Postoperative cognitive recovery and prevention of postoperative cognitive complications in the elderly patient.

Elderly patients undergoing surgery are at higher risk of life-altering and costly complications. This challenge is increasingly recognized with the growing geriatric surgical population. Advanced age and comorbid conditions, such as disability and frailty that often develop with age, are all independent risk factors of postoperative morbidity and mortality. A common factor in this age group is cognitive impairment, which poses a challenge for the patient and clinician in the perioperative setting. It affects the capacity for informed consent and limits optimization before surgery; furthermore

Acupuncture-related techniques for postoperative cognitive complications: a systemic review and meta-analysis.

BACKGROUND: Postoperative cognitive complications are major challenges for postoperative recovery. Acupuncture-related techniques have been used for treating neurocognitive dysfunctions. However, whether they help to prevent postoperative cognitive complicationss remains unclear. We intend to evaluate the effect of acupuncture-related techniques on the incidence of postoperative cognitive complications (PCC) in patients undergoing surgery under general anesthesia. METHODS: Based on PRISMA guidelines, a search of PubMed, EMBASE, Web of Science, and the Cochrane Central Register of Controlled T

The Impact of Cognitive Impairment on Postoperative Complications After Spinal Surgery: A Matched Analysis.

OBJECTIVE: The coprevalence of age-related comorbidities such as cognitive impairment and spinal disorders is increasing. No studies to date have assessed the postoperative spine surgery outcomes of patients with mild cognitive impairment (MCI) or severe cognitive impairment (dementia) compared with those without preexisting cognitive impairment. METHODS: Using all-payer claims database, 235,123 persons undergoing either cervical or lumbar spine procedures between January 2010 and October 2020 were identified. Exact 1:1:1 matching based on baseline patient demographics and comorbidities was u

Cerebrovascular disease and postoperative cognitive-related complications after knee arthroplasty: evidence from a nationwide cohort.

BACKGROUND: Patients with a history of cerebrovascular disease may be at an increased risk for postoperative complications following knee arthroplasty; however, previous studies have been limited by small sample sizes and insufficient adjustment for confounding variables. This study aimed to evaluate whether cerebrovascular disease is associated with postoperative complications using a nationwide Japanese database. METHODS: A retrospective cohort study was conducted using Japan's Diagnosis Procedure Combination database from April 2016 to March 2023. Patients who underwent total knee arthropl

Poor Performance on a Preoperative Cognitive Screening Test Predicts Postoperative Complications in Older Orthopedic Surgical Patients.

BACKGROUND: The American College of Surgeons and the American Geriatrics Society have suggested that preoperative cognitive screening should be performed in older surgical patients. We hypothesized that unrecognized cognitive impairment in patients without a history of dementia is a risk factor for development of postoperative complications. METHODS: We enrolled 211 patients 65 yr of age or older without a diagnosis of dementia who were scheduled for an elective hip or knee replacement. Patients were cognitively screened preoperatively using the Mini-Cog and demographic, medical, functional,

Association of preoperative cognitive frailty with postoperative complications in older patients under general anesthesia: a prospective cohort study.

BACKGROUND: Cognitive frailty (CF) is characterized by the coexistence of physical frailty and cognitive impairment, and it is associated with adverse health outcomes. Older adults are particularly vulnerable to CF due to factors such as age-related brain changes and the presence of comorbidities. OBJECTIVE: To investigate the effect of preoperative CF on postoperative complications in older patients. METHODS: This prospective cohort study was conducted among 253 patients aged 60-85 years, who underwent elective orthopedic and abdominal surgery (with a postoperative hospital stay of ≥ 3 days

Increased Risk of Postoperative Complications in Elderly Hip Fracture Patients With Cognitive Impairment: Evidence From a Japanese Nationwide Database.

BACKGROUND: Hip fractures are a major cause of morbidity and mortality in the elderly. Cognitive impairment is known to be associated with poor postoperative outcomes, but its specific impact on complications following hip fracture surgery remains under-investigated in Japan. METHODS: This nationwide retrospective cohort study used the Diagnosis Procedure Combination (DPC) database to analyze patients aged ≥ 65 years who underwent surgical treatment for hip fractures between April 2016 and March 2022. Cognitive impairment was identified using ICD-10 codes. Propensity score matching (1:1) was

Early postoperative neurocognitive complications in elderly patients: comparing those with and without preexisting mild cognitive impairment- a prospective study.

BACKGROUND: As societies age, increasing numbers of older adults undergo surgeries with anesthesia. Postoperative delirium (POD) and postoperative cognitive dysfunction (POCD) frequently occur in older surgical patients. Most of these patients already have preoperative mild cognitive impairment (MCI). However, the correlation between MCI and POD remains unclear. This study aimed to determine the incidence of POD in elderly patients with and without preexisting MCI. METHODS: A prospective study enrolled patients aged 60 years and above scheduled for major surgeries between December 2017 and Ap

Hospital exposome and physiological resilience are associated with postoperative delirium, major complications, and long-term cognitive decline in older adults undergoing cancer surgery: A prospective multidomain cohort study.

BACKGROUND: Postoperative delirium (POD) and long-term cognitive decline remain frequent and incompletely explained complications in older adults undergoing cancer surgery. Hospital environmental exposures (exposomes) may be associated with age-related physiological vulnerability, yet prospective evidence integrating ward-level exposome data with multidomain physiological resilience is limited. OBJECTIVE: To examine the associations of hospital environmental burden and physiological resilience with POD, major complications, cognitive trajectories, and long-term oncologic outcomes. Preoperative Cognitive Optimization and Postoperative Cognitive Outcomes: A Narrative Review.

BACKGROUND: Perioperative neurocognitive disorder (PND) is a growing concern and affects millions of older adult surgical patients each year in the United States. However, the effective prevention of PND has yet to be established. Recently, preoperative brain exercise has been suggested to decrease postoperative delirium incidence in older patients. This review aims to interpret existing preoperative cognitive optimization research, determine if the research supports preoperative cognitive optimization, and identify gaps in the knowledge of the older surgical population. METHODS: A literature

Determining influences of intraoperative s-ketamine on postoperative delirium and cognitive function: methodology is important.

The letter to the editor was written in response to the recent article by Wang et al. "The influence of low-dose s-ketamine on postoperative delirium and cognitive function in older adults undergoing thoracic surgery", which is published in Journal of Cardiothoracic Surgery. 2024; 19(1):324. This article concludes that intravenous low-dose S-ketamine during thoracic surgery in older patients significantly reduces the incidences of postoperative delirium and cognitive dysfunction, indicating a potential neuroprotective effect of intraoperative low-dose S-ketamine. In view to the retrospective n

Older Patients' Postoperative Neurocognitive Recovery: A Narrative Review.

Perioperative neurocognitive disorders, including postoperative delirium, delayed neurocognitive recovery and mild/major postoperative neurocognitive disorder, are common complications that affect older adults after surgery. Postoperative delirium, which occurs in 10-60% of major surgery patients, can increase the risk of delayed neurocognitive recovery and postoperative neurocognitive disorder, which affects 10-25% of patients. Preoperative and postoperative assessment of neurocognitive functioning typically involve the use of screening tools, such as Mini-Cog or 4AT. Despite the availability

Postoperative cognitive dysfunction: a concept analysis.

BACKGROUND: Post-operative cognitive dysfunction (POCD) is a concern for clinicians that often presents post-surgery where generalized anesthesia has been used. Its prevalence ranges from 36.6% in young adults to 42.4% in older individuals. Conceptual clarity for POCD is lacking in the currently body literature. Our two-fold purpose of this concept analysis was to (1) critically appraise the various definitions, while also providing the best definition, of POCD and (2) narratively synthesize the attributes, surrogate or related terms, antecedents (risk factors), and consequences of the concept
