We describe the case of an elderly woman with elderly-onset rheumatoid arthritis, where the use of 4 mg/kg/day of hydroxychloroquine (HCQ) was followed by the onset of psychomotor agitation with marked physical and verbal violence towards her partner, including throwing objects at her partner. No disturbance in sleep and no anxiety, nervousness, or irritability had emerged before the onset of her psychomotor agitation. The disappearance of agitation following targeted pharmacologic intervention and HCQ interruption, its re-onset after reintroduction of the drug, and the high score (9) of Naran

Psychomotor agitation (PMA) is a prominent clinical issue frequently observed in various psychiatric and neurological conditions, including schizophrenia, bipolar disorder, Parkinson disease, dementia and substance use disorder. Characterized by motor restlessness, anxiety and irritability, PMA can rapidly escalate into aggression and violence, necessitating prompt intervention to ensure patient and caregiver safety. The prevalence of PMA in psychiatric emergency settings ranges from 4.3% to 10%, imposing a substantial burden on healthcare systems. Despite the critical nature of PMA, there is

Psychomotor agitation is a syndrome characterized by abnormal psychic and motor activation, with a concrete possibility of escalation to aggression or violence. It represents a frequent reason for attending emergency departments and can stem from various organic conditions and mental disorders. In emergency settings, prompt identification of the possible underlying cause and optimal management are crucial to ensure the safety of both patients and healthcare providers. However, current management strategies are based on evidence focused on too specific clinical contexts or aspects of patient ca

PURPOSE: Bilateral thalamic ischemia can, paradoxically, manifest itself with various non-sensory clinical symptoms, thereby complicating diagnosis.
CASE DESCRIPTION: A 59-year-old woman was admitted to an Emergency Department about and hour and a half after she was found with altered mental status. Her initial symptoms were psychomotor agitation and slight confusion. No evident focal neurological deficit was observed at that time and computed tomography was negative. After acute drug intoxication was excluded, a transfer to a psychiatric unit was considered. Simultaneously, short stenosis of

Psychomotor agitation in somatic units is a medical and psychiatric emergency requiring rapid intervention by the team in charge of the patient and the liaison psychiatrist. Verbal de-escalation is attempted throughout the treatment. Securing the surroundings is needed to protect both the patient and the team. The etiology of the agitation, somatic and/or psychiatric, should be investigated to allow the administration of an adequate treatment. Antipsychotics and benzodiazepines are often recommended. Physical restraint measures can be applied as a last resort if necessary. The continuation of

Psychomotor agitation (PmA), a common yet understudied symptom in Major Depressive Disorder (MDD), is characterized by heightened motor activity such as restlessness and fidgeting. PmA, like other symptoms of MDD, may persist into remission. This study examined the role of PmA in remitted MDD by using a novel smartphone based physiological measure of tremor. Participants (N = 157) consisted of healthy controls (N = 76) and those with remitted MDD (N = 81, to reduce the impact of current MDD symptoms on PmA), and tremor was assessed under the manipulation of position (palm-up, arm extended) and

The early and correct assessment of psychomotor agitation (PMA) is essential to ensure prompt intervention by healthcare professionals to improve the patient's condition, protect healthcare staff, and facilitate future management. Proper training for recognizing and managing agitation in all care settings is desirable to improve patient outcomes. The best approach is one that is ethical, non-invasive, and respectful of the patient's dignity. When deemed necessary, pharmacological interventions must be administered rapidly and avoid producing an excessive state of sedation, except in cases of s

In bipolar depression, psychomotor agitation is relatively common and often is associated with other noneuphoric hypomanic symptoms and suicidal ideation. Our goal in this retrospective study was to ascertain the co-occurrence of agitation, bipolar features, and suicidal ideation in unipolar disorder. We retrospectively evaluated 314 inpatients with DSM-IV major depressive disorder (MDD) and no other Axis I diagnosis with the National Institutes of Mental Health (NIMH) Life Chart Method and the Operational Criteria for Psychotic Illness (OPCRIT) checklist to ascertain their symptom profiles ac

Mean duration and frequency of pauses within speech, pauses between speakers, and vocal activity were measured in 28 endogenous depressive during a psychiatric interview. A factor analysis on these variables revealed two factors accounting for 50% and 24% of the variance, respectively. Factor 1 could be interpreted as a pause factor. The mean durations of both types of pauses had high negative loadings, whereas the frequency of vocal activity, simultaneous speech, and pauses, and the mean duration of vocal activity had high positive loadings on this factor. The degree of retardation (a measure

BACKGROUND: Kraepelin's partial interpretation of agitated depression as a mixed state of "manic-depressive insanity" (including the current concept of bipolar disorder) has recently been the focus of much research. This paper tested whether, how, and to what extent both psychomotor symptoms, agitation and retardation in depression are related to bipolarity and anxiety.
METHOD: The prospective Zurich Study assessed psychiatric and somatic syndromes in a community sample of young adults (N = 591) (aged 20 at first interview) by six interviews over 20 years (1979-1999). Psychomotor symptoms of

OBJECTIVES: We aimed to investigate the characteristics of adolescents with Bipolar disorder-I with irritability and agitation (Mania+IA) compared to those without irritability and agitation (Mania-IA) in a multi-center representative sample.
METHODS: Data of 145 patients from three tertiary-care inpatient units between 2016 and 2021 were obtained. Psychomotor agitation was defined as a score of ≥3 on the YMRS "Increased Motor Activity--Energy" item, irritability as a score of ≥4 on the YMRS 'irritability' item, and severity anchors of speech and thought disturbance on the YMRS '6 and 7' item

The frameworks of impulsivity and temperamental irritability of people with intellectual disabilities, presenting disabilities both for organic cause, e.g. postpartum, and for congenital mental retardation, may often present impulse control disorders, irritability and psychomotor restlessness, busyness and insomnia, from likely non-epileptic neuronal dysfunction. The patients with intellectual disabilities presenting mood disorders need to be treated with a therapy similar to that of other patients with mood disorders in the bipolar spectrum (namely predominantly with mood regulators), even if

BACKGROUND AND OBJECTIVES: Psychomotor restlessness and agitation (PMA) is a putatively important, yet understudied, psychopathologic correlate of smoking. The scant smoking research on PMA previously conducted has been narrow in scope and conducted among psychiatric patients. To examine the generalizability and relevance of PMA to smoking, this cross-sectional study investigated associations between PMA and a variety of smoking processes in a community sample.
METHODS: Participants in this study were non-treatment-seeking smokers (N = 254, ≥10 cig/day, M age = 44 years) from the community wi
