The Medial Pontine Mid-Tegmentum Syndrome.

Isolated pontine infarcts are common and are often associated with well-described syndromes that are classified based on their specific clinical presentation and arterial stroke territories. Here we present a case of acute combined diplopia and unilateral lower extremity sensory abnormality. Diffusion-weighted magnetic resonance imaging revealed a punctate area of acute ischemia in the right medial pontine mid-tegmentum. These findings suggest a unique pontine stroke syndrome characterized by acute ischemic injury at the intersection of the medial lemniscus and cranial nerve VI.

[Transient severe orthostatic hypotension and hematoma of the pontine tegmentum].

A 57-year old patient presented with a posterior pontine hematoma and severe postural hypotension. The lesions were localized in the pontine tegmentum. They were bilateral and involved the area of the nucleus reticularis pontis caudalis, left medial lemniscus and roots of several cranial nerves, especially the VIth. The patient had lasting postural hypotension, with reduction of the physiological tachycardia. Investigation of heart rate regulation during manoeuvres however revealed that the accelerating reaction partially persisted, especially with Valsalva manoeuvre. Heart rate recording for

Afferent projections related to attack sites in the pontine tegmentum.

Attack by a cat on a rat was elicited by electrical stimulation of the ventral parabrachial nucleus or the paralemniscal pontine reticular formation. The attack was quiet and it differed from similar behavior elicited at other sites in that the cat not only bit the rat but struck it with its paw. Horseradish peroxidase was deposited at the attack sites. Cells sending projections to the attack sites were located in gyrus proreus, anterior, central, and medial amygdala, the bed nucleus of the stria terminalis, lateral, dorsal and perifornical hypothalamus, as well as the central gray matter, sup

Pontine tegmentum hematoma. Report of a case with pure hemiplegia.

The authors report the case of a 50 year-old hypertensive male patient with a pontine hematoma. The clinical presentation was characterized by pure pyramidal deficit signs (no other signs or symptoms were present). A pure hemiplegia syndrome, although common in supratentorial lesions, is considered to be a rare event in pontine vascular lesions. The pathophysiologic mechanisms of these neurological findings are unclear. The exclusive involvement of the pyramidal tract in this case is likely due to a variation in the vascular anatomy of the pons but, in some cases, a vascular malformation may b

[Effects of lesions in the pontine tegmentum on the sleep stages in the rat].

1. Limited coagulations of the locus coeruleus and subcoeruleus nuclei have been performed in rats and the sleep-waking cycle was continuously monitored during 9 days. The cortical and diencephalic noradrenaline content was mesured at the termination of the experiment, on the 10th post lesion day. 2. The bilateral destruction of the locus coeruleus is followed by the appearance of a uro-genital syndrome consisting of hema turia, bladder distension and penile erection. The states of sleep are disturbed during the first two days (increase of slow-wave sleep and decrease of paradoxical sleep time

Ocular flutter associated with a lesion of the right upper pontine tegmentum.

Ocular flutter is a relatively rare eye movement disorder characterized by intermittent bursts of conjugate horizontal saccades without an intersaccadic interval. Its anatomical and neurophysiological basis remains controversial. Here we present a case of ocular flutter in which the flutter declined as the clinical stage of the associated ailment declined, until it eventually disappeared. Radiological examination revealed a tiny lesion in the right upper pontine tegmentum that included areas of the reticular formation, superior cerebellar peduncle and locus ceruleus. Because the lesion occupie

[A case of infarction in the pontine tegmentum involving the bilateral MLF and unilateral paramedian pontine reticular formation].

A 61-year-old man with lesions of the bilateral medial longitudinal fasciculus (MLF) and left paramedian pontine reticular formation (PPRF) caused by an infarction in the pontine tegmentum was reported. On admission, the ocular position of the left eye on forward gaze was fixed at the midline, while the right eye was abducted, and skew deviation was observed. On leftward gaze, neither eye could pass the midline, and on rightward gaze the right eye was abducted with monocular nystagmus, indicating paralytic pontine exotropia. On the fifth day, the bilateral eyes were abducted on forward gaze. O

[Myorhythmia emerging on day 15 in a case of an infarction in the midbrain-pontine tegmentum].

A 72-year-old woman was admitted to our hospital because of dysarthria, dysstasia, and diplopia. Examination demonstrated wall-eyed bilateral internuclear ophthalmoplegia (WEBINO) syndrome, dysarthria, truncal ataxia, and dysmetria of the four limbs. Cranial MR images demonstrated an acute infarct in the paramedian, lower midbrain-pontine tegmentum. Peduncular hallucinosis was observed during the first several days of hospitalization. Myorhythmia (skeletal myoclonus) appeared as early as day 15 and has persisted to date, with a frequency of approximately 1.5 Hz on the head and chin, and 3.5 Hz

Opioid action on spinal cord reflexes due to dorsolateral pontine tegmentum stimulation.

Electrical stimulation of the dorsolateral pontine tegmentum (DLPT) produces phasic facilitatory and inhibitory actions on the lumbar spinal monosynaptic reflexes (MSRs) of both flexor and extensor muscle nerves in the decerebrate cat. Naloxone, an opioid receptor antagonist, given intravenously or intraspinally enhanced the DLPT-induced potentiation of MSRs in most of the reflexes studied. However, systemic naloxone had no significant effect on the unconditioned MSR of the spinal cord. Intraspinal microinjections of naloxone significantly attenuated the DLPT-induced inhibition of MSRs of both

Repetitive involuntary leg movements in patients with brainstem lesions involving the pontine tegmentum: evidence for a pontine inhibitory region in humans.

Repetitive involuntary limbs movements have been mostly reported in patients with extensive brainstem pathologies, but the region responsible is unknown. We describe two patients with progressive basilar artery infarcts who showed automatic stepping and one patient with an osmotic demyelination disorder who showed periodic involuntary leg movements. By subtracting diffusion-weighted images before and after the development of repetitive involuntary leg movements, the brainstem lesion responsible for the involuntary movements was distinctively located in the vicinity of the pontine tegmentum, wh

[Isolated trigeminal neuropathy involved with motor dysfunction due to small infarction in the pontine tegmentum].

We present a case of isolated trigeminal neuropathy involved with motor dysfunction due to a small infarction in the pontine tegmentum. A 70-year-old man presented with sudden numbness around the left mouth accompanied by maxillary pain. Neurological examinations revealed jaw deviation to the left when opening the mouth, but no other abnormal findings including those of the cranial nerves were detected. X-rays indicated hypomobility of the left temporomandibular joint, and diffusion-weighted magnetic resonance imaging (MRI) revealed a small infarction in the left pontine tegmentum. Unilateral

Bilateral horizontal gaze palsy with unilateral peripheral facial paralysis caused by pontine tegmentum infarction.

Clinical features of pontine infarction depend on the topography of vascular lesion and most remarkably sometimes the same topographic region can lead to different clinical syndromes (e.g., dorsal pontine tegmentum). In this report we describe an elderly patient with acute dorsal pontine infarction leading to a unique syndrome of bilateral horizontal gaze palsy and unilateral peripheral facial paralysis. We propose that this syndrome could be included as a part of a continuum that involves one-and-a-half syndrome, eight-and-a-half syndrome, and other variants of pontine tegmentum infarction.

Bilateral hypogeusia and food aversion due to lacunar infarct in the right dorsomedial pontine tegmentum.

A 70-year-old right-handed housewife suffered an acute loss of taste, an unpleasant change in the taste of foods and liquids, and a strong aversion to all kinds of food due to a small lacune in the right dorsomedial pontine tegmentum. Eating became so unpleasant that she lost 7 kg in three weeks. Olfaction and the sensibility of the tongue were spared. The right medial longitudinal fascicle, the central tegmental tract, or both, were injured by the tegmental lesion. A discrete right-sided lesion in the upper pontine tegmentum may cause a reversible syndrome consisting of bilateral hypogeusia w

The one-and-a-half syndrome--a unilateral disorder of the pontine tegmentum: a study of 20 cases and review of the literature.

The one-and-a-half syndrome is a clinical disorder of extraocular movements characterized by a conjugate horizontal gaze palsy in one direction plus an internuclear ophthalmoplegia in the other. The syndrome is usually due to a single unilateral lesion of the paramedian pontine reticular formation or the abducens nucleus on one side (causing the conjugate gaze palsy), with interruption of internuclear fibers of the ipsilateral medial longitudinal fasciculus after it has crossed the midline from its site of origin in the contralateral abducens nucleus (causing failure of adduction of the ipsila
