Rotational intraperitoneal pressurized aerosol chemotherapy in a porcine model.

Peritoneal carcinomatosis (PC) commonly represent drug resistance to intravenous (IV) chemotherapy in advanced or recurrent disease of solid tumors. For improving the prognosis of PC, intraperitoneal (IP) chemotherapy has been introduced in the clinical setting, and phase III trials proved the superiority of IP chemotherapy to IV chemotherapy, in particular, in advanced ovarian cancer. However, increased toxicities by IP chemotherapy lead to reduced cycles of chemotherapy, which does not guarantee its effectiveness. Moreover, hyperthermic IP chemotherapy after cytoreductive surgery also showed

BACKGROUND: Pressurized intraperitoneal aerosol chemotherapy (PIPAC) is a novel approach for treating peritoneal carcinomatosis. First encouraging results have been obtained in human patients. However, delivering chemotherapy as an aerosol might result in an increased risk of exposure to health care workers, as compared with other administration routes. METHODS: PIPAC was applied in two human patients using chemotherapeutic drugs (doxorubicin and cisplatin), and air contamination levels were measured under real clinical conditions. Air was collected on a cellulose nitrate filter with a flow o

Pressurized intraperitoneal aerosol chemotherapy enhances cisplatin efficacy in colorectal cancer organoids.

Pressurized Intraperitoneal Aerosol Chemotherapy (PIPAC) is an emerging method that delivers chemotherapeutic agents as aerosols directly into the peritoneal cavity to overcome poor tissue penetration. Although early clinical outcomes are promising, challenges remain, such as variable patient responses and the lack of appropriate preclinical models. In this study, we investigated the efficacy of PIPAC combined with three cytotoxic agents (Cisplatin, Oxaliplatin, and Paclitaxel) using patient-derived colorectal cancer organoid models. Our results demonstrates that PIPAC, especially when combine

[PIPAC--Pressurized intraperitoneal aerosol chemotherapy. A novel treatment for peritoneal carcinomatosis].

Peritoneal carcinomatosis remains a diagnostic challenge with sparse treatment options. The effect of systemic chemotherapy remains limited inside the peritoneum due to low penetration and a relative resistance of peritoneal nodules. Heated IntraPeritoneal Chemotherapy (HIPEC) improves survival in selected patients but entails a high incidence of complications. Pressurized IntraPeritoneal Aerosol Chemotherapy (PIPAC) allows to disperse the active agents inside the peritoneal cavity by laparoscopy. Distribution and tissue penetration of chemotherapy by PIPAC are superior to HIPEC and systemic c
