[Simple nebulization of racemic epinephrine in the treatment of acute laryngitis (croup) (author's transl)].

Five infants with acute laryngitis (croup) were treated with nebulized racemic epinephrine. In contrast to previous reports the mist was not delivered by intermittent positive pressure ventilation via a respirator but by simple inhalation via a jet-type nebulizer and face mask. Immediate though only transitory relief of symptoms without any significant alteration of heart rate was achieved by low doses of the diluted drug. Increase in dosage resulted in a moderate elevation of heart rate without improvement in therapeutic action. Inhalations had to be repeated frequently; there were no signifi

Local facial cutaneous vasoconstriction: an unusual complication of inhaled racemic epinephrine in a neonate.

A term infant treated for post-extubation stridor with nebulized racemic epinephrine developed localized facial blanching due to cutaneous absorption of the aerosolized vasoconstrictor. Local application of heat restored circulation to the afflicted area. This complication of a commonly used medication is not previously reported in the medical literature and has the potential for severe sequelae.

[Acute laryngitis in children].

Acute laryngitis is the most common form of upper airway obstruction in young children. Laryngeal obstruction requiring hospitalization and sometimes intubation may be due to viral infection or occasionally to allergic reaction. The natural course of the disease is impossible to predict; therefore, repeated clinical assessments are needed. Continuous worsening of dyspnea may suggest a diagnosis of bacterial tracheitis. High doses of corticosteroids combined with aerosolized racemic epinephrine can relieve the respiratory difficulties.

Variation in Organizational Clinical Practice Guidelines for Croup.

BACKGROUND: Croup is one of the most common respiratory complaints in pediatric emergency departments (EDs), yet little is known about clinical practice guidelines (CPGs) for this condition. OBJECTIVES: To describe variation in CPGs across US children's hospitals. METHODS: We describe the prevalence and features of CPGs among hospitals that submit data to the Pediatric Health Information System. Each hospital was contacted between January 10, 2022, and April 25, 2022, for their most recent croup CPG and any revisions. Characteristics reported were based on the most recent CPG revision. Croup.

Viral croup is the most common form of upper airway obstruction in children 6 months to 6 years of age. It typically presents in the late fall or early winter, is often preceded by an upper respiratory infection, and is characterized by a low-grade fever, barking cough, and inspiratory stridor. Diagnosis is made on clinical grounds with no specific confirmatory test. The differential diagnosis of croup, including epiglottitis and retropharyngeal abscess, must always be considered in evaluating children with inspiratory stridor. Three therapeutic modalities are available for the treatment of cr

[Accidental administration of 50 mg racemic adrenaline to a 2-year-old boy].

A two-year-old boy received by mistake 50 mg racemic adrenaline intravenously, corresponding to 1.8 mg kg-1 of L-adrenaline. Blood pressure increased to 160/105 mm Hg, heart rate to 160 beats min-1 and pulmonary oedema developed over the next two hours. He was treated with nitroprusside, nitroglycerine and digitoxin, and was intubated and ventilated. After three hours a hypotensive phase occurred, probably due to down-regulation of the beta- and alpha-adrenoceptors. This recessitated infusions of very high concentrations of catecholamines for 72 hours. Renal failure recessitated renal transpla

[Accidental administration of racemic adrenaline. Three life-threatening cases after intravenous injection in children].

The inhalation of racemic adrenalin is an important part of the treatment of inflammatory airway obstruction in children. In Norway during the last few years there have been several cases of adrenal solutions intended only for inhalation being accidentally administered as intravenous injections. The solution for inhalation contains an adrenalin concentration 110 times greater than the adrenalin intended for emergency use (0.1 mg/ml). The instant consequences of intravenous injections of inhalation adrenalin include arterial hypertension followed by hypotension, cardiac ischemia and cardiac ins

Two Case Reports of Life-Threatening Croup Caused by the SARS-CoV-2 Omicron BA.2 Variant in Pediatric Patients.

Croup is a common upper airway infection characterized by a barking cough, stridor, and hoarseness. It is usually caused by viral infection. A small number of croup caused by coronavirus disease 2019 (COVID-19) has been reported in children before the omicron variant surge. Previously reported cases indicated that croup caused by COVID-19 can be treated in the same manner as those with other viral causes. We describe two cases (9-month-old girl and 11-month-old boy) of previously healthy infants who presented with a barking cough and chest retraction and required endotracheal intubation and ca

Nebulized epinephrine for croup in children.

BACKGROUND: Croup is a common childhood illness characterized by barky cough, stridor, hoarseness and respiratory distress. Children with severe croup are at risk for intubation. Nebulized epinephrine (NE) may prevent intubation. OBJECTIVES: To evaluate the efficacy and safety of NE in children presenting to emergency department (ED) or admitted to hospital with croup. SEARCH STRATEGY: We searched CENTRAL (The Cochrane Library 2010, Issue 4), containing the Cochrane Acute Respiratory Infections Group's Specialized Register, MEDLINE (1966 to November Week 1, 2010), EMBASE (1980 to November 20

Adverse effects of racemic epinephrine in epiglottitis.

Acute epiglottitis, a potentially fatal disease, may be difficult to diagnose if the presentation is atypical. We present three such patients who were initially diagnosed as acute laryngotracheitis. They deteriorated rapidly after administration of racemic epinephrine via nebulizer. We caution that: diagnosis of acute epiglottitis should be considered in similar patients; and appropriate precautions must be taken prior to visualization of the airway.

Standardization of Clinician Outcomes for Croup Using a Clinical Pathway.

OBJECTIVE: Croup management varies significantly between hospitals and clinicians. We assessed the level of clinician variation with a clinical croup pathway in the pediatric emergency department (PED) and the impact of a pathway change on admission rates. METHODS: This was a retrospective cohort study of children with croup in a PED at an academic referral hospital between 2018 and 2022. Primary outcome measures were the proportion of admissions to the hospital and mean PED length of stay. The main exposures were the primary clinician and their use of racemic epinephrine. We used generalized

Nebulized racemic epinephrine for extubation of newborn infants.

BACKGROUND: Following a period of mechanical ventilation, post-extubation upper airway obstruction can occur in newborn infants, especially after prolonged, traumatic or multiple intubations. The subsequent increase in upper airway resistance may lead to respiratory insufficiency and failure of extubation. The vasoconstrictive properties of epinephrine, and its proven efficacy in the treatment of croup in infants, has led to the routine use of inhaled nebulised epinephrine immediately post-extubation in some neonatal units. It is also recommended for neonates with post-extubation tracheal obst

Thoracoabdominal asynchrony in acute upper airway obstruction in small children.

The assessment of the severity and response to therapy of acute upper airway obstruction (UAO) in small children relies on subjective parameters. Using a noncalibrated respiratory inductance plethysmograph (RIP), we quantitated the rib cage (RC) to abdominal (AB) asynchrony and the lag phase in chest wall expansion by the phase angle from the RC versus AB signal curve. Phase angles were obtained in 17 children aged 1 to 50 months with acute UAO and 30 normal control subjects. The phase angle in UAO (16 to 165 degrees; mean = 83.6 degrees) was significantly higher than in control subjects (3 to

Renal failure and other serious sequelae of epinephrine toxicity in neonates.

Epinephrine has numerous potent pharmacologic actions with protean manifestations. We have described a patient in whom inadvertent intra-aortic administration of a large dose of racemic epinephrine produced serious adverse effects, including hypertension, acidemia, tachycardia, and protracted but reversible renal failure. In view of the ubiquitous use of epinephrine in neonatal intensive care, we hope that this report heightens awareness of its more serious and potentially fatal toxic effects.

Therapy changes in the treatment of subglottic laryngitis: A 30-year experience in Croatia.

BACKGROUND: Croup (viral laryngotracheitis, also referred to in some settings as subglottic laryngitis) can cause acute upper-airway obstruction and requires prompt management to prevent respiratory compromise. This study evaluated long-term changes in reported croup management in Croatia. METHODS: Questionnaires were distributed to pediatricians and otolaryngologists in multiple Croatian hospitals in 1993, 2003, 2013, and 2023. A standardized clinical vignette of a 2-year-old child with mild-to-moderate croup was used for the core longitudinal comparisons. Categorical variables are reported

Recent advances in the treatment of bronchiolitis and laryngitis.

Progress has been made in the treatment for patients with croup and bronchiolitis in the past decade. By intervening with pharmacologic agents, a better outcome has been documented in children with these diseases. A lower probability of hospital admission means that fewer health care dollars need to be expended in this area. The present state of evidence substantiates the following. Bronchiolitis . Nebulized albuterol causes significant short-term improvement in clinical scores in bronchiolitic children, but there is no evidence that it reduces admission rates or decreases length of hospitaliz

Physical evaluation and the prevention of medical emergencies: vital signs.

It was assumed that dentists employ a complete system of physical evaluation for all new patients in their dental practices. Results of a survey of 1,588 dentists demonstrated that the use of a written medical history questionnaire was commonplace; however, recording of blood pressure and heart rate and rhythm on all new patients was quite limited. A greater percentage of dentists monitored blood pressure when there was a history of cardiovascular disease or high blood pressure. Monitoring of the heart rate and rhythm, even in patients with cardiovascular disease or high blood pressure, was se
