Pseudotumor cerebri.

PURPOSE OF REVIEW: To discuss the current standard in diagnosis and treatment of pseudotumor cerebri (PTC), the syndrome of increased intracranial pressure without a brain tumor. Since PTC is a diagnosis of exclusion, the differential diagnostic work-up is of special importance. RECENT FINDINGS: Modern imaging techniques have facilitated the differential diagnosis of PTC. Various therapeutic options are available to prevent irreversible visual loss. New treatment modalities are being explored, but the standard therapy has undergone little change in the past years. SUMMARY: PTC, a potentially

Pseudotumor cerebri.

Pseudotumor cerebri is a clinical syndrome in which signs and, sometimes, symptoms of raised intracranial pressure are present but in which mental and neurological function are unaffected. Therefore, the diagnosis is reached after mass and other structural causes of raised pressure have been excluded. Many causes of pseudotumor have been suggested, not all of them well documented. Pathogenesis, however, includes cerebral edema, increased cerebral blood volume, and decreased cerebro spinal fluid (CSF) absorption. Except for the risk of vision in a minority of cases, the prognosis is excellent.

Pseudotumor cerebri.

Probable cases of pseudotumor cerebri were described more than a hundred years ago. A great deal of controversy existed regarding proper terminology and disease characterization. The clinical entity was doubtful before the routine use of ventriculography in the 1930s. Throughout history, many terms have been used, including serous meningitis, pseudotumor cerebri, otitic hydrocephalus, angioneurotic hydrocephalus, toxic hydrocephalus, meningeal hypertension, hypertensive meningeal hydrops, pseudoabscess, intracranial hypertension of unknown cause, papilledema of indeterminate etiology, intracra

[Pseudotumor cerebri syndrome].

Pseudotumor cerebri is a non-tumor related idiopathic or secondary intracranial hypertension. It is typically associated with papilledema and headache and patients are typically female, young and obese. Diagnosis is established by neuroimaging by magnetic resonance imaging (MRI), cerebral MR venography (MRV) and lumbar puncture. Papilledema is assessed by ophthalmoscopy, ultrasound and optical coherence tomography. Ophthalmologists monitor visual function by measuring visual acuity and visual field testing. Therapy of patients not facing immediate visual loss encompasses weight reduction and a

Pediatric pseudotumor cerebri.

PURPOSE OF REVIEW: Recent findings in the literature regarding the epidemiology, diagnosis, and treatment of pediatric pseudotumor cerebri (PTC) are described. In the past, children with PTC have been treated like adult cases; however, the literature highlights important differences between children and adults with PTC. RECENT FINDINGS: Revised diagnostic criteria define an opening cerebrospinal fluid pressure greater than 28 cm water as elevated in the pediatric population. Obesity is an important risk factor for primary and secondary PTC in post-pubertal children. Magnetic resonance imaging

Pseudotumor Cerebri with Blindness.

Pseudotumor cerebri, or idiopathic intracranial hypertension (IIH), is a syndrome of elevated intracranial pressure (ICP) of unknown etiology that occurs predominantly in obese women of childbearing age. Pseudotumor cerebri literally means "false brain tumor". It is a "diagnosis of exclusion" therefore a complete work-up to rule out life-threatening causes for increased ICP must be performed through a comprehensive history, complete physical examination, diagnostic imaging, and cerebrospinal fluid (CSF) analysis before the diagnosis can be made. The authors present the case of a young woman wi

Doxycycline-induced pseudotumor cerebri.

Tetracyclines are a commonly prescribed medication for the treatment of acne vulgaris that are associated with pseudotumor cerebri (PTC). With doxycycline specifically, however, the incidence of PTC is very rare. A patient was using oral doxycycline and topical retinoids for acne, and within two months she developed PTC. This case illustrates that despite the rarity of doxycycline-induced PTC, patients and physicians should be aware of this possibility. Furthermore, in the setting of new-onset headaches or visual changes, early ophthalmologic examination for papilledema is recommended for earl

Drug-induced pseudotumor cerebri.

Pseudotumor Cerebri (PTC) is an uncommon disorder whose etiology is largely unknown, although its association with steroid withdrawal, hypervitaminosis A, and the use of the tetracycline group of drugs has been well documented. We report here a case in which a patient on chronic divalproex therapy for a seizure disorder developed PTC. Changing his antiepileptic medication from divalproate to topiramate effected a remission of PTC symptoms while maintaining his seizure-free status. It is recommended that physicians treating epilepsy, vascular headaches, or mood disorders with divalproate consid

The Pseudotumor Cerebri Syndrome.

Pseudotumor cerebri syndrome is a syndrome of increased cerebrospinal fluid pressure without ventriculomegaly, mass lesion, or meningeal abnormality. It is either primary (idiopathic intracranial hypertension, IIH) or secondary. A secondary cause is unlikely when adhering to the diagnostic criteria. Permanent visual loss occurs if undetected or untreated, and the associated headaches may be debilitating. Fulminant disease may result in blindness despite aggressive treatment. This study addresses the diagnosis and management of IIH including new insights into the pathobiology of IIH, updates in

Pediatric Pseudotumor Cerebri Syndrome.

Idiopathic intracranial hypertension, otherwise known as primary pseudotumor cerebri syndrome (PTCS), most frequently occurs in obese women of childbearing age. However, children may be affected as well. This review will address recent findings regarding demographics, diagnosis, and treatment of pediatric PTCS. Prepubertal children with primary PTCS have an equal sex distribution and less frequent obesity compared with adult patients. However, female gender and obesity are risk factors for primary PTCS in postpubertal children. Compared with adults, children with PTCS more frequently present w

[Epidemiology of pseudotumor cerebri].

OBJECTIVE: To determine the epidemiology of pseudotumor cerebri (PTC) as seen in patients attending our ophthalmology department. METHODS: The medical records of patients diagnosed with PTC in our hospital during a ten-year period (1994 to 2004) were obtained, with only those patients with PTC diagnosed in our ophthalmology department being considered. The following data were reviewed: sex, age at diagnosis, body mass index (BMI), opening cerebrospinal fluid (CSF) pressure, and the results of CT and MRI. RESULTS: Twenty-nine patients were diagnosed with PTC during this time period. Twenty-si

Idiopathic intracranial hypertension: pseudotumor cerebri.

BACKGROUND: Idiopathic intracranial hypertension (IIH), as defined by the modified Dandy criteria, is a diagnosis of exclusion. It is important to rule out other sight- and life-threatening causes of optic disk edema. IIH primarily affects obese women of childbearing age. CONCLUSIONS: Permanent visual loss is the primary morbidity associated with IIH. Quantitative perimetry is key in management decision-making to prevent blindness. Treatment consists of weight loss, medication, and surgery.

[Pseudotumor cerebri in minocyline treatment].

Pseudotumor cerebri or benign intracranial hypertension is a syndrome of raised intracranial pressure without obvious explanation. Most patients are obese women at childbearing age. Symptoms and signs usually include headache, nausea, vomiting, edema of the papilla, visual obscurations and rarely palsy of the nervus abducens. The prognosis is generally good, but progressive visual loss and eventual blindness are major risks. We report the case of a 21-year-old non-obese young woman who developed pseudotumor cerebri while taking minocycline for acne therapy. Identical symptoms occurred upon ina

Lumboperitoneal shunting for pseudotumor cerebri.

To clarify the appropriate role of lumboperitoneal (LP) shunting in the surgical management of pseudotumor cerebri (PTC), we retrospectively analyzed the clinical data from 30 patients who underwent this procedure. We found LP shunting to be an effective means of acutely lowering intracranial pressure. Symptoms of increased intracranial pressure improved in 82% of patients. Among 14 eyes with impaired visual acuity, 10 (71%) improved by at least two lines. Worsening of vision occurred in only one eye. Of 28 eyes with abnormal Goldmann perimetry, 18 (64%) improved and none worsened. The inciden

Pediatric pseudotumor cerebri: descriptive epidemiology.

OBJECTIVE: To determine the epidemiological profile for pediatric pseudotumor cerebri. METHODS: A retrospective case series was identified from a geographically limited region of 205,765 children aged 2-15 years at the only tertiary care pediatric hospital with all pediatric neurologists and ophthalmologists. Health records identified 35 children with ICD9 code 348.2 (benign intracranial hypertension) presenting between April 1, 1979 and March 31, 1994. After chart review, 29 cases were identified which satisfied modified Dandy's diagnostic criteria for pseudotumor cerebri. RESULTS: The 29 c

[Pseudotumor cerebri without optic papilledema].

Four cases of benign intracranial hypertension in two nonobese women (23 and 35 yr. old) and two nonobese men (both 47 yr. old) are presented. The most remarkable finding of the clinical investigation of these cases was the absence of papilledema. Our finding is consistent with other reports, and suggest that the presence of papilledema should not be considered critical for the diagnosis of benign intracranial hypertension. Additionally, the authors present a hypothesis for the absence of papilledema in cases otherwise typical of benign intracranial hypertension.

Otologic manifestations of pseudotumor cerebri.

Otologic manifestations of pseudotumor cerebri heightened awareness of pseudotumor cerebri and its varied subtle presentations will allow for a more expedient diagnosis by the otolaryngologist. Pseudotumor cerebri is defined as increased intracranial pressure and papilledema without a mass lesion or obstruction of the ventricular system. Presenting symptoms most commonly are headache and visual dysfunction. However, patients may present with associated symptoms of tinnitus, dizziness and hearing loss. These patients may be seen first by the otolaryngologist. Fourteen patients with a diagnosis
