PURPOSE OF REVIEW: In the past few years, procalcitonin has been proposed as a sensitive and specific inflammatory marker in various fields of medicine, especially in infectivology, where it has been used to discriminate between bacterial infections, viral infections and inflammation processes. Recently, different studies have emerged in the literature on the use of this marker to identify renal involvement in febrile urinary tract infections.
RECENT FINDINGS: Procalcitonin seems to be a valid biological marker, with an acceptable sensitivity and specificity, which predicts a renal involvemen

OBJECTIVE: To detail the biology and diagnostic usefulness of serum procalcitonin in critical illness.
DATA SOURCES: A review of articles published in peer reviewed journals from 1990 to 2001 and identified through a MEDLINE search on procalcitonin.
SUMMARY OF REVIEW: Procalcitonin (PCT) is a prohormone of calcitonin. Serum levels are elevated during sepsis and have been identified as a potential marker of infection in critical illness. However, its function and precise source of origin during sepsis remain unclear. The value of estimating serum PCT appears to be in the differentiation of in

Procalcitonin (PCT) is a new early indicator of developing systemic bacterial infection. As compared with other anti-inflammatory markers (cytokines, acute stage proteins), the attained plasma levels during extensive but uncomplicated surgery are by orders lower than maximal levels during sepsis. The authors assume that PCT will be used in surgical practice as an early indicator of developing systemic bacterial infection (in multiple injuries, after surgery) and for subsequent evaluation of the effectiveness of treatment of septic conditions.

In hantavirus infections levels of serum leukocytes or C-reactive protein are usually elevated to levels found in serious bacterial infections. However, procalcitonin in patients infected with hantavirus has not yet been discussed in the literature. A total of 29 adult patients with hantavirus infection, 30 with sepsis, and 19 with tick-borne encephalitis were included in this observational retrospective study. The median procalcitonin level in patients with hantavirus infection was 0.53 μg/L (range 0.09-11.71 μg/L), in the group with sepsis 4.33 μg/L (range 0.08-161.1 μg/L) and in patients wi

Procalcitonin (PCT), a protein of 116 amino-acids with molecular weight of 13 kDa, was discovered 25 years ago as a prohormone of calcitonin produced by C-cells of the thyroid gland and intracellularly cleaved by proteolytic enzymes into the active hormone. Circulating levels of PCT in healthy subjects are below detection limit. Since 1993 when its elevated level was found in patients with bacterial infection, PCT became an important protein in the detection and differential diagnostics of inflammatory states. The production of PCT during inflammation is linked with a bacterial endotoxin and w

Procalcitonin (PCT) has been increasingly evaluated as a biomarker in clinical situations where it is important to distinguish bacterial infection from viral and other inflammatory conditions. When added to other clinical and laboratory markers, PCT has comparably high sensitivity but markedly better specificity for detecting invasive bacterial infections in febrile young infants than decision algorithms not including PCT. For febrile infants, evidence shows PCT reduces unnecesary lumbar punctures (LPs), hospitalizations, and antibiotic use. PCT has also been documented to accurately distingui

OBJECTIVE: To evaluate the level and prognostic value of procalcitonin (PCT) in a West African out-patient cohort with pulmonary tuberculosis (PTB).
METHOD: Patients were clinically scored (TB score), grouped into severity classes (SCs) upon diagnosis and followed for 12 months. Patients were categorised by comparisons of severity class (SC I+II or SC III) and levels of PCT and C-reactive protein (CRP) at diagnosis. Fifty healthy volunteers from the study area were used as controls. The association with TB score was explored using Spearman's rank correlation test. Survival curves stratified a

Procalcitonin (PCT) levels are below the detection level in healthy subjects, while pre-procalcitonin mRNA is over expressed in human medullar thyroid carcinoma, in small cell lung tumor, and occasionally in other rare neuroendocrine tumors such as phaeochromocytoma. PCT is known as a sensitive and specific biomarker for bacterial sepsis, being produced by extra-thyroidal parenchymal tissues, mainly hepatocytes. The increase in plasma level correlates with the severity of infection and the magnitude and the time course of its increase can be strictly related to the patient's outcome, and to th

Procalcitonin is a specific marker of severe bacterial infections with systemic inflammation. Quantitative evaluation of serum procalcitonin was performed in 140 patients with Legionella pneumonia. Positive values (>0.5 ng/mL) were found in 57.1% of the subjects. The positive sample rate was higher in early sera (73.9% in the first week) and decreased progressively. Mean values and the positive sample rate (69.8%) were higher in sera collected in the first 2 weeks of disease. Procalcitonin was positive in 86.7% of legionellosis cases confirmed by culture and mean positive values were significa

BACKGROUND: Respiratory infections remain a leading cause of morbidity and mortality worldwide, encompassing a broad spectrum of conditions ranging from mild upper respiratory tract infections to severe pneumonia and sepsis. One of the major clinical challenges in their management is differentiating bacterial from viral infections, which has direct implications for appropriate antibiotic use. Overuse and misuse of antibiotics contribute significantly to antimicrobial resistance, making the identification of reliable biomarkers increasingly important. Procalcitonin has emerged as a promising bi

Due to its high accuracy for the diagnosis of bacterial infections, the inflammatory biomarker procalcitonin (PCT) is increasingly being used in patients with suspected infection. In patients with infections of the respiratory tract, it allows rapid rule out of bacterial etiology and facilitates decisions pertaining to antibiotic management. A growing body of evidence also supports PCT testing in patients with cardiovascular disorders including, but not limited to, those with shortness of breath, possible heart failure, suspected endocarditis, and acute coronary syndromes. In these clinical si

Procalcitonin is a 14-kDa protein encoded by the Calc-1 gene along with calcitonin and katacalcin. The function and regulation of this protein are quite different from those of the other gene products. Blood concentrations of procalcitonin are increased in systemic inflammation, especially when this is caused by bacterial infection. Studies of its behaviour in patients with bacterial sepsis have led to the proposal that it may be a useful marker of systemic bacterial infection, with greater specificity and sensitivity than acute phase proteins such as C-reactive protein.

PURPOSE: Bacterial translocation (BT) is common in colon cancer patients and may be associated with increased occurrence of septic complications as well as with adverse oncologic outcomes. The aim of the present study was to correlate the BT detectable through peritoneal lavage culture or identified by abnormal inflammatory parameters with the clinicopathologic parameters and the short-term prognosis in a prospective series of patients.
METHODS: Fifty-four consecutive patients with histologically proven colorectal cancer were included in this prospective study. White blood cells (WBC), erythr
