue infections. The severe course of this patient may be attrib

Necrotizing pneumonia caused by Penicillium chrysogenum.

We report a case of necrotizing pneumonia due to Penicillium chrysogenum in a 57-year-old woman operated on for lung cancer. The residual right lower pulmonary lobe was infiltrated by Penicillium chrysogenum. The patient underwent a second pulmonary right lobectomy and was successfully treated with oral itraconazole. To our knowledge, this is the first case of pneumonia due to P. chrysogenum.

Bacteremic necrotizing pneumococcal pneumonia in children.

Necrotizing pneumonia, massive necrosis of lung tissue, is a serious, often fatal, complication of lobar pneumonia. Four children 1.3 to 7.5 yr of age were hospitalized with bacteremic pneumococcal pneumonia. All of them were acutely ill on presentation with arterial desaturation, and they developed anemia and thrombocytosis. Two patients had pleural effusion requiring drainage. A chest CT scan revealed segmental or lobar pulmonary liquification, which led to the diagnosis of necrotizing pneumonia. This finding could be demonstrated early in the course of the disease. Subsequently, all of the

Necrotizing Pneumonia In Children: A Review.

The objective of the review was to determine the long-term outcomes of necrotising pneumonia (NP). Studies published since 1990 in English, Portuguese, or Spanish, published on PubMed and Scielo were evaluated. Our findings showed ultrasound scanning is the diagnostic modality of choice. Despite prolonged hospitalisation (median 13-27 days) and fever (median 9-16 days), most patients recover completely. Empyema and bronchopleural fistulae are frequent in bacterial NP. Streptococcus pneumoniae is the most prevalent cause. Seventeen studies with 497 patients followed for 30 days to 8.75 years sh

Necrotizing Pneumonia With Extensive Lobar Cavitation.

Pneumonia occupies one of the leading positions in morbidity and mortality worldwide. It is frequently categorized depending on the site of acquisition. Here, we present a case of a young woman who was admitted to the Emergency Department (ED) with cough, dyspnea, fever, and progressive worsening associated with palpitations and hypotension. An initial x-ray was followed by a computed tomography (CT) scan of the chest, which revealed signs of extensive left lung pneumonia with pleural effusion. Despite initial improvement after antibiotic treatment, the patient's condition declined. Epithelioid Hemangioendothelioma Presenting as Necrotizing Pneumonia.

Epithelioid hemangioendothelioma (EHE) is a very rare vascular neoplasm that is often asymptomatic. A 40-year-old woman presented to the emergency department for evaluation of a nonproductive cough, chest pain, and dyspnea. A chest computed tomography angiography (CTA) demonstrated necrotizing pneumonia, a loculated left-sided pleural effusion, and an acute pulmonary embolus. She was started on broad-spectrum intravenous antibiotics and heparin infusions, and a chest tube was placed. After minimal improvement in her pleural effusion following instillation of fibrinolytics, she underwent video-

A Comprehensive Review of Pediatric Necrotizing Pneumonia.

Necrotizing pneumonia is a serious complication of pediatric pneumonia, characterized by liquefaction and cavitation of the lung parenchyma. Streptococcus pneumoniae and Staphylococcus aureus are the most implicated organisms. Mycoplasma pneumoniae has been an increasingly recognized pathogen, especially is Asian and Pseudomonas aeruginosa is mainly noted in a higher percentage of patients with complex chronic conditions. Clinical presentation typically includes fever, respiratory distress, and failure to respond to standard antibiotic therapy. These patients are more likely to have pleural in

Necrotizing pneumonia (aetiology, clinical features and management).

PURPOSE OF REVIEW: Necrotizing pneumonia is a severe form of community-acquired pneumonia characterized by rapid progression of consolidation to necrosis and cavitation which may lead to pulmonary gangrene. Morbidity and mortality are high and chronic sequelae are frequent. The lack of guidance supports the review of the latest recommendations in the management of these pneumonias. RECENT FINDINGS: Antibiotic therapy alone may not be enough to alter the course of the infection, and regimens, adjunctive therapies like intravenous immunoglobulins, surgery may be required to alter the course of

[Early predictors of necrotizing pneumonia in children].

OBJECTIVE: To investigate the early predictors of necrotizing pneumonia in children. METHODS: The clinical data of 43 children with necrotizing pneumonia and 83 children with lobar pneumonia were retrospectively analyzed. Sex, age, the number of days with fever, laboratory examination results, and bronchoscopic findings were compared between the two groups. The multiple logistic regression analysis was used to identify the early predictors of necrotizing pneumonia. RESULTS: The necrotizing pneumonia group had a higher percentage of girls than the lobar pneumonia group (P<0.05). Compared with
