IgE antibody levels to Hymenoptera (honey bee, hornet, wasp or yellow jacket) venom or venom sac were measured in the serum of ninety-six patients with a history of immediate hypersensitivity reactions to these insect stings. The usefulness of the modified scratch test in predicting clinical allergy and its correlation with the RAST were studied in 83 atopic children. A total of 423 scratch tests were compared with the same number of RAST. Percentage agreements between the two tests for individual allergens ranged from 87.5% (milk) to 50% (cat epithelium). Radioallergosorbent tests for specific IgE antibodies to Schistosoma haematobium and S. mansoni were positive in 82 and 72%, respectively, of sera from 136 African schistosomiasis patients. 99% of sera from controls (5 free of parasitic disease, 75 infested with hookworm, Ascaris, Trichuris, and/or Onchocerca) were negative. When titres in indirect fluorescent antibody tests were set at the same level of specificity only 38% of patients' sera were positive. A cut-off at a higher concentration increased sensitivity to 65% but lowered specificity (10% controls positive).

Serum IgE concentrations and the presence of allergen-specific IgE were determined in a series of 23 children with atopic dermatitis. In this group 83% had significantly elevated serum levels of IgE, 91% had coexistent respiratory allergy, 70% had radioallergosorbent test (RAST) evidence of pollen hypersensitivity, and 43% gave a history and demonstrated a RAST score consistent with milk or egg hypersensitivity. In patients with eczema a significant proportion of the elevated serum IgE appears to be antigen specific.

The radioallergosorbent test (RAST) is a recently developed radioimmunological in-vitro method for determining IgE-specific serum antibodies in reaginic allergy. There was 63.4% overall correspondence between RAST and bronchial provocation test in 423 tests with a total of eight perennial allergens on adults with asthma. Mattress dusts from the beds of 51 asthmatic children with positive skin tests to house dust mite were assayed for Der p I, Fel d I and certain viable fungi. A technical modification was introduced by the manufacturer (Pharmacia) of a commercially available RAST, with the use of an anti-IgE antibody specific for the domain epsilon 2 of the IgE molecule.
