Hydrostatic reduction of intussusception has, until recently, been accomplished using barium sulfate or meglumine sodium diatrizoate, and has resulted in successful reduction in up to 80% of cases. Pneumatic reduction is an old technique but is new to North America. In a short time, air reduction has been widely accepted. It is easy to perform and success is achieved in over 80% of cases. The advantages of pneumatic reduction include a lower cost, shorter procedure time, and if perforation occurs, there is a lower likelihood of fecal contamination of the peritoneum. The vein of Galen malformat

Interventional radiology has important applications for the diagnosis and treatment of chest diseases. Fine-needle aspiration biopsy of a lung nodule is the most commonly performed procedure, but lesions in the mediastinum, hilum, and chest wall also can be biopsied. Needle aspiration and catheter drainage help in treating pleural effusion, empyema, and lung and mediastinal abscesses. Emphasis on teamwork and patient care is essential. In this article, the authors offer practical advice on how and when to undertake these procedures and how to manage any associated complications.

In the last several years, we have witnessed a steady growth in both the number and the complexity of interventional procedures in the pediatric population. This article presents our approach to interventional techniques in children of different ages, specifying our methods of sedation, patient monitoring, and selection or modification of equipment. We present our experience with nonvascular and vascular intervention. In the nonvascular group, we have focused on genitourinary and gastrointestinal procedures, outlining the techniques we have found to be safe and effective. Percutaneous aspirati

Several kinds of nonvascular interventional radiology of the urinary tract are reviewed. Transurethral balloon dilation of the prostate (TU-DP) is a newly developed nonsurgical treatment for benign prostate hyperplasia, which is performed under local anesthesia with minimal morbidity and requires no hospitalization. The TUDP technique involves dilating the prostatic urethra and bladder neck to 75 F for 10 minutes under fluoroscopic control using a high-pressure dilating balloon. Long-term follow-up studies are required. Percutaneous nephrolithotomy (PNL) and transurethral ureterolithotomy (TUL

Pediatric patients in the neurointerventional radiology setting pose the dual challenges of caring for relatively sick patients in the outfield environment. For safe and successful practice, the anesthesiologist must not only understand the nuances of pediatric anesthesia and the physiologic demands of the cerebral lesions. They must also help maintain a team-based approach to safe, compassionate care of the child in this challenging setting. In this review article, we summarize key aspects of success for several of these topics.

Ultrasound is a valuable imaging technique for musculoskeletal pathology. It is increasingly used for image-guided procedures such as aspiration of superficial or deep collections, injection of drugs, or biopsies. Ultrasound guidance is an interesting alternative to procedures performed either blindly or under fluoroscopic or CT guidance. Advantages of US-guided procedures include the absence of ionizing radiation, real-time monitoring during needle placement, decreased risk of injury to neighboring structures (namely vessels and nerves), real time confirmation of procedure success (injection,

A survey of therapeutic possibilities offered by pediatric interventional radiology is given. Authors emphasize that the percutaneous interventional radiology procedures are performed either as an adjuvant treatment facilitating other (surgical) methods or as definite therapy replacing traditional treatment modalities. It is also stressed that interventional procedures are cost effective and relative less expensive than surgery, and they are not so cumbersome to the patients and can shorten the time for hospitalization significantly. Authors present 5 illustrative cases including locoregional

Ultrasonography has become extremely useful in the evaluation of masses in the head and neck. It enables us to determine the anatomic location of the masses as well as the characteristics of the tissues that compose them, thus making it possible to orient the differential diagnosis toward inflammatory, neoplastic, congenital, traumatic, or vascular lesions, although it is necessary to use computed tomography or magnetic resonance imaging to determine the complete extension of certain lesions. The growing range of interventional procedures, mostly guided by ultrasonography, now includes biopsie

Three different techniques will be discussed. The first procedure is the biopsy of the lumbar vertebra or lumbar intervertebral disc for patients with tumors or infections of the lumbar spine. The different needles that can be used in function of consistency and location of the lesion will be shown. The transpedicular and posterolateral techniques will be described. Cementoplasty for tumors and selected patients with osteoporotic vertebral collapse generates much interest. The transpedicular and posterolateral techniques will be described. Percutaneous cholecystostomy is an established procedure for the management of patients with acute cholecystitis and with significant medical comorbidities that would make laparoscopic cholecystectomy excessively risky. In this review, we will explore the role of percutaneous cholecystostomy in the management of acute cholecystitis as well as other applications in the management of biliary pathology. The indications, grading, technical considerations, and postprocedure management in the setting of acute cholecystitis are discussed. In addition, we will discuss the potential role of percutaneou

There are many radiological interventions necessary for pediatric oncology patients, some of which may be covered in other articles in this publication. I will discuss a number of interventions including percutaneous biopsy for solid tumor and hematological malignancy diagnosis or recurrence, for the diagnosis of graft versus host disease after stem cell or bone marrow transplantation, and for the diagnosis of complications of immunosuppression such as invasive pulmonary aspergillosis. In the past, tumor localization techniques have been necessary to biopsy or resect small lesions. However imp

The past year has seen important papers on the risk of acquired immunodeficiency syndrome in the invasive radiology laboratory with emphasis on awareness and prevention. Interventional catheterization techniques in congenital heart disease are reviewed. Valvoplasty of pulmonic stenosis has the greatest success and least complication rate. The majority of complications appear to be at the vascular access site and are more common in neonates and in procedures that are ultimately unsuccessful. Intrauterine valvoplasty of the aortic valve is reviewed as is angioplasty for a recurrent coarctation o

Interventional radiologic techniques for the treatment of gallstone disease are becoming more widely available. Percutaneous cholecystostomy [corrected] can be lifesaving in critically ill elderly patients who have acute cholecystitis. Contact dissolution of stones with methyl tert-butyl ether and percutaneous extraction (with or without fragmentation) of calculi are treatment options in patients who are at high risk for surgery or who do not desire an operation. With increasing use of intracorporeal gallstone lithotripsy techniques in the gallbladder and bile ducts, combined fluoroscopic and

Despite the widespread use of interventional radiologic techniques, there has been reluctance to apply these to the spleen. Concern for bleeding and difficulty in negotiating around the colon and pleura have limited its use. The authors report their experience with interventional radiology of the spleen in 35 cases, including percutaneous biopsy (n = 5), diagnostic and therapeutic fluid aspiration (n = 14), and catheter drainage of abscesses (n = 9), hematomas (n = 2), intrasplenic pancreatic pseudocysts (n = 2), and necrotic tumor (n = 1). The development of radiographically controlled interventional procedures directed at neurologic disease can be attributed to improved fluoroscopic and catheterization equipment, new angiographic techniques, continued refinement of embolic materials, and the drive and determination of individual physicians over the past 25 years. In this article, the authors present a comprehensive review of the use of interventional radiology in neurology.

Pediatric Interventional Radiology (IR) is progressively expanding and gaining popularity as it provides minimally invasive procedures and treatments, with very good outcomes and low adverse event rates. The image-guided body procedures include central venous access, gastrointestinal procedures, such as gastrostomy tubes, biopsies of solid organs and tumors, pleural and abdominal drain placements, endovascular procedures, and embolizations. Pediatric patients who require IR are often medically complex and have significant pathophysiological derangements. Pediatric IR intersects with multiple s

The accuracy of the robotic device not only relies on a reproducible needle advancement, but also on the possibility to correct target movement at chosen checkpoints and to deviate from a linear to a nonlinear trajectory. We report our experience in using the robotic device for the insertion of trocar needles in CT guided procedures. The majority of procedures were targeted organ biopsies in the chest abdomen or pelvis. The accuracy of needle placement after target adjustments did not significantly differ from those patients where a linear trajectory could be used. The steering capabilities of

Traumatic injuries to the kidney and collecting system can range widely from small lacerations to significant bleeding and its sequelae. Urinary obstruction can occur in the renal pelvis, ureters, or urethra. Interventional radiology plays a significant role in treatment and management, in many cases requiring emergent action. Endovascular embolization is frequently the first-line approach to treating hemorrhage. Percutaneous interventions for urinary obstruction include nephrostomy and suprapubic catheter placement. In this article, we outline the clinical approach and interventional methods

UNLABELLED: Postpartum hemorrhage (PPH) is one of the common causes of morbidity as well as mortality among pregnant women. Obstetric hemorrhage embolization (OHE)/uterine artery embolization (UAE) is the preferred treatment for PPH which has failed medical therapy. In cases of placental accreta spectrum (PAS), balloon catheter can be prophylactically placed in internal iliac arteries (IIAs) bilaterally before delivery to enable postpartum control of bleeding. An inferior vena cava (IVC) filter can be placed under fluoroscopy for a pregnant woman with deep vein thrombosis (DVT) for whom antico
