Fragmentation of the Golgi apparatus of the anterior horn cells in patients with familial amyotrophic lateral sclerosis with SOD1 mutations and posterior column involvement.

The Golgi apparatus (GA) of the anterior horn cells in the spinal cord was examined by immunohistological methods with an antibody against the MG-160 protein, a conserved intrinsic membrane sialoglycoprotein of the medial cisternae of the GA, in three patients with familial amyotrophic lateral sclerosis (FALS) with posterior column involvement. Large motor neurons in the anterior horns were markedly reduced in number and 10 of total 14 remaining large motor neurons showed fragmentation and a reduction in the number of the elements of the GA. The fragmentation of the GA was identical to that pr

Reorganization of synaptic inputs to spinal dorsal horn neurons in neuropathic pain.

Peripheral nerve injuries produce a variety of negative structural and functional changes in the central terminal sites of damaged axons, as well as the injured primary afferents. Such changes have been shown to be involved in the development of neuropathic pain, which includes abnormal pain sensations such as allodynia and hyperalgesia. Since the spinal dorsal horn is the first central site where signals from peripheral sensory nerves are transmitted and shows a variety of changes after peripheral nerve injury or chronic inflammation of peripheral tissues, it is one of the most important site

[Anesthetic mechanisms in the spinal cord].

The essential elements of anesthesia are : hypnosis, amnesia, analgesia, immobility, and inhibition of untoward reflexes. The spinal cord is responsible for the latter three. Suppression of excitatory transmission and stimulation of inhibitory transmission are the anesthetic mechanisms in the spinal cord. Each anesthetic, however, has a unique effect on the transmission systems in the spinal cord. Some exclusively suppress excitatory transmission or stimulate inhibitory transmission, and others have a dual effect. The minimum alveolar/anesthetic concentration (MAC) is spinally mediated. Excitatory and Inhibitory Neurons of the Spinal Cord Superficial Dorsal Horn Diverge in Their Somatosensory Responses and Plasticity in Vivo.

The superficial dorsal horn (SDH) of the spinal cord represents the first site of integration between innocuous and noxious somatosensory stimuli. According to gate control theory, diverse populations of excitatory and inhibitory interneurons within the SDH are activated by distinct sensory afferents, and their interplay determines the net nociceptive output projecting to higher pain centers. Although specific SDH cell types are ill defined, numerous classifications schemes find that excitatory and inhibitory neurons fundamentally differ in their morphology, electrophysiology, neuropeptides, a

Effects of Corticosterone on the Excitability of Glutamatergic and GABAergic Neurons of the Adolescent Mouse Superficial Dorsal Horn.

Stress evokes age-dependent effects on pain sensitivity and commonly occurs during adolescence. However, the mechanisms linking adolescent stress and pain remain poorly understood, in part due to a lack of information regarding how stress hormones modulate the function of nociceptive circuits in the adolescent CNS. Here we investigate the short- and long-term effects of corticosterone (CORT) on the excitability of GABAergic and presumed glutamatergic neurons of the spinal superficial dorsal horn (SDH) in Gad1-GFP mice at postnatal days (P)21-P34. In situ hybridization revealed that glutamaterg

Analysis of spontaneous activity of superficial dorsal horn neurons in vitro: neuropathy-induced changes.

The superficial dorsal horn contains large numbers of interneurons which process afferent and descending information to generate the spinal nociceptive message. Here, we set out to evaluate whether adjustments in patterns and/or temporal correlation of spontaneous discharges of these neurons are involved in the generation of central sensitization caused by peripheral nerve damage. Multielectrode arrays were used to record from discrete groups of such neurons in slices from control or nerve damaged mice. Whole-cell recordings of individual neurons were also obtained. A large proportion of neuro

Computational Optimization of Spinal Cord Stimulation for Dorsal Horn Interneuron Polarization.

OBJECTIVES: The proposed mechanisms of spinal cord stimulation (SCS) follow the polarization of dorsal column axons; however, the development of subparesthesia SCS has encouraged the consideration of different targets. Given their relative proximity to the stimulation electrodes and their role in pain processing (eg, synaptic processing and gate control theory), spinal cord dorsal horn interneurons may be attractive stimulation targets. MATERIALS AND METHODS: We developed a computational modeling pipeline termed "quasiuniform-mirror assumption" and applied it to predict polarization of dorsal

Sensory plasticity of dorsal horn silent neurons as a critical mechanism for neuropathic pain.

The spinal cord dorsal horn (DH) integrates sensory processing but undergoes critical plasticity following nerve injury, leading to mechanical allodynia, or "touch-evoked pain". It has been proposed that after nerve injury, innocuous sensory neurons gain access to nociceptive-specific (NS) circuits in the DH due to altered spinal inhibitory controls, thereby converting touch into pain. It is however unclear how sensory processing is reorganized under these conditions to generate this symptom. In this study, we used two-photon calcium imaging in mice to show that spinal disinhibition or nerve i

Spinal Dorsal Horn Neurons Receiving Descending Input from the Primary Somatosensory Cortex Contribute to Aβ Fiber-Induced Neuropathic Allodynia in Male Rats.

Mechanical allodynia is the predominant symptom of neuropathic pain following peripheral nerve injury (PNI) and is characterized by pain evoked by innocuous sensory signals transmitted through low-threshold mechanoreceptive primary afferents, including Aβ fibers. However, the underlying neural mechanisms remain insufficiently understood. Previous studies have suggested that the pathological conversion of tactile input into nociceptive signals involves maladaptive alterations in neural circuits and function within the spinal dorsal horn (SDH). Somatosensory processing and transmission in the SD

Opposite regulation of medullary pain-related projection neuron excitability in acute and chronic pain.

Pain hypersensitivity is associated with increased activity of peripheral and central neurons along the pain neuroaxis. We show that at the peak of acute inflammatory pain, superficial medullary dorsal horn projection neurons (PNs) that relay nociceptive information to the parabrachial nucleus reduce their intrinsic excitability and, consequently, action potential firing. When pain resolves, the excitability of these neurons returns to baseline. Using electrophysiological and computational approaches, we found that an increase in potassium A-current (IA) underlies the decrease in the excitabil

Grpr expression defines a population of superficial dorsal horn vertical cells that have a role in both itch and pain.

Neurons in the superficial dorsal horn that express the gastrin-releasing peptide receptor (GRPR) are strongly implicated in spinal itch pathways. However, a recent study reported that many of these correspond to vertical cells, a population of interneurons that are believed to transmit nociceptive information. In this study, we have used a GRPR CreERT2 mouse line to identify and target cells that possess Grpr mRNA. We find that the GRPR cells are highly concentrated in lamina I and the outer part of lamina II, that they are all glutamatergic, and that they account for ∼15% of the excitatory n

Neuronal Sensitization and Synaptic Facilitation in the Superficial Dorsal Horn of a Rat Reserpine-induced Pain Model.

Chronic widespread pain is one of the important issues to be solved in medical practice. Impaired spinal descending pain inhibitory system due to decreased monoamine neurotransmitters is assumed to cause nociceptive hypersensitivities in chronic painful conditions like that described in patients with fibromyalgia (FM). However, response behaviors and synaptic transmission of the spinal dorsal horn neurons in response to reserpine remain to be clarified. Here we examined the activities of superficial dorsal horn (SDH) neurons, as well as excitatory and inhibitory postsynaptic inputs to SDH neur

Peripheral nerve injury-induced rearrangement of neural circuit in the spinal dorsal horn revealed by cross-correlation analysis.

Peripheral nerve injury often induces abnormal pain states, such as hyperalgesia and allodynia. In this study, we attempted to elucidate how neurons are synaptically integrated into the neuronal circuitry in the spinal dorsal horn and how synaptic connectivity patterns among dorsal horn neurons are altered by peripheral nerve injury. Experiments were performed on 6-8-week-old ICR mice. Partial sciatic nerve ligation was performed. Transverse slices of the lumbar spinal cord were prepared. Spike activities were simultaneously recorded from multiple neurons in the superficial dorsal horn (SDH) u

A quantitative pathological investigation of the cervical cord, roots and ganglia after long-term amputation of the unilateral upper arm.

A pathological study was conducted on an autopsied patient who had undergone amputation of the right arm at the level of the shoulder 38 years prior to death. The numbers of anterior horn cell, spinal ganglion cells and myelinated fibers in the anterior and posterior spinal roots at the cervical segments were examined quantitatively and compared with those of age-matched control subjects. On the amputation side, anterior horn cells, spinal ganglion cells and large myelinated fibers of the anterior and posterior roots were decreased in number. In addition, on the spared side, the medium-sized n

Ontogeny of the spinal cord dorsal horn.

The dorsal horn of the mammalian spinal cord is organized into laminae where each layer is populated by different neuron types, has distinctive circuit connections, and plays specialized roles in behavior. An outstanding question is how this organization emerges during development from an apparently homogeneous pool of neural progenitors. Here, we show that mouse dorsal neurons are diversified by time, with families of related cell types born as temporal cohorts, and by a spatial-molecular gradient that specifies individual cell types. Excitatory neurons settle into a chronotopic arrangement t

Sodium-calcium exchanger-3 regulates pain "wind-up": From human psychophysics to spinal mechanisms.

Repeated application of noxious stimuli leads to a progressively increased pain perception; this temporal summation is enhanced in and predictive of clinical pain disorders. Its electrophysiological correlate is "wind-up," in which dorsal horn spinal neurons increase their response to repeated nociceptor stimulation. To understand the genetic basis of temporal summation, we undertook a GWAS of wind-up in healthy human volunteers and found significant association with SLC8A3 encoding sodium-calcium exchanger type 3 (NCX3). NCX3 was expressed in mouse dorsal horn neurons, and mice lacking NCX3 s

NaV1.7 mRNA and Protein Expression in Resident Neurons of the Human Spinal Dorsal Horn.

The voltage-gated sodium channel NaV1.7 is a pain target supported by human genetics, and many compounds have been developed to inhibit NaV1.7 but have disappointed in clinical trials due to cardiovascular effects. Because some recent reports suggest that pharmacological inhibition of NaV1.7 in the rodent spinal dorsal horn can achieve pain relief, we sought to better understand NaV1.7 expression in the human spinal cord. We report that NaV1.7 mRNA is expressed in putative projection neurons (NK1R+, GPR83+) in the human spinal dorsal horn, predominantly in lamina I and II, as well as in deep d

Network model of nociceptive processing in the superficial spinal dorsal horn reveals mechanisms of hyperalgesia, allodynia, and spinal cord stimulation.

The spinal dorsal horn (DH) processes sensory information and plays a key role in transmitting nociception to supraspinal centers. Loss of DH inhibition during neuropathic pain unmasks a pathway from nonnociceptive Aβ-afferent inputs to superficial dorsal horn (SDH) nociceptive-specific (NS) projection neurons, and this change may contribute to hyperalgesia and allodynia. We developed and validated a computational model of SDH neuronal circuitry that links nonnociceptive Aβ-afferent inputs in lamina II/III to a NS projection neuron in lamina I via a network of excitatory interneurons. The exci

Activation of spinal dorsal horn astrocytes by noxious stimuli involves descending noradrenergic signaling.

Astrocytes are critical regulators of neuronal function in the central nervous system (CNS). We have previously shown that astrocytes in the spinal dorsal horn (SDH) have increased intracellular Ca2+ levels following intraplantar injection of the noxious irritant, formalin. However, the underlying mechanisms remain unknown. We investigated these mechanisms by focusing on the role of descending noradrenergic (NAergic) signaling because our recent study revealed the essential role of the astrocytic Ca2+ responses evoked by intraplantar capsaicin. Using in vivo SDH imaging, we found that the Ca2+

Dorsal horn DCC amplification loop induced by endplate osteoclasts generates chronic nociplastic low back pain in male mice.

Spine degeneration is associated with low back pain (LBP), a major cause of disability and functional decline. However, effective therapies for non-specific LBP remain limited because its underlying mechanisms are poorly understood. Here, we show that spine degeneration induces netrin-1 secretion in porous endplates to activate DCC signaling in dorsal root ganglion (DRG) neurons in both LBP and naturally aged male mice. Conditional loss of netrin-1 in Trap+ osteoclasts, or Dcc in Avil+ DRG neurons reduced spinal hypersensitivity. Importantly, DCC activation induces Src phosphorylation and SNAR
