Hepatobiliary scintigraphy after hepatic portoenterostomy.

Hepatobiliary scintigraphy with 99mTc-E-HIDA was carried out in 10 patients who had undergone hepatic portoenterostomy for "non-correctable" biliary atresia. In all the patients there was an active bile flow and disappearance of jaundice, post-operatively. Elimination of 99mTc-E-HIDA from the liver was slow during the first 3 post-operative months despite active bile flow and disappearance of jaundice. The elimination rate of the radioactive agent from the liver in most long-term survivors was comparable with findings in controls.

Portal vein thrombosis following hepatic portoenterostomy.

Life-threatening ascites developed in a 4-month-old infant with biliary atresia 1 month after hepatic portoenterostomy. Although initially ascribed to progressive liver failure, the ascites was subsequently found to be partially caused by portal vein thrombosis. Documented recanalization of the portal venous system was accompanied by resolution of the ascites. Refractory ascites in patients with biliary atresia generally signals end-stage cirrhosis and consideration of liver transplantation. Portal vein patency should be evaluated before referral of such patients for this procedure.

Biliary lipid excretion after hepatic portoenterostomy.

Since 1974, 16 consecutive infants with biliary atresia have been treated by hepatic portoenterostomy employing an exteriorized Roux-en-Y intestinal segment (Miluliez). Simultaneous, sequential analyses of bile pigments and lipids in serum and biliary drainage were performed. In the 11 patients with sustained bile drainage, progressive increases in bile volume, bilirubin and biliary lipid concentrations correlated well with their subsequent return toward normal in the serum. Despite relief of biliary obstruction, four patients have had progressive liver cirrhosis. The other 7 have residual liv

Kasai portoenterostomy--new insights from hepatic morphology.

BACKGROUND/PURPOSE: The aim of this paper was to investigate the mechanism of long-term biliary drainage after Kasai portoenterostomy by clinicopathologic study of hepatic morphology in explanted livers. METHODS: Explanted livers from 13 consecutive children undergoing transplantation for biliary atresia were examined in detail using a standardized protocol. Group 1 (n = 6) had no Kasai procedure before transplantation at a median age of 8 m. Group 2 (n = 4) were transplanted at a median age of 10 m after a failed Kasai portoenterostomy. Group 3 (n = 3) had a successful Kasai but required tra

Hepatic portoenterostomy (the Kasai operation) for biliary atresia.

Twenty-two infants and children with extrahepatic biliary atresia were treated by Kasai's hepatic portoenterostomy operation since 1972. Postoperative bile drainage occurred in 11 infants. Success never was obtained in patients older than 12 weeks nor in the absence of microscopic biliary ductules in the excised fibrotic remnant of the extrahepatic bile ducts. With these exclusions almost 90 percent of infants with biliary atresia had sustained bile drainage after operation. In the early cases, ascending cholangitis was a major and recurrent complication with sequellae in many instances of per

[Portal hypertension after successful hepatic portoenterostomy in biliary atresia].

Sixty percent of 139 patients with biliary atresia who underwent hepatic portoenterostomy (Kasai operation) from 1965 to 1994 became jaundice-free after the operation. Portal hypertension, which was confirmed by endoscopic observation of esophageal varices and/or thrombocytopenia (less than 100 x 10(3)/mm3), developed in 50% of unicteric long-term survivors. Esophageal varix was found initially between 11 months and 5 years of age in more than 70% of these portal hypertensive patients. On the contrary, thrombocytopenia developed at 6 years or older age in 63% of the patients. The incidence of

Morio Kasai Corrects the Uncorrectable: Hepatic Portoenterostomy for Biliary Atresia.

One of the fundamental innovations of pediatric surgery is hepatic portoenterostomy for biliary atresia, widely known as the Kasai procedure for its originator, Morio Kasai (1922-2008), of Sendai, Japan. It was the first effective operation for the "uncorrectable" form of biliary atresia, where death within months from biliary cirrhosis was certain. His radical premise was that microscopic ductules present in the fibrous tissue of the porta hepatis, when transected, drained enough bile to relieve cholestatic jaundice. His first operations from 1955 to 1961 had a jaundice-free survival rate of

Focal nodular hyperplasia in biliary atresia patient after Kasai hepatic portoenterostomy.

We present the case of a 10-year-old girl with biliary atresia in whom focal nodular hyperplasia (FNH) of the liver occurred following successful "Kasai" hepatic portoenterostomy at the age of 2 months. Her jaundice completely disappeared a few weeks after the operation. A 4-cm diameter liver tumor in segment IV was found when she was 5-years old. Needle biopsy could not establish a definitive diagnosis. Because the tumor size was not changing, she was conservatively followed by abdominal echo at an outpatient clinic. She showed progressive liver dysfunction and ongoing cirrhosis, and so at th

The future role of hepatic portoenterostomy as treatment of biliary atresia.

According to our recent study, 38 of 93 patients (40.9%) who underwent portoenterostomy at Juntendo University Hospital between 1977 and 1986 survived for more than 5 years. In order to learn the future improvement of the prognosis of biliary atresia patients who undergo portoenterostomy, we investigated the relationship between the prognosis of biliary atresia patients and the age and the histological changes of liver at the time of surgery. Twenty-seven of 30 patients (90%) who survived for more than 5 years with no jaundice and no findings of liver cirrhosis had a mild degree of liver fibro

A study on hepatic portoenterostomy for the treatment of atresia of the biliary tract.

The results of our investigation have shown the crucial points of hepatic portoenterostomy used by us which is different from Kasai's portoenterostomy. First, the dissection of the rudimentary extrahepatic bile duct should be performed under magnification. Second, the transection of the rudimentary extrahepatic bile duct should be done using the microsurgical technique. Third, histologic verification of patency of the rudimentary extrahepatic bile ducts with frozen section during the operation should be carried out and the transection should be repeated under microsurgical control until the pa

Icteric flare-up in patients with biliary atresia after hepatic portoenterostomy.

The relapse of icterus may be a sign of serious postoperative complication after hepatic portoenterostomy for biliary atresia. To analyse the cause of such icteric flare-up, a retrospective study was made on 27 patients. A total of 38 episodes of icteric flare-up occurred in 19 patients. An accompanying disease, which had not directly involved the biliary tract, was assumed to be the cause in 11 episodes (29%). Fourteen episodes (37%) were due to biliary tract infection caused by the cyst formed at the porta hepatis (5 episodes) or so-called ascending cholangitis (9 episodes). The other 13 epi

An Unexpected Hepatic Hydrothorax After a Successful Kasai Portoenterostomy: A Case Report.

Hepatic hydrothorax (HH) represents a rare complication of portal hypertension among adult cirrhotic patients. Here, we describe a pediatric case of HH, observed in a biliary atresia infant. The child presented with recurrent right-sided pleural effusion, after a successful Kasai portoenterostomy with restoration of bile flow and without overt signs of hepatic failure. Recurrence of HH led the patient to liver transplant despite a low pediatric end-stage liver disease value. Although rare, HH can also occur in children and should be suspected in patients with portal hypertension and respirator

Intrahepatic biliary cysts after hepatic portoenterostomy in four children with biliary atresia.

We report our experience with 4 cases of cystic dilatation of intrahepatic bile ducts following hepatic portoenterostomy for biliary atresia. Two of the cases did not achieve satisfactory bile excretion and all four cases developed recurrent cholangitis after hepatic portoenterostomy. The attacks of cholangitis seemed to be associated with the presence of intrahepatic cysts. Although one case resulted in death from hepatic failure, three other cases are now outpatients. Patients who develop recurrent cholangitis following hepatic portoenterostomy, should be examined to exclude the presence of

Factors influencing outcome after hepatic portoenterostomy for biliary atresia: a logistic regression analysis.

BACKGROUND/OBJECTIVE: The association of many factors with the outcome in Biliary atresia (BA) after hepatic portoenterostomy has drawn the attention of many pediatric hepatologists and hepatobiliary surgeons. Understanding these factors will become an important subject in prediction of the postoperative status and in indicating further proper management. MATERIAL AND METHOD: During the last 9 years, 159 BA babies were treated by hepatic portoenterostomy. The authors reviewed the following factors and how they related to outcome: age at operation, total bilirubin (TB) level, type of BA, posto

Cystic dilatation of the intrahepatic biliary system in biliary atresia after hepatic portoenterostomy.

Five cases of intrahepatic biliary cyst or cystic dilatation and one suspicious case were found (6.4%) among 93 survivors of biliary atresia. The age at manifestation of such cysts ranged from 6 months to 12 years. These cysts or cystic dilatations were treated surgically in two cases, by percutaneous transhepatic cholangiodrainage (PTCD) in two, and not treated in one. Four patients are alive; one died of biliary atresia. Twenty-nine well-documented cases of intrahepatic biliary cyst were found in the literature, including five treated by the authors. The modes of dilatation were as follows:

Hepatic portoenterostomy for biliary atresia. A comparative study of histology and prognosis after surgery.

Specimens of excised tissue from the porta hepatis in 26 infants with extrahepatic biliary atresia undergoing hepatic portoenterostomy were analysed histologically for the presence and size of biliary ductules. No correlation could be found between the establishment of effective biliary drainage and the number or size of biliary ductules. it is suggested that prognosis after surgery may be related to the intrahepatic lesion and age of the child at operation rather than to the histology of the extrahepatic bile duct remnants.

Congenital biliary atresia. Analysis of 97 cases with reference to prognosis after hepatic portoenterostomy.

The prognosis of 97 patients with congenital biliary atresia treated by hepatic portoenterostomy was studied. In 37 of 97 patients (38%), good bile excretion was observed after surgery, and jaundice cleared. Sixteen of the 37 patients developed ascending cholangitis; 11, esophageal varices; and nine died. Seven died of ascending cholangitis and two of hepatic failure. Fourteen patients had an uneventful course. In the remaining 60 patients, bile excretion was scanty, and the disease progressed to biliary cirrhosis; 52 died. Causes of death were hepatic failure (53%), intracranial hemorrhage (1

Effect of repeat Kasai hepatic portoenterostomy on pediatric live-donor liver graft for biliary atresia.

OBJECTIVES: Treatment for patients with biliary atresia is a Kasai hepatic portoenterostomy; however, the efficacy of repeat Kasai hepatic portoenterostomy is unclear. This study sought to examine the effect of a prior Kasai hepatic portoenterostomy, especially a repeat Kasai hepatic portoenterostomy, on the outcomes of living-donor liver transplant. MATERIALS AND METHODS: One hundred twenty-six of 170 children that underwent a living-donor liver transplant between May 2001, and March 2010, received a living-donor liver transplant for biliary atresia. These patients were divided into 2 groups
