Systematic scoping review of school-based psychological first aid programs.

INTRODUCTION: With a prevalence of around 20%, mental illness in young people has become a major global health issue. A growing number of school-based psychological first aid programs have lately been developed to empower teachers and students to provide acute support to peers with mental health problems. This systematic scoping review aims to provide an overview of the different psychological first aid programs available in the school context. METHODS: A systematic search for school-based psychological first aid programs was conducted in various databases using a predefined set of search ter

HSN-helping in mental distress: a new emotion focused psychological first aid program.

Psychological first aid programs aim to train lay people to help others who are experiencing a mental health crisis or developing mental health problems. The current article introduces the new German psychological first aid training program HSN. The acronym HSN stands for "Helfen in Seelischer Not" (helping in mental distress) but also for the action chain "hear, speak, network." The HSN program differs from other well-examined mental health first aid programs by (1) focusing primarily on emotions instead of disorders, (2) using a blended approach, combining face-to-face and online elements ri

Designing, Implementing, and Evaluating an Adapted Stress First Aid and Psychological First Aid Model in a Hospital Setting: Results from a Multi-year Study.

Healthcare workers are exposed to many stressors including long hours, intense physical and emotional labor, and consistent exposure to suffering and death. Together these stressors can culminate into more chronic and severe stress responses when not appropriately addressed. To help mitigate the impact of these stressors, peer-administered post-event psychosocial support programs have been developed, such as Psychological First Aid and Stress First Aid. Unfortunately, few resources exist to help those interested in designing, implementing, and evaluating such a program within their organizatio

Indiscriminate trauma preventions may discriminate: A randomised controlled trial of group psychological first aid.

Psychological First Aid (PFA) has been widely promoted as a universal intervention following traumatic incidents. However, early interventions once considered preventative were later found harmful, underscoring the importance of testing before widespread use. This randomised controlled trial examined the efficacy of PFA compared to natural recovery in reducing psychological distress using a validated stress-video paradigm. Based on recent evidence, it was hypothesised that PFA would alleviate short-term anxiety but show no long-term benefits. Eighty-four participants were randomised to either

Effectiveness of psychological first aid in infectious disease pandemics: An overview of systematic reviews.

There is insufficient research on the usefulness of psychological interventions, such as psychological first aid (PFA), during outbreaks. We searched for and critically appraised systematic reviews that examined the effectiveness of PFA during infectious disease outbreaks, such as the novel coronavirus disease (COVID-19). Systematic reviews that examined the efficacy of PFA in the severe acute respiratory syndrome, Middle East respiratory syndrome coronavirus, Ebola virus disease, and COVID-19 outbreaks were searched through PubMed on February 19, 2021. The three included systematic reviews we

The use of psychological first aid (PFA) training among nurses to enhance population resiliency.

Disaster mental health research has found that psychological casualties from a given disaster can be expected to far outnumber physical casualties. Amidst a shortage of mental health professionals and against the backdrop of natural disasters, continued terrorism, and pandemic influenza, there is a striking need to expand and operationalize available human resources to enhance the psychological resiliency of those affected. Through the utilization of psychological first aid (PFA) as an early crisis intervention tool, and by virtue of their occupation and experience, nurses are particularly wel

HUGS@Home - An initial evaluation of a psychological first aid programme for families and friends of first responders.

BACKGROUND: First responders such as paramedics and firefighters increasingly rely on their family and friends for psychosocial support due to workplace barriers such as culture and stigma. Despite many benefits, such support is often perceived as placing a burden on family and friends. The ability to provide Psychological First Aid (PFA) has not yet been explored in the context of family members and friends providing support to first responders. In this study, we explored the impacts of PFA on first responders and family members/friends. METHODS: This pre-post study design was informed by a

Developing, piloting, and evaluating a psychological first aid training for medical students in South Africa.

BACKGROUND: Common mental disorders are highly prevalent among medical students. One way to ameliorate risk for mental disorders in this group is to improve medical students’ psychological literacy and provide psychoeducation to promote appropriate help seeking, which could potentially be achieved through changes to the formal curriculum. OBJECTIVES: Our aim was to develop, pilot and evaluate a context-sensitive and culturally appropriate Psychological First Aid (PFA) training programme for medical students at a South African University which was delivered as a course module. Methods: 285 stud

Psychological First Aid for Wilderness Trauma: Interventions for Expedition or Search and Rescue Team Members.

When exposed to actual or threatened death or serious injury in austere settings, expedition members are at risk of acute stress reactions, as are search and rescue members involved with extricating the patient. Acute stress reactions are a normal response to significant trauma and commonly resolve on their own. If they do not, they can lead to post-traumatic stress disorder (PTSD), a set of persistent symptoms that cause significant effects on the person's life. Medication has a limited preventive role in the field for treatment of stress partly because so few are trained to administer it. Co

First Psychological Aid for Families During War: Integrating the Narrative Approach With Emotionally Focused Therapy (EFT).

The article describes a first psychological support project for families living in the seam zone between Israel and areas controlled by the Palestinian Authority. Since October 7th, the proximity to the border and potential of terrorist infiltration into communities have generated heightened anxiety among residents, alongside concern for family members enlisted to reserve duty or on mandatory service. Therefore, the Family Therapy Unit of Menashe Regional Council initiated a psychological support program designed to strengthen families' resources and enhance their ability to cope with the ongo

Pre-field conceptualizations of psychological first aid among disaster and emergency management students: A reflexive thematic analysis.

Psychological First Aid is recognized as a fundamental early psychosocial intervention in disaster response. This study explored how undergraduate students enrolled in a Disaster and Emergency Management program conceptualize Psychological First Aid before field experience, focusing on their anticipated helping approaches, emotional and cognitive orientations, and emerging understandings of professional roles and identities. A qualitative study using reflexive thematic analysis was conducted. Data in the form of open-ended questionnaires were collected from 32 students. Following reflexive the

The Effectiveness and Implementation of Psychological First Aid as a Therapeutic Intervention After Trauma: An Integrative Review.

Psychological First Aid (PFA) is known to be an initial early intervention following traumatic exposure, yet little is known about its optimal implementation and effectiveness. This review aims to examine the evidence for the effectiveness of PFA interventions and how PFA interventions have been designed, implemented, and experienced. MEDLINE, the Cumulative Index to Nursing and Allied Health Literature (CINAHL), Cochrane Library, PsychINFO, Embase, Web of Science, PILOTS, and China National Knowledge Infrastructure (in Chinese) databases were searched. Accessible Psychological First Aid (PFA) training for disaster preparedness: effects on coping strategies among deaf adults in India.

Deaf individuals are often disproportionately vulnerable during disasters due to communication barriers, limited access to early warning systems, and reduced inclusion in emergency response mechanisms. Strengthening psychosocial preparedness and coping capacities within Deaf communities is therefore critical for inclusive disaster risk reduction. Psychological First Aid (PFA) is a widely used approach in disaster and emergency contexts that aims to promote safety, emotional stabilization, and adaptive coping. However, limited empirical research has examined the effectiveness of accessible PFA

Implementation and evaluation of a psychological first aid program to manage post-traumatic stress injuries among Canadian police officers.

BACKGROUND: Police officers are exposed to potentially psychological traumatic events and are at high risk of developing post-traumatic stress injuries (PTSI). Development and wide implementation of best practices for managing PTSI are needed. The psychological first aid (PFA) framework encompasses trauma-informed knowledge to guide the development of best practices. Based on the framework and on a pilot performed among Canadian police officers, we propose a PFA program including: 1) PTSI awareness e-learning; 2) PFA training provided by local trainers; and, 3) peer-to-peer PFA intervention to

Case Report: The SIX Cs model integrating empathy: a structured cognitive framework for psychological first aid during acute threat.

This article presents a theory-driven case study examining the integration of empathy within the SIX Cs Model of psychological first aid (PFA) during a real-time, life-threatening event involving two children trapped in a terrorist attack in Israel. Empathy, encompassing cognitive, emotional, and compassionate components, plays a vital role in psychological support; however, its application during acute stress requires a structured approach that preserves cognitive functioning. The SIX Cs Model targets core features of the acute stress response (ASR), including confusion, perceptual narrowing,

Towards scalable psychological first aid training: an autoethnographic exploration of the potential of large language models in simulation-based learning.

Background: Psychological First Aid (PFA) is a widely used approach to provide psychosocial support to individuals in the aftermath of a stressful event. PFA training requires a costly, structured approach, including repetitive simulation-based training with actors. The rapid emergence of large language models (LLMs) could provide easy access to PFA simulation training, enabling trainees to practice conversations repeatedly and flexibly, learn from mistakes, and build emotional resilience. In addition, this would allow for training in geographically isolated regions where PFA training may be m

Adapting and Co-Producing a Psychological First Aid Intervention for Care Home Staff: A Person-Based Approach to Enhance Workforce Resilience.

Care home staff are routinely exposed to stressful and traumatic events, increasing risks of psychological distress, burnout, and reduced workforce resilience. Psychological First Aid (PFA), recommended by the World Health Organization, provides an evidence-based framework for delivering immediate emotional and practical support; however, its adaptation for care home contexts is limited. This study aimed to co-produce and adapt an existing PFA training resource for care home staff using a person-based approach (PBA) to enhance contextual relevance, acceptability, and feasibility.
