Post-Cardiac Arrest Syndrome.

More than 356 000 out-of-hospital cardiac arrests occur in the United States annually. Complications involving post-cardiac arrest syndrome occur because of ischemic-reperfusion injury to the brain, lungs, heart, and kidneys. Post-cardiac arrest syndrome is a clinical state that involves global brain injury, myocardial dysfunction, macrocirculatory dysfunction, increased vulnerability to infection, and persistent precipitating pathology (ie, the cause of the arrest). The severity of outcomes varies and depends on precipitating factors, patient health before cardiac arrest, duration of time to

[Managing Post Cardiac Arrest Syndrome].

Four conditions occur after cardiac arrest resuscitation and are referred to as the post-cardiac arrest syndrome. Moreover, post-cardiac arrest brain injury has the greatest impact on outcomes. Brain injury can be primary as a result of global cerebral ischemia during cardiac arrest. It may be secondary(reperfusion injury)after initiation of cardiopulmonary resuscitation. After cardiac arrest resuscitation, the patient must be managed in the intensive care unit, and it is recommended to avoid hypotension(MAP<65 mmHg), hypoxemia, and hyperoxemia. Oxygen saturation should be maintained at 94%-98

Management of post-cardiac arrest syndrome.

Post-cardiac arrest syndrome is a complex and critical issue in resuscitated patients undergone cardiac arrest. Ischemic-reperfusion injury occurs in multiple organs due to the return of spontaneous circulation. Bundle of management practicies are required for post-cardiac arrest care. Early invasive coronary angiography should be considered to identify and treat coronary artery obstructive disease. Vasopressors such as norepinephrine and dobutamine are the first-line treatment for shock. Maintainance of oxyhemoglobin saturation greater than 94% but less than 100% is recommended to avoid fatal

Mechanism of post cardiac arrest syndrome based on animal models of cardiac arrest.

Cardiac arrest (CA) is a critical condition in the field of cardiovascular medicine. Despite successful resuscitation, patients continue to have a high mortality rate, largely due to post CA syndrome (PCAS). However, the injury and pathophysiological mechanisms underlying PCAS remain unclear. Experimental animal models are valuable tools for exploring the etiology, pathogenesis, and potential interventions for CA and PCAS. Current CA animal models include electrical induction of ventricular fibrillation (VF), myocardial infarction, high potassium, asphyxia, and hemorrhagic shock. Although thes

Prognostic utility of the revised post-cardiac arrest syndrome for therapeutic hypothermia score in pediatric patients with post-cardiac arrest syndrome.

BACKGROUND: The accurate prognosis of post-cardiac arrest syndrome (PCAS) is critical for guiding intensive care and family communication. The revised post-Cardiac Arrest Syndrome for Therapeutic hypothermia (rCAST) score incorporates six readily available clinical variables and has shown promise in children. However, prior validations involved older children. We aimed to externally validate rCAST scores in a cohort of infants and young children. METHODS: We retrospectively examined patients with PCAS (with pre-arrest pediatric cerebral performance category scores of 1-3 and receiving targete

Status Myoclonus with Post-cardiac-arrest Syndrome: Implications for Prognostication.

BACKGROUND: Status myoclonus (SM) after cardiac arrest (CA) may signify devastating brain injury. We hypothesized that SM correlates with severe neurologic and systemic post-cardiac-arrest syndrome (PCAS). METHODS: Charts of patients admitted with CA to Mayo Clinic Saint Marys Hospital between 2005 and 2019 were retrospectively reviewed. Data included the neurologic examination, ancillary neurologic tests, and systemic markers of PCAS. Nonsustained myoclonus was clinically differentiated from SM. The cerebral performance category score at discharge was assessed; poor outcome was a cerebral pe

[Consideration of early rehabilitation in the treatment of post-cardiac arrest syndrome].

Resumption of spontaneous circulation (ROSC) after cardiac arrest is an unnatural pathophysiological state. In 2008, ILCOR has proposed "post-cardiac arrest syndrome (PCAS)". Clinicians must focus on treating to reverse the pathophysiological manifestations of PCAS in bed. Immobility, deconditioning, and weakness are common problems in patients with critical illness. Therapeutic strategies have to be identified to give patients after ROSC the best chance for survival with good neurological function. Concerning the beneficial effects of early mobilization after stroke, and the efficacy of a str

Acute lung injury and post-cardiac arrest syndrome: a narrative review.

BACKGROUND: Post-cardiac arrest syndrome (PCAS) presents a multifaceted challenge in clinical practice, characterized by severe neurological injury and high mortality rates despite advancements in management strategies. One of the important critical aspects of PCAS is post-arrest lung injury (PALI), which significantly contributes to poor outcomes. PALI arises from a complex interplay of pathophysiological mechanisms, including trauma from chest compressions, pulmonary ischemia-reperfusion (IR) injury, aspiration, and systemic inflammation. Despite its clinical significance, the pathophysiolog

Optimal Targeted Temperature Management for Patients with Post-Cardiac Arrest Syndrome.

Background: To prevent hypoxic-ischemic brain damage in patients with post-cardiac arrest syndrome (PCAS), international guidelines have emphasized performing targeted temperature management (TTM). However, the most optimal targeted core temperature and cooling duration reached no consensus to date. This study aimed to clarify the optimal targeted core temperature and cooling duration, selected according to the time interval from collapse to return of spontaneous circulation (ROSC) in patients with PCAS due to cardiac etiology. Methods: Between 2014 and 2020, the targeted core temperature was

Vagus Nerve Stimulation Improves Mitochondrial Dysfunction in Post-cardiac Arrest Syndrome in the Asphyxial Cardiac Arrest Model in Rats.

Cerebral mitochondrial dysfunction during post-cardiac arrest syndrome (PCAS) remains unclear, resulting in a lack of therapeutic options that protect against cerebral ischemia-reperfusion injury. We aimed to assess mitochondrial dysfunction in the hippocampus after cardiac arrest and whether vagus nerve stimulation (VNS) can improve mitochondrial dysfunction and neurological outcomes. In an asphyxial cardiac arrest model, male Sprague-Dawley rats were assigned to the vagus nerve isolation (CA) or VNS (CA + VNS) group. Cardiopulmonary resuscitation was performed 450 s after pulseless electrica

CytoSorb® Hemoadsorption in Post-Cardiac Arrest Syndrome After Out-of-Hospital Cardiac Arrest: A Propensity Score-Matched Cohort Study.

Background: Post-cardiac arrest syndrome (PCAS) following out-of-hospital cardiac arrest (OHCA) is driven by global ischemia-reperfusion injury, endothelial dysfunction, and a dysregulated inflammatory response. This cascade frequently culminates in profound vasoplegia and multiorgan failure, even when guideline-directed post-resuscitation management is applied. Hemoadsorption using the CytoSorb device may attenuate hyperinflammation and vasoplegia by removing circulating inflammatory and injury-related mediators. Methods: This single-centre, retrospective cohort study compared adults with PCA

Oxidative stress and abnormal cholesterol metabolism in patients with post-cardiac arrest syndrome.

Patients with post-cardiac arrest syndrome (PCAS) suffer from whole body ischemia/reperfusion injury similar to that experienced by newborn babies. Increased oxidative stress was confirmed in PCAS patients (n = 40) at the time of hospitalization by a significant increase in the percentage of the oxidized form of coenzyme Q10 in total coenzyme Q10 compared to age-matched healthy controls (n = 55). Tissue oxidative damage in patients was suggested by the significant increase in plasma levels of free fatty acids (FFA) and the significant decrease in polyunsaturated fatty acid contents in total FF

Acute respiratory failure and inflammatory response after out-of-hospital cardiac arrest: results of the Post-Cardiac Arrest Syndrome (PCAS) pilot study.

BACKGROUND: Although the lungs are potentially highly susceptible to post-cardiac arrest syndrome injury, the issue of acute respiratory failure after out-of-hospital cardiac arrest has not been investigated. The objectives of this analysis were to determine the prevalence of acute respiratory failure after out-of-hospital cardiac arrest, its association with post-cardiac arrest syndrome inflammatory response and to clarify its importance for early mortality. METHODS: The Post-Cardiac Arrest Syndrome (PCAS) pilot study was a prospective, observational, six-centre project (Poland 2, Denmark 1,

Advances in basic research on post-cardiac arrest syndrome in adults: a comprehensive review.

Post-cardiac arrest syndrome (PCAS) represents a critical systemic ischemia-reperfusion injury occurring after the return of spontaneous circulation in patients who have experienced cardiac arrest. This syndrome encompasses multiple organ dysfunctions and involves complex pathophysiological mechanisms that remain incompletely understood. Despite advances in clinical management, high morbidity and mortality rates persist, underscoring the urgent need for deeper mechanistic insights and novel therapeutic strategies. Current basic research has increasingly focused on elucidating the cellular and

The post-cardiac arrest syndrome: A case for lung-brain coupling and opportunities for neuroprotection.

Systemic inflammation and multi-organ failure represent hallmarks of the post-cardiac arrest syndrome (PCAS) and predict severe neurological injury and often fatal outcomes. Current interventions for cardiac arrest focus on the reversal of precipitating cardiac pathologies and the implementation of supportive measures with the goal of limiting damage to at-risk tissue. Despite the widespread use of targeted temperature management, there remain no proven approaches to manage reperfusion injury in the period following the return of spontaneous circulation. Recent evidence has implicated the lung

Helium ventilation for treatment of post-cardiac arrest syndrome: A safety and feasibility study.

AIM: Besides supportive care, the only recommended treatment for comatose patients after cardiac arrest is target temperature management. Helium reduces ischaemic injury in animal models, and might ameliorate neurological injury in patients after cardiac arrest. As no studies exist on the use of helium in patients after cardiac arrest we investigated whether this is safe and feasible. METHODS: The study was an open-label single arm intervention study in a mixed-bed academic intensive care unit. We included 25 patients admitted after circulatory arrest, with a presenting rhythm of ventricular

Gut microbiota dysbiosis in post-cardiac arrest syndrome: mechanisms, clinical implications, and therapeutic perspectives.

Post-cardiac arrest syndrome (PCAS) is a complex pathophysiological condition triggered by systemic ischemia-reperfusion injury following cardiac arrest and is frequently associated with multi-organ dysfunction and poor neurological outcomes. Emerging evidence suggests that the gut microbiota, increasingly recognized as a key regulator of host immunity, metabolism, and intestinal barrier integrity, may participate in the pathogenesis and progression of PCAS. Alterations in microbial composition and function following cardiac arrest have been associated with systemic inflammation, immune dysreg
