Incremental Propensity Score Interventions: A Primer for Pharmacoepidemiologists.

BACKGROUND: In observational pharmacoepidemiology, estimating average treatment effects (ATEs) is often challenging due to a lack of practical positivity. In highly selective clinical settings, certain patients almost always or never receive treatment, causing ATE estimators to rely on unstable extrapolation. Incremental propensity score interventions (IPSIs) offer a stochastic alternative by shifting each patient's probability of treatment, providing a more clinically realistic framework that circumvents positivity violations. METHODS: We illustrate the IPSI approach, including key identific

A comparison of 12 algorithms for matching on the propensity score.

Propensity-score matching is increasingly being used to reduce the confounding that can occur in observational studies examining the effects of treatments or interventions on outcomes. We used Monte Carlo simulations to examine the following algorithms for forming matched pairs of treated and untreated subjects: optimal matching, greedy nearest neighbor matching without replacement, and greedy nearest neighbor matching without replacement within specified caliper widths. For each of the latter two algorithms, we examined four different sub-algorithms defined by the order in which treated subje

Beyond Single-Score Matching: A Two-Dimensional Propensity Score Method for Mixed Covariate Types.

Propensity score matching (PSM) reduces confounding bias in observational studies, yet traditional single-score approaches assume linear covariate-treatment relationships. This assumption often fails when applied to complex electronic health record data that contain mixed categorical and continuous variables and nonlinear interactions. We introduce two-dimensional PSM (2D-PSM), which estimates separate propensity scores for categorical and numerical confounders and then performs matching in two-dimensional space under an elliptical caliper constraint. We compared 2D-PSM with traditional single

Propensity Score-Matched Comparison of Robotic and Laparoscopic Pancreaticoduodenectomy.

BACKGROUND: Robotic pancreaticoduodenectomy (RPD) has theoretical advantages over laparoscopic pancreaticoduodenectomy (LPD), offering enhanced dexterity with EndoWrist technology and improved ergonomics. This study aimed to compare surgical outcomes between RPD and LPD. METHODS: Patients undergoing minimally invasive pancreaticoduodenectomy, including 679 RPD and 94 LPD, were included for comparison before and after propensity score matching (PSM). RESULTS: After 1:1 PSM, 88 cases were analyzed in each group. The RPD group showed significantly shorter operation times (median: 7.5 vs. 11.0 h

Traumatic Injury in Pregnancy: A Propensity Score-Matched Analysis.

INTRODUCTION: Trauma represents the leading cause of nonobstetrical maternal death. How in-hospital outcomes of acutely injured pregnant patients (PP) compares to that of similarly aged nonpregnant control groups (CGs) has not been described. We hypothesized that PPs suffering acute traumatic injuries would have worse outcomes compared to a matched CG. MATERIALS AND METHODS: The American College of Surgeons Trauma Quality Improvement Program (TQIP) was used to identify traumatically injured females between 2017 and 2019. Propensity score matching on age, race, injury severity score , and type

Applying Propensity Score Methods in Clinical Research in Neurology.

Propensity score-based analysis is increasingly being used in observational studies to estimate the effects of treatments, interventions, and exposures. We introduce the concept of the propensity score and how it can be used in observational research. We describe 4 different ways of using the propensity score: matching on the propensity score, inverse probability of treatment weighting using the propensity score, stratification on the propensity score, and covariate adjustment on the propensity score (with a focus on the first 2). We provide recommendations for the use and reporting of propens

Cigarette Smoking and Pterygium: A Propensity Score Matching Analysis.

PURPOSE: To assess the association between smoking and pterygium among elderly adults by applying a propensity score matching (PSM) approach and to examine a potential dose-response relationship. METHODS: The study was designed as a community-based cross-sectional study based on a Chinese cohort aged 60 years or older in China. Anterior segment examination was performed without pupil dilation using a slitlamp. Pterygium was defined as a raised fleshy triangular fibrovascular tissue growth of the conjunctiva encroaching onto the clear cornea. Information regarding smoking history was collected

Preemptive kidney transplantation: a propensity score matched cohort study.

BACKGROUND: The reasons for improved outcomes associated with preemptive kidney transplantation (PKT) are incompletely understood, and post-transplant complications have been scarcely investigated. METHODS: We evaluated the outcomes of PKT in both unmatched (n = 1060) and propensity score matched cohorts (n = 186) of adults who underwent living kidney transplant between 2000 and 2014. Outcomes were estimated glomerular filtration rate (eGFR), biopsy-proven rejection, cytomegalovirus (CMV) infection, post-transplant diabetes mellitus (PTDM), cardiovascular disease (CVD), graft failure (non-cen

Propensity Score Matching Analysis for Alcohol-Related Liver Disease.

OBJECTIVES: This study aims to investigate the impact of comorbidity with chronic hepatitis B (CHB) on the survival rates and incidence of liver cancer in patients with alcohol-related liver disease (ARLD). METHODS: Patients with ARLD and those with ARLD co-morbid with CHB were included in this study and designated as the ARLD group and the ARLD + HBV group, respectively. Propensity score matching (PSM) was then employed to compare survival rates and liver cancer development between these two groups. RESULTS: Among the 404 patients, 254 were in the ARLD group and 150 in the ARLD + HBV group.

Comparing the high-dimensional propensity score for use with administrative data with propensity scores derived from high-quality clinical data.

Administrative healthcare databases are increasingly being used for research purposes. When used to estimate the effects of treatments and interventions, an important limitation of these databases is the lack of information on important confounding variables. The high-dimensional propensity score (hdPS) is an algorithm that generates a large number of empirically-derived covariates using administrative healthcare databases. The hdPS has been described as enabling adjustment by proxy, in which a large number of empirically-derived covariates may serve as proxies for unmeasured confounding varia

Postoperative pain score and infection risk in lung cancer resection: a propensity score-matching study.

OBJECTIVE: To explore the relationship between postoperative pain score and infection after lung cancer resection using propensity score matching. METHODS: A retrospective analysis was conducted on 206 cases of malignant lung tumors that underwent surgical resection between September 2022 and October 2025. Patients were divided into a low numerical rating scale (NRS) group (NRS ≤ 3, n = 134) and a high NRS group (NRS > 3, n = 72) based on their postoperative pain scores. A 1:1 propensity score matching was used to balance baseline characteristics, and postoperative pulmonary infection rates w

Prognostic and recurrence factors after endoscopic injection sclerotherapy for esophageal varices: Multivariate analysis with the propensity score matching: Multivariate analysis with propensity score matching.

BACKGROUND: Peri-esophageal collateral veins have been reported to be associated with the recurrence of esophageal varices (EVs). In this study, we retrospectively analyzed whether endoscopic ultrasonography (EUS) findings obtained just before endoscopic injection sclerotherapy (EIS) are associated with the success rate of intravariceal injection, recurrence rate of EVs, and overall survival rate (OS) of patients. Furthermore, we investigated the independent predictors associated with these factors by multivariate analysis. METHODS: A total of 91 patients with risky EVs treated by EIS were an

Robotic versus laparoscopic adrenalectomy: a propensity-score matched retrospective study.

INTRODUCTION: Laparoscopic adrenalectomy is the standard minimally invasive approach for adrenal tumors, and robotic-assisted surgery has increasingly been adopted as an alternative. Comparative studies have not consistently demonstrated superiority of either technique, and most are limited by confounding by indication, as robotic programs are introduced after laparoscopic practice is already established. We compared perioperative outcomes between robotic and laparoscopic adrenalectomy at a single center using propensity-score matching to reduce this confounding. METHODS: This retrospective,

Propensity score analysis for health care disparities: a deweighting approach.

BACKGROUND: Propensity score weighting is a useful tool to make causal or unconfounded comparisons between groups. According to the definition by the Institute of Medicine (IOM), estimates of health care disparities should be adjusted for health-status factors but not for socioeconomic status (SES) variables. There have been attempts to use propensity score weighting to generate estimates that are concordant with IOM's definition. However, the existing propensity score methods do not preserve SES distributions in minority and majority groups unless SES variables are independent of health statu

Early childhood hospitalization and problematic behaviors: A propensity score analysis.

Existing research suggests that children who experience poverty and hospitalization in early childhood are at risk of developing behavior problems. We examined whether the association between early childhood hospitalization and children's internalizing and externalizing behaviors were moderated by family poverty status and child sex. Participants included 224 children from the National Institute of Child Health and Human Development Study of Early Child Care and Youth Development. There was no direct association between hospitalization and problematic behaviors. Poverty status during early chi

Spouse bereavement and brain pathologies: A propensity score matching study.

AIM: Spouse bereavement is one of life's greatest stresses and has been suggested to trigger or accelerate cognitive decline and dementia. However, little information is available about the potential brain pathologies underlying the association between spouse bereavement and cognitive decline. We aimed to investigate that lifetime spouse bereavement is associated with in vivo human brain pathologies underlying cognitive decline. METHODS: A total of 319 ever-married older adults between the ages of 61 and 90 years underwent comprehensive clinical assessments and multimodal brain imaging includ

Preventing epilepsy after traumatic brain injury: A propensity score analysis.

BACKGROUND: Due to the potential consequences of post-traumatic epilepsy (PTE) exacerbating secondary injury following traumatic brain injury (TBI), the use of antiepileptic drugs (AEDs) is an accepted option for seizure prophylaxis. However, there is only a paucity of data that can be found regarding outcomes surrounding the use of AEDs. The purpose of this retrospective study is to evaluate whether the prophylactic administration of AEDs significantly decreased the incidence of PTE, when considering the severity of TBI. METHODS: All trauma patients who had been newly diagnosed with TBI from
