Interventional radiography and mortality risks in U.S. radiologic technologists.

With the exponential increase in minimally invasive fluoroscopically guided interventional radiologic procedures, concern has increased about the health effects on staff and patients of radiation exposure from these procedures. There has been no systematic epidemiologic investigation to quantify serious disease risks or mortality. To quantify all-cause, circulatory system disease and cancer mortality risks in U.S. radiologic technologists who work with interventional radiographic procedures, we evaluated mortality risks in a nationwide cohort of 88,766 U.S. radiologic technologists (77% female

[Diagnostic value of transthoracic ultrasound compared to chest radiography in the detection of a post-interventional pneumothorax].

PURPOSE: Chest radiography is standard for the diagnosis of a pneumothorax. However, also ultrasound of the chest has considerable value in the detection of a pneumothorax. A typical sonographic feature is the lack of the lung gliding sign. Aim of our trial was to evaluate the significance of ultrasound of the chest in the diagnosis of a pneumothorax supplemented by using power colour Doppler imaging. PATIENTS AND METHODS: Following transbronchial biopsy, patients received transthoracic ultrasound of the chest for evaluation of a pneumothorax. Immediately afterwards, a chest radiograph was pe

Caries development in a young population managed by a restrictive attitude to radiography and operative intervention: II. A study at the surface level.

OBJECTIVES: To longitudinally assess the distribution of fillings and carious lesions in permanent posterior tooth surfaces, caries states fillings were made, and survival time of proximal lesions in different states in a young population managed by minimal operative treatment and a restrictive attitude to radiography. METHODS: Bitewing radiographs from age 6-20 in 285 patients were analysed. Proximal and occlusal surfaces were coded for caries depth and fillings. Start and end dates for different caries states in proximal surfaces were assigned and survival times in these states calculated b

Population dose from medical exposure in Taiwan for 2008.

PURPOSE: The largest contribution to the population dose from man-made ionizing radiation sources is the medical exposure. Exposure to patients from medical examinations is of interest because it is a global indicator for the quality of radiology practice. Due to the different healthcare systems and the considerable variations in the equipment and manpower in radiology, the population dose from medical exposure varies by a large extent in different countries. This dose from different diagnostic procedures provides information that can be used to establish national reference levels. It is also

[Massive obstetric haemorrhage in a patient with placenta percreta].

We present a case of a 38 year-old patient with prenatal diagnosis of placenta praevia. When the elective caesarean began it was found a placenta accreta. In spite of an emergency hysterectomy, embolisation using interventional radiography was needed after a massive obstetric haemorrhage. The post-operative period progressed without incidents.

Radiographic evaluation of the neonate with congenital heart disease.

Remarkable advances in pediatric cardiology have been spurred by the explosion of technologies both in interventional and surgical techniques and the ability to manipulate the genome of experimental animals. After a brief discussion concerning the striking advances in the molecular understanding of congenital heart disease, this article focuses on clues to the diagnosis of congenital heart disease and on chest radiography and common, specific lesions of the neonate such as hypoplastic left heart, transposition of the great vessels, and severe tetralogy of Fallot. The impact of treatment protoc

Intracardiac migration of distal catheter-a rare complication of VP shunt insertion: case report and literature review.

Intracardiac migration is a rare complication of ventriculoperitoneal shunt insertion. Only 15 cases have been reported, 7 of which were paediatric cases, treated with techniques including interventional radiography, open thoracotomies and direct extraction through the initial shunt incision. The authors report the youngest case of intracardiac shunt migration complicated by significant coiling and knotting within the cardiac chambers and pulmonary vasculature. Migration likely began when the SVC was pierced during initial shunt placement and progressed due to negative intrathoracic pressure.

Variant arterial supply to the lesser curvature of the stomach and duodenum from double inferior phrenic arteries.

A rare case of the left accessory inferior phrenic artery (IPA) supplying the lesser curvature of the stomach and the right accessory IPA supplying the duodenum was observed during interventional radiography. To our knowledge, variation arteries supplying the stomach and duodenum have never been reported in English literature; herein, we report the first case of variant arterial supply to the lesser curvature of the stomach and duodenum from double IPAs.

Transthoracic US of the chest: clinical uses and applications.

Transthoracic ultrasound (US) of the chest is useful in the evaluation of a wide range of peripheral parenchymal, pleural, and chest wall diseases. Furthermore, it is increasingly used to guide interventional procedures of the chest and pleural space. The technique lends itself to bedside use in the intensive care unit, where suboptimal radiography may mask or mimic clinically significant abnormalities. The authors discuss the uses, techniques and applications of US of the chest. The sonographic appearances of pleural diseases (pleural effusion, pneumothorax, pleural mass, and mesothelioma), p

Popping the Balloon: A Giant Colonic Diverticulum Complicated by Bladder Neck Compression.

Giant colonic diverticulum, defined as a single diverticulum ≤ 4 cm, is rarely encountered. Due to the high incidence of complications related to the disease, obtaining the correct diagnosis early in the disease course is essential. Diagnosis is usually reached by conventional and cross-sectional abdominal radiography. Treatment decisions should be ideally made by a multidisciplinary discussion among surgeons, interventional radiologists, and the patient. The treatment of choice is the surgical management by open or laparoscopic approach.

[Risk of developing cataract related to fluoroscopy].

It is well known that ionising radiation can cause cataract. In the healthcare sector, fluoroscopic procedures are increasing, and exposure to low-energy X-rays can be substantial. Several cross-sectional studies in interventional cardiology have reported a increased prevalence of posterior subcapsular cataract. However, these studies have notable methodological limitations. There may also be a risk associated with work in radiography and nuclear medicine. Considering the overall body of evidence, this review finds that preventionreducing should be a priority.

Portable CT: assessing thoracic disease in the intensive care unit.

OBJECTIVE: The objective of this study was to assess the usefulness of a portable CT scanner to evaluate and treat thoracic disease in patients in the intensive care unit. MATERIALS AND METHODS: Fourteen patients who were being treated in the intensive care unit underwent 20 portable CT scans. Twice a CT scan was obtained to guide an interventional chest procedure. The remaining 18 scans were assessed for findings not evident on portable chest radiography and for findings that altered treatment. Image quality was judged in comparison with fixed CT scans. RESULTS: Unsuspected abnormalities, m

Imaging of Tendons of the Knee: Much More than just the Extensor Mechanism.

The tendons of the knee play a critical role in the motion and stability of the knee. Knee pain, a common clinical complaint, can frequently be attributed to pathology of these structures. While the cornerstones of the radiological evaluation of the knee are radiography and magnetic resonance imaging, ultrasound can also play an important diagnostic role and can be useful for guided interventional procedures. Understanding the utility of each modality in the assessment for tendon pathology and a familiarity with the normal and abnormal imaging appearance of these structures are crucial for acc

Benign Spontaneous Pneumomediastinum in a Late Preterm Newborn.

Spontaneous pneumomediastinum is defined as free air in the mediastinum without an explanation or known cause. Late preterm neonates are rarely affected; however, a known risk factor is aggressive resuscitative maneuvers. Moreover, spontaneous pneumomediastinum is a rare cause of neonatal respiratory distress diagnosed in the post-natal period using chest radiography. In contrast, preterm babies with respiratory distress syndrome are associated with pneumomediastinum. The condition is considered benign, and a conservative, non-interventional management approach is widely accepted, with complet

An 80-Year-Old Woman With Dyspnea and a Lung Mass.

An 84-year-old woman presented to the interventional pulmonary clinic for evaluation of a right middle lobe lung mass. Her medical history was notable for atrial fibrillation on rivaroxaban and recurrent bilateral breast cancer that had required multiple lumpectomies, radiation, and chemotherapy. She is a former smoker of five-pack years. She underwent a right and left heart catheterization at an outside facility 2 months prior to her presentation for evaluation of dyspnea that showed minimal coronary artery disease but elevated pulmonary artery pressures of 55/24 mm Hg. The procedure itself w
