Primary posterior laser-assisted capsulotomy.

PURPOSE: To describe new techniques for performing primary posterior laser-assisted capsulotomy (PLC) to prevent posterior capsule opacification after cataract surgery. METHODS: After lens and cortex removal, three different techniques were used to cut the posterior capsule with an image-guided (optical coherence tomography [OCT]) femtosecond laser. Technique 1: Prior to intra-ocular lens (IOL) implantation, the posterior capsule was carefully opened and elevated with ophthalmic viscosurgical device (OVD) whereas the anterior hyaloid surface remains intact. After sterile re-docking, the poste

Complications of Nd:YAG laser posterior capsulotomy.

A number of ocular complications are seen to be associated with the use of Nd:YAG laser for posterior capsulotomy after cataract surgery. We have outlined possible hazards to the cornea, trabecular meshwork, vitreous and intraocular lenses and have discussed their consequences. It would appear that the degree of severity of complications is related to the pulse energy used upon the capsule.

Retinal detachment following late posterior capsulotomy.

When we compared the incidence of retinal detachment in 27 eyes after extracapsular cataract surgery and late posterior capsulotomy with the incidence of general aphakic detachment, we found no relationship between the onset of retinal detachment and the numbers, types, or locations of breaks. The interval between cataract extraction and discission had a significant effect on recovery of visual acuity. Final visual acuities of 20/400 or better were attained in 22 of 23 eyes in which discission followed cataract extraction by one year or more but in only one of four eyes in which the procedures

Uveoscleritis after excessive neodymium:YAG laser posterior capsulotomy.

A 66-year-old woman developed severe and recurrent scleritis and uveitis after neodymium:YAG capsulotomy performed 18 months after cataract extraction. Four cracks on the intraocular lens and plastic splinters in the vitreous indicated that excessive laser energy had been used. Inflammation was treated successfully only when a combination of dexamethasone acetate 0.1% drops (Maxidex), drops of diclofenac sodium 0.1% (Voltaren Ophtha), and systemic diclofenac sodium (Voltaren) was used. Inflammation might be explained by chronic irritation of the ciliary body by a displaced haptic or by an immu

Secondary closure of neodymium:YAG laser posterior capsulotomy.

A 67-year-old man reported reduced vision after cataract surgery and neodymium:YAG (Nd:YAG) laser posterior capsulotomy. The diagnosis was complicated due to severe glistenings within the optic of an acrylic foldable intraocular lens (IOL), proliferation of lens epithelial remnants over the capsulotomy opening, and presumed vitreous opacity. Although IOL exchange surgery was considered, a second Nd:YAG laser intervention successfully removed the proliferated lens materials and restored the visual acuity. The glistenings were not the cause of the reduced vision.

[Complications following Nd: YAG laser posterior capsulotomy].

We used neodymium-YAG laser to perform discissions of the posterior capsules in 221 eyes of 203 patients who had undergone previous extracapsular cataract extractions. After a follow-up period ranging from 1-96 months, in the mean 10.5 months, 47.06% of the eyes achieved corrected visual acuities of > or = 0.5. The complications included rhegmatogenous retinal detachment in 3 eyes (1.36%) and bullous keratopathy in one eye (0.45%). Although cystoid macular edema and glaucoma were not seen in this series, they had been reported. The author suggests that patients undergoing Nd: YAG laser capsulo

[Posterior YAG capsulotomy: selection of the application pattern].

BACKGROUND: The posterior neodymium-yttrium-aluminum-garnet laser capsulotomy is an established and simple method with a low complication rate for the treatment of secondary cataracts; however, the risk of intraocular pressure elevation, pit marks of the intraocular lens (IOL), anterior hyaloid damage, cystoid macular edema and retinal detachment may increase with high pulse number, pulse energy, and total energy. PURPOSE: The optimization potential of the method through a rational choice of the laser pattern and the strategy, taking into account the mechanical properties of the posterior cap

Posterior Approach and Inferior Capsulotomy in Bipolar Hemiarthroplasty for Femoral Neck Fractures: Comparison with Superior Capsulotomy.

BACKGROUND: Hemiarthroplasty is frequently used to treat displaced femoral neck fractures in elderly patients, but it has a higher risk of postoperative dislocation. We introduced the posterior approach and inferior capsulotomy (PAICO) to enhance joint stability after bipolar hemiarthroplasty for femoral neck fracture. We evaluated whether the PAICO would have a lower dislocation rate than the conventional posterior approach with superior capsulotomy. METHODS: From January 2021 to December 2021, we prospectively recruited 25 patients (25 hips) aged 50 years or older who underwent bipolar hemi

[Retinal detachment following posterior capsulotomy using a YAG laser].

This work analyzes 9 cases of retinal detachment occurred after posterior capsulotomy with Yag laser. No relationship has been found between the timing of the Yag capsulotomy after cataract extraction, and seriousness or precocity of the retinal detachment. The size of the capsulotomy does not seem to play a main part either. The study shows the part of the posterior capsule and the importance of associated risks (myopia, peripheral degenerative lesions, previous retinal detachment in the fellow eye). The role of the Yag laser treatment seems doubtful.

Long-term follow-up of Nd:YAG laser posterior capsulotomy.

A Nd:YAG laser developed for ophthalmic surgery was studied for safety and efficacy in secondary discission of the posterior capsule in aphakic and pseudophakic subjects. The study involves 6,800 subjects, 526 of whom had completed the six-month postoperative course by the time the data base was closed for analysis. Of the 526 subjects who were treated with the laser, 87.8% had improved vision, with 82.9% achieving a visual acuity of 20/40 or better. This result compares favorably with results for the surgically treated population in which only 68.4% experienced improved vision, with 80.2% ach

Retrospective study of retinal detachment following neodymium:YAG laser posterior capsulotomy.

A retrospective study of 1,000 cases that had Nd:YAG laser posterior capsulotomy after cataract surgery is presented. We analyzed the correlation of the patient's age, axial length of the eye, method of cataract surgery, and laser parameters (exposures, energy, and burst mode) with the incidence of retinal detachment (1.6% overall). The highest risk for retinal detachment (12.3%) was in patients with an axial eye length of 26.1 mm to 28.0 mm. The average age of patients with retinal detachments was 60.6 years, ten years younger than the collective average age. Laser parameters, such as energy,
