Revision Posterior Cruciate Ligament Reconstruction Technique.

BACKGROUND: Revision posterior cruciate ligament reconstruction (PCLR) is challenging, especially in existing semi-anatomic tunnels, whereas there is an increased risk of overlapping with a new positioned anatomic tunnel. Few cases were published with no consensus regarding the optimal operative technique. INDICATIONS: A 22-year-old male patient, with failed PCLR with hamstring autograft in 2020 due to improper tunnels placement, presented for pain and instability of his right knee. Physical examination revealed a positive posterior drawer and reverse Pivot-Shift test, with no varus-valgus or

Posterior cruciate ligament reconstruction: current trends.

A survey was conducted of the Herodicus Society membership to assess the current status of treating posterior cruciate ligament (PCL) injuries and compared to a similar survey conducted in 1991. The survey addressed indications, graft choice, surgical technique, graft tensioning, and graft fixation. Seventy-eight active surgeons were sent a 34-question survey pertaining to isolated PCL injuries. The majority of respondents (78%) performed < or = 10 PCL reconstructions per year. The arthroscopic assisted with posteromedial portal technique was the most commonly used (49%). The tibial inlay/onla

Arthroscopic femoral tensioning and posterior cruciate ligament reconstruction in chronic posterior cruciate ligament injury.

For the new arthroscopic femoral tensioning and posterior cruciate ligament (PCL) reconstruction in chronic PCL injury, we make a transtibial tunnel, using the posterior trans-septal portal, while preserving the remnant bundle of the original PCL and meniscofemoral ligament. Also, to preserve the remnant femoral fibers of the PCL and provide femoral tensioning, we manipulate the suture hook with No.1 PDS by hand so that the sharp tip penetrates the remnants of the PCL near the femoral attachment site. After pulling the sutures previously placed at the remnant PCL fibers, the notchplasty osteot

One-stage posterior cruciate ligament inlay reconstruction combining anterior cruciate ligament reconstruction following knee dislocation.

Knee dislocation is a rare but severe injury that involves damage to the anterior cruciate ligament (ACL), posterior cruciate ligament (PCL), lateral or medial ligamentous structures, and other soft tissues or bony structures surrounding the knee joint. No consensus exists regarding the best treatment method. This work presents a method of 1-stage ACL and PCL reconstruction in which a contralateral quadriceps tendon-bone autograft with tibial inlay technique is used for the PCL and contralateral hamstring tendon autograft with suspension fixation is used for the ACL. After harvesting grafts, t

Posterior Cruciate Ligament Agenesis Requiring Surgical Reconstruction.

Agenesis of the posterior cruciate ligament (PCL) is a rare disorder often found in conjunction with various other abnormalities within the knee. A 15-year-old adolescent boy presented with an absent PCL, leading to intermittent symptoms. At the age of 20 years, the patient underwent arthroscopically assisted PCL reconstruction. The patient was permitted to weightbear immediately after the procedure and started physical therapy 4 weeks after the procedure. No complications were encountered at the 2-year follow-up.

Posterior Cruciate Ligament Reconstruction in the Multiple Ligament Injured Knee.

Multiligamentous knee injuries are challenging to treat and diagnose. Posterior cruciate ligament (PCL) injuries are commonly found in the constellation of injuries included in a multiligamentous knee injury and are caused by a posteriorly directed force on the proximal tibia with relation to the femoral condyles. A thorough history and physical examination should be performed to evaluate for associated neurovascular injuries and associated ligamentous, chondral, or bony injuries. Nonsurgical management is reserved for patients who are critically ill or have very low activity demands. Surgical

An evaluation of posterior cruciate ligament reconstruction surgery.

BACKGROUND: The nature of posterior cruciate ligament (PCL) injuries and the scarcity of data on this issue have made reports on clinical and epidemiological features of PCL injuries valuable. We aimed to report our experiences with PCL injuries in our region. METHODS: Any patient who referred with a diagnosis of PCL rupture from 2004 to 2018 to our center, was included in this report. We evaluated pre- and postoperative outcomes and compared patients with isolated and combined (multi-ligament) PCL injuries. RESULTS: Overall, 55 patients were included in our study. Majority of patients were

Arthroscopic tibial inlay for posterior cruciate ligament reconstruction.

The treatment of posterior cruciate ligament injuries remains controversial. There is no consensus on the ideal technique for reconstruction or the optimal graft design. Specifically, there is intense debate about which technique is superior: the open tibial inlay technique or the transtibial arthroscopic technique. We present a novel all-arthroscopic tibial inlay technique using a bifid bone-patellar tendon-bone allograft. This technique offers the advantages of the open tibial inlay technique, including avoidance of the killer turn and restoration of biomechanical stability. Furthermore, thi

A new technique for posterior cruciate ligament reconstruction.

We describe a new technique for arthroscopically assisted posterior cruciate ligament reconstruction. We use the TransFix (Arthrex, Naples, FL) pin to provide a single and strong tibial attachment point for the 4-stranded semitendinosus and gracilis tendons. The TransFix technique for anterior cruciate ligament reconstruction has been found to give better initial fixation strength than other techniques using semitendinosus and gracilis grafts, and it provides adequate graft length for secure fixation. Strong and secure graft fixation in knee ligament reconstruction is very important for early

Reconstruction of osseous rupture of posterior cruciate ligament.

UNLABELLED: Osseous avulsion of the posterior cruciate ligament from the articular surface of the tibia requires surgical treatment. If the avulsed fragment is large enough, fixation by means of two crossed drill wires exiting on the extensor surface of the tibia is the recommended treatment. Advantages of this method: a) technically simple operation; b) wires can be removed in the outpatient department under local anesthesia. CASE REPORTS: five patients were operated on, all of them with good results.

Combined chronic anterior cruciate ligament and posterior cruciate ligament reconstruction: functional and clinical results.

PURPOSE: Multiligamentous injury to the anterior cruciate ligament (ACL) and the posterior cruciate ligament (PCL) is an uncommon but debilitating event. Patients with combined ligament injuries typically complain of painful, debilitating knee instability that restricts their sports and daily activities. The purpose of this retrospective study was to evaluate functional and clinical outcomes of patients with chronic ACL and PCL deficiency who underwent simultaneous single-stage arthroscopic reconstruction of the central pivot. METHODS: Medical records of 20 consecutive patients with chronic A

Dorsal approach for reconstruction of the posterior cruciate ligament.

The operative treatment of a posterior cruciate ligament rupture through a straight central approach in the hollow of the knee is described. Following evaluation of the different operative procedures used in our clinic during the years of 1977 through 1990, this is now the preferred technique. The results of the different procedures are summarized. Clinical follow-up examinations were carried out in 75 patients operated on between 1977 and 1990, after a mean postoperative follow-up period of 6 years. For each patient the clinical results were evaluated both subjectively and objectively using O

How to Set Anterior and Posterior Tibial Translation for Combined Anterior Cruciate Ligament and Posterior Cruciate Ligament Reconstruction.

Anterior and posterior cruciate ligament (ACL and PCL) reconstruction poses the risk of residual anterior or posterior tibial translation, respectively. As a result, patients may have residual knee instability and laxity, which can contribute to poor outcomes and graft failure. Ultimately, minimizing anterior and posterior tibial translation through proper ACL and PCL graft tensioning has the potential to improve clinical and functional outcomes in the setting of ACL and PCL reconstruction surgery. This technique aims to show a reproducible method for "setting" anterior and posterior tibial tr

Tips for Successful Double-Bundle Posterior Cruciate Ligament Reconstruction.

Posterior cruciate ligament (PCL) injuries are a relatively common knee injury that can occur in isolation but are more commonly associated with multiligamentous knee injuries. PCL reconstruction is a technically challenging and complex orthopaedic procedure. Improper reconstruction techniques may result in significant complications, including residual instability, graft failure, and damage to adjacent neurovascular structures. Several strategies can be implemented to obtain improved outcomes while mitigating complications, including techniques for optimizing arthroscopic visualization, graft

A biomechanical comparison of posterior cruciate ligament reconstruction techniques.

Most posterior cruciate ligament reconstruction techniques use both tibial and femoral bone tunnels for graft placement. Because of the acute angle the graft must make to gain entrance into the tibial tunnel, abnormal stresses are placed on the graft that could lead to graft failure. An alternative technique for posterior cruciate ligament reconstruction involves placement of the bone plug from the graft anatomically on the back of the tibia (inlay), preventing formation of an acute angle at the tibial attachment site. We used six pairs of human cadaver knees to compare the biomechanical prope

Posterior cruciate ligament reconstruction using a septum-preserving technique.

OBJECTIVE: Description of a reproducible surgical technique for single-bundle anterolateral reconstruction of the posterior cruciate ligament (PCL) based on a septum-sparing approach. This technique is less traumatic than the trans-septum approach. The article illustrates surgical steps to simplify the technical aspects of the procedure. INDICATIONS: A complete grade III symptomatic tear of the PCL associated with instability and often discomfort (deceleration, stairs) or subsequent gonalgia arising from the medial compartment or patellofemoral joint. Injury of the peripheral joint stabilizer

Bicondylar tibial plateau fracture after posterior cruciate ligament reconstruction.

The authors present a report of a bicondylar tibial plateau fracture in an adolescent athlete after posterior cruciate ligament (PCL) reconstruction. The procedure was performed via arthroscopic transtibial PCL reconstruction with quadrupled semi-tendinosus and gracilis autograft. The patient recovered uneventfully postoperatively and was able to participate in high-level sports activity, such as baseball and track, with no limitations, no subjective complaints, and no episodes of instability. He continued to be asymptomatic up to 3.5 years postoperatively. Almost 4 years postoperatively, the

Popliteal artery laceration during arthroscopic posterior cruciate ligament reconstruction.

We report a case of acute laceration of the popliteal artery during an arthroscopic posterior cruciate ligament reconstruction. This injury can occur during the creation of the posteromedial portal, the manipulation of the tissues in the posterior part of the capsule of the knee joint, or when drilling the tibial hole. We recommend that a qualified vascular surgeon should be immediately available at the time of the surgery. In case of suspecting the occurrence of a popliteal artery injury, the vascular surgeon should be immediately consulted and arteriography and vascular repair should be perf
